Just want to add a few things: 1) Capt of this flight was also part owner of the Co. The Co was kind of bankrupt and desperately needed some good PR and they got this football team to fly with them. 2) Capt had flown previously straight to Medellin without refuelling, the cargo was always lighter then, so they managed the insanity. 3) Declaring fuel emergency would be fined lots of money which LaMia did not afford at the time due to financial problems. 4) Capt was related to aviation authority people who granted LaMia license to fly despite them having troubles already.

@@cedricye1767 Do you mean the ATC at the arrival airport? Because she didn't do anything wrong. The pilots didn't declare an emergency, and it's not in her job description to be able to read minds. That she didn't give them immediate priority is on the pilots and their failure to declare an emergency or even pan pan. "Problems" could be anything and isn't standard phraseology. Or do you mean the ATC that approved a flight plan with a refueling stop that the pilots chose to skip? Nah, this is entirely on the pilots. They're the ones who are ultimately responsible for the safety of the flight. They're the ones who are going to die if the plane crashes. If the airline is pressuring them, they should say no. They have a responsibility to their passengers and the general public, not to mention themselves and the aviation industry as a whole. At every time they didn't take responsibility, made horrible choices and continued to make the situation worse by making more horrible choices. Even when they got an alarm that they had only 20 minutes worth of fuel left, they did not declare an emergency, and did not even use the term "minimum fuel". Based on their experience, this crew thought they could pull it off again BUT this time going the opposite direction from Santa Cruz to Medellín. But, there is a problem. Medellín elevation is 7,028 ft while Santa Cruz is 1,225 ft. The aircraft burnt less fuel on take off from Medellin because of its higher elevation. Taking off from Santa Cruz (and being lower elevation) they burnt more fuel on ascend, which they did not take into consideration. As a result, on this journey they ran out of the little extra fuel they had when they performed their (illegal) non stop flight from Medellín to Santa Cruz earlier.

Few additional details 4:14 - That's because the ACT tweaked the flight plan so they could skip a waypoint and save a LITTLE time with more direct approach to Medellin. 1. They had to lower their landing gear and well as deploy full flaps at 18,000 feet!! Which caused the plane to decrease altitude and impact the speed - very early. The reason they had to deploy full flaps and landing gear so early at that great speed/altitude is because once the engines fumed out (which was shortly thereafter) they could not work the landing gear and flaps. 2. This crew did the same route (against regulations) from Medellín to Santa Cruz on three other occasions and JUST made it and landed on fume - but they made it! Based on their experience, this crew thought they could pull it off again BUT this time going the opposite direction from Santa Cruz to Medellín. But, there is a problem. Latin male machismo was a strong factor in the cause of this crash, in my opinion. Oh, you think that's a racist comment? I've lived and worked in Puerto Rico, Brazil, and the Dominican Republic -- as an engineer/project manager for a large pharmaceutical corporation. From my experience, many Latino males in positions of authority, or even lesser positions, have a very tough time relinquishing their inherent belief in themselves as superior to others (like this pilot). I've watched them pay the price for this arrogant attitude on the job by making bone-headed decisions that bring adverse consequences and costs to project effort and people. This captain's decisions cost 71 people their lives. PS. This male "machismo" behavior is mirrored in other cultures too but is very strong in Latin cultures all over the globe.
@bobgreene2892
5 жыл бұрын
@@wtcashel -- Rather than a cultural or gender issue, the problem is insufficient screening and improper training. Many good pilots come from the same culture, but have been selected and trained properly. In addition, as another poster commented, the pilot may have had a financial interest in the aircraft, which, if true, explains why seemingly rash decisions were made to skip the time and cost of refueling. The fundamental explanation for the crash is poor regulation of regional airline industry, which is why many first-world airports refuse to allow substandard (literal "fly-by-night") airlines even to land. Lacking qualified personnel, the regional airlines regularly lose their brightest to more profitable markets in the prosperous countries. Also, a multitude of poorer countries host airliner "maintenance" facilities where all manner of critical maintenance procedures are performed by unskilled, uncertified technicians, given only "wink and nod" surveillance by some larger airlines which send their aircraft for maintenance at a fraction of the cost.

