The Phillips disaster was a devastating series of fire's on October 23, 1989, starting at the Phillips Petroleum Company's Houston Chemical Complex near the Houston Ship Channel in Pasadena, Texas. The initial blast registered 3.5 on the Richter scale, killing 23 employees and injured 314.
The horrific disaster is a fascinating lesson on why fail safe systems are needed.
