100% agree with you. It it satisfying to see a clean ear canal at the end. Sometimes he can’t clean out the ear canal completely for various reasons (too painful for example) and I catch myself thinking “Man, I wish he could clean it up all the way”. Then I have to remind myself, the main reason he does what he does, is to help his patients/clients and NOT for our entertainment. We are lucky he takes the time making these videos to share his work on KZitem for us weirdos (I say this with lots of love) who enjoy watching them.

People are impatient. They want immediate results, and they're forgetting that you aren't making these videos solely for their own individual entertainment; you're a medical professional doing your job, and the videos are just a part that comes with it. You keep on doing what you do best, and let the armchair audiologists cry that you aren't giving them the time of day.

Do people normally criticise him in the comments?? It's never occurred to me to judge his use of instruments, he's the professional lol. We're simply seeing the reality of the job, and the difficulty of cases like this lady's left ear shows why practitioners like yourself and Rhys Barber get a number of patients on whom other practitioners have given up. Mr Neel, I just had hip replacement surgery. I can only imagine my surgeons reaction if I watched a video on KZitem & then proceeded to tell him how I think he should preform the surgery & what instruments would work best on me. I would, I’m sure, still have my old hip!! He went to medical school & i happily went to sleep! I know as a retired educator I would not have been ok with untrained individuals coming to my classroom offering unsound advise. I do have to admit, in my mind the Jobson Horn could clear any blockage, but my realistic part knows that’s ridiculous & I would never be so pretentious as to tell you which instruments you should use. There’s always going to be back seat audiologists!! 😷😷😷😷😷👍🏻💚

You pretty much said what I was thinking. When he said people questioned why he didn't use this instrument or that instrument I was amazed. Why would anyone be foolish enough that they would think that based on their having watched some videos on KZitem they are now masters and qualified to give advice to a man who went to school for who knows how many years to learn how to do what he is doing. I am watching this video as a form of entertainment. I do not think of this as a tutorial that will qualify me to start working on patients sometime in the near future.

I know what your saying ive had 20 back surgeries and i can only imagine my surgeon in his position having ppl tell him how to operate on me when hes been operating for longer the most of these ppl been alive. He was the one who went to mesical school. Unless you have a medical degree dont tell these doctors how to care for their patients. They know why they arent using certain tools.

I never hated or get bored from seeing the left overs in the ear canal because I like too see every last bit of skin and serum get removed and have a perfectly cleaned ear so that way no wax can be formed and prevent more damage. And less dry skin since the skin won’t be so irritating once everything is removed

I’d be interested in an explanation of after care for when the ear canal is so degraded. I’d imagine now that the keratin plug is gone, she may feel more sensations of itching, dampness, even potentially cold as we’re in colder months as the skin is trying to repair itself. I’d imagine some sort of earplug or cotton ball would be detrimental but at the same time isn’t there an increased bacteria concern? It looks like you took great care of her all things considered and I’m sure she felt incredible relief!

That's a good question. I get occasional infections in just one ear and in winter have to use ear muffs otherwise the ear pain in just that ear from the cold and wind is intense. I wonder if that is true for this patient as well.

In the past he used to take time to explain things to those criticisms but was usually met with more abuse and people telling him he is an awful audiologist and naming all the better ones (usually students of his!) So i think its only natural for him to be annoyed when people with no training tell him.how to perform his procedures

I watch a lot of ear care videos, including all of yours. I really appreciate your patience with and concern for your clients. I would never question your education, experience, or your use of tools during a procedure. I find watching ear cleaning (and skin care) videos relaxing - they reduce my stress. That said, I would not ever intentionally cause stress for a medical practitioner who is trying to educate the public AND relieve a client's discomfort. I used to suffer from the worst ear infections as a child, and still have some interest in the workings; these videos are really enlightening, and also oddly satisfying. It's nice to know this gross but cool practice is really helping someone. I hope she heals up nicely

Sometimes, when reading comments on KZitem, I wonder if the people making the snarky comments would be open to me and 112 of my closest friends coming to their workplace, standing outside and screaming, "You're doing it all wrong!!!!" even though none of us has any actual experience doing your job. When you started to clean her other ear after the first one, I thought this lady had to be practically deaf. She has to be hearing in high definition after that cleaning. That was insane. And that plug was so big and to think that half of it got sucked up when removing. Excellent job, thank goodness her friend found you and brought her. She has to feel 100x better.

I have been watching for about a year. This is 3 years before the year I started watching. Because I have learned so much in that time, I was watching from the standpoint of recognizing things all the way along. Yay! As for critical about the use of suction almost the whole time, that would be because you’re the most knowledgable and best at doing this. If you need this, Neel is the one you want in your ear. Don’t criticize. Watch, listen and learn. He’ll explain why.

I'd call that ear a hot mess or a train wreck. Definitely feel for the patient. Very interesting case. Great job sir.

Fascinating as always. You've inspired me to visit an audiologist myself. I've had pain issues with my left ear since an ear infection 12 years ago!

My ears are damaged due to me using swabs and Bobbie pins. My ear dr. Uses a small vacuum to clear my ear, dr Martin in Jonesboro Georgia.amazing ENT.

I'm not one of "those people," however I wonder if they made a tool like the ear hook but with a broader point to the hook almost like a spoon, if that would be more helpful for peeling the big mushy sheets like we see here. Back in HS chemistry, the "spatulas" had a small spoon tip that is kinda what I imagine here. Another idea is those wire loop blackhead extractors... I wonder if a tool like that would be helpful for this type of mushy gunk that gives way under the pressure when pulled on by a small point. IDK, I'm just an audience member with no expertise. I never question Neil's tool choice, I just have an active imagination. :)

I don't think there's a problem with wondering if there might be a more efficient way to design the tools. I've wondered myself why this over that, and if it's for a reason or simply no one questioned if there was a better way. It's the ones that instantly assume "I alone have ultimate knowledge" that are annoying! Besides, some of the biggest innovations have come from idle thought, a side comment to someone in the profession.

Rhys Barber uses something similar, though it's only used to safely get behind hardened debris to help push it out of the canal without relying on the suction tool.

Looks very difficult and frustrating. I feel for the patient. These videos can serve a vital purpose if you can incorporate solutions or preventative measures how to avoid/prevent this from happening to yourself.

I live in the US and not only are your videos interesting but your explanations with the video is informative. I would definitely come to you for an ear issue as you are a kind & gentle Audiologist. Oh my goodness you must of had sore arms after that. But to be honest with you doc, I really like watching the whole procedure. From being to end. That way you can assess how successful the procedure was. Furthermore you can then ask sensible questions,, even if they don't get answered.

Congratulations! I know it was frustrating for you as well as the patient, but you did absolutely everything you could to keep her from having to be put under general anesthesia. I don’t know the age of the patient but the older the patient the harder it is for general anesthesia to be on the patient. And for those who don’t know you’ve never dealt with an elderly patient in any type of procedure.

Oh my goodness, that has to be one of the most interesting cases 😯👂👂 The keratosis obturans was such a tough job but the tympanic membrane rupture was so amazing especially when she didn't even know about it 😲👂👂

Did she feel instant relief when the plug finally came out? Or because of the infection was she still experiencing a lot of pain? Did you do another course of antibiotics afterward for the infection, or follow up with more drops? I’m so fascinated! My first time watching a procedure like this and I’m really curious how she felt later!

I admire the work you do. It has given me the encouragement needed to have my own ears done. I've never had them done , but have decided to get a hearing test done first, as my family say I speak loudly, or rather shout. So thank you.