OMG !! I can't imagine what that pilot was thinking. Seems like they managed to breach all operating procedures. Basically, he thought he was a sly fox, saving 45 minutes by not stopping for fuel. But, when he started running dry, he didn't have the guts to tell ATC he had been cheating. Instead, he sat in the traffic, on dry tanks, and wasted the 7 minutes which would have allowed them to get safely past the threshold. He simply didn't dare to declare their emergency. Some other pilots have been declaring emergency just because they couldn't be bothered to hold...

In a continent crazy for football (soccer), there was a lot of rooting for Chapecoense at those finals of the South American Cup because they were the underdogs. A small team from a small city in Southern Brazil that made its way brightly to the finals. This added an extra bitterness to the tragedy. The other finalist, Atlético Nacional from Colombia, in the aftermath made a formal request to declare Chapecoense the South American Champion. It was granted and Fifa and also Brazilian authorities gave Fair Play condecorations to Atlético Nacional.

Another magnificent production by TFC. There's accidents and criminal neglect. This incident was without question, the latter. just about every rule in the book was broken regarding sufficient fuel reserves for diversion etc. What idiotic moron decides that the aircraft can go out with just about enough fuel to complete the journey and have nothing to fall back on. The crew didn't help by not declaring a fuel emergency on initial contact with the approach controller, but even if they did, it would be questionable whether they would still have made it down. What a disgusting tale of woe brilliantly told by TFC

Also, when in such an emergency a pilot must *IGNORE* all requests to purposely reduce altitude and enter "best glide distance" configuration until the pilot *KNOWS FOR SURE* the airplane can reach the target airport. Seems like because he was embarrassed, he decided to kill everyone rather than behave rationally. Again... unless I missed something important.

That was certainly the primary cause, but the airline and air traffic control system were also at fault for not having better controls in place to ensure flights are following established regulations. A good system is built to prevent one person's mistake from causing a crash.

They were flying above the calculated range altitude's ceiling. As a result, the Pilots surely "bet" they would conserve more than enough fuel from that. Does not appear to be a CVR (or it was destroyed) as we only seen / heard ATC conversations and the Pilot. The Pilots must have discussed the fuel gauge readings BEFORE the engines stalled. I believe reducing all engines to idle speed is a viable option. Yes it will lose altitude quickly but they needed a rapid decent anyway. Gear down consumes fuel so wait late as possible. And an absolute direct vector to waypoint / runway. But failing to do this + not being crystal clear on being super low on fuel spelled the death knell ! I was once a passenger on a rental Bonanza flown by my student pilot friend. We were unexpectedly VFR into IMC, lost "on top of clouds". My buddy was reluctant to call for help. But with 1/4 tank fuel or lesser remaining I implored him to swallow his pride & make the call, as we're going to be on the ground in 45 min safe or scattered all in pieces. Praise the Lord he made the call & ATC-FS talked us in straightaway, landing on fumes. So sorry to see this La Mia flight not end safely as our "near disaster" did ! RIP 71 Souls - Vaya Con Dios !

David A you make a good point - reducing engines to idle speed, except that jet airliners, unlike piston-engine aircraft, immediately and drastically lose altitude at idle. The scenario you described presumes perfect flying conditions, zero air traffic in area and flight crew capable of adding power at EXACTLY the right time, considering that airliners add power when rounding out, unlike small aircraft which are essentially just glided onto the ground.

Mike Patton , yes,,they didNOT DECLARE AN EMERGENCY, just we’re getting low on fuel, but here is something you would not know, the fuel gauges on those early jets were AWFUL, , you could NOT RELY ON THEM, THATS WHAT the flight engineer also did calculate fuel, B707 B720, Dc9, DC 8, today’s car fuel system is FAR MORE ACCURATE from Maplewood nj
@scotty3034
5 жыл бұрын
Leighton Samms That's sort of a myth about that aircraft. The fuel indicators are fine. The problem was several major things, the airline overloaded the aircraft by 1000lbs and didn't adjust the fuel calculations to get to an alternate airport and the captain purposely went far beyond the minimum reserve fuel threshold which is usually about 40 minutes. Most importantly, they did not declare an emergency until it was too late. And you don't time out to full empty, you time out to when MRF starts.

Not only that, but two of them trekked through the Andes without any climbing gear, and got help to rescue the others. It was an incredible story of survival, and was portrayed very well in a movie called "Alive", which is definitely worth watching.
Thank’s for sharing. Remember when it happened and watching games right after crash. All the teams came together to show respect for loss of the team . The airline failed in so many ways that upper management should spend time in jail.

Atlético Nacional gave the 2016 Sudamericana title to Chapecoense after the tragedy. It's by far the biggest "glory" of the club's history. They were a small and very young club who stormed like a flash through Brazilian lower divisions in the early 2010's. At the time of the accident, they were sustaining their spot in Série A for 3 years. They were eventually relegated to Série B in 2019, but now hold 2nd place in the championship, almost locked up for a return to Brazilian's top football division. The lives are lost forever, but the legacy of those players still lives on.

As a professional mariner, a pilot, and a SCUBA instructor that has lived and worked in many countries, I have seen this mindset a lot. In Latin America it is known as machismo. But certainly not limited to Latin American men. It is a cultural pride that many men ( but certainly not all ) have. It can result in good accomplishments. It can also result in death, and tends to be dangerous. I lost good friends that were on a commuter flight in rural Panama. They flew the airplane directly into a mountain side, flying into IMC that they were ill equipped to fly in. I have seen dive masters lead divers into hazardous conditions clearly “out of bounds “. I have rescued fisherman far at sea that were sure that they would be fine, until their luck ran out. So unnecessary for there 71 people to have died this way. The flight crew must have been terrified when the realized what they had done. Wow, RIP

This video here adds quite a bit of data that was not included in another video, of the same crash. It puts a different light on things, knowing that the flight had the opportunity to refuel in Bogota, and bypassed it. Seems that was the major mistake. Such loss of life, over such a decision of convenience, and such an entirely preventable tragedy. Like another poster here (Larry Be) noted, daring has no place in flying an airplane. Even arriving one entire week later than estimated arrival (let alone one hour due to a refueling stop) is much, much better than never arriving at all. Passengers complain about delays, but they may not realize how happy they should be to have responsible teams at the airlines and airports.

Captain was a reckless idiot, RIP to all those on board. What I'm also wondering though, why in the heck were they operating that particular aircraft with a very short 1,600 mile fuel range??? That would be a pretty typical route to Medelline, a city of 2.5 million. I don't see how they'd make $$$$ if most of the places they fly to you have to do a fuel stop. I'm also baffled why the team didn't fly directly from San Paulo to Medelline. Guessing money was tight or something.
@cr10001
5 жыл бұрын
The RJ85 / BAe146 was designed for short hops into secondary airports (RJ = 'Regional Jet'). Not for long-haul. You don't make money if you have fuel capacity for thousands of miles and most of the places you fly to are just hundreds of miles away. Lamia's intended business was domestic Bolivian charter flights. Much easier to fill a RJ85 than a 737. Any aircraft with the range to do San Paulo to Medelline direct would (I'm guessing here) have been much bigger and hence more expensive for the team to hire.
@aaronbays4
5 жыл бұрын
@@cr10001 You look at a map of South America, major population centers in Columbia and Venzeula are going to require a refuel stop with the RJ85. Flying anywhere east of Rio in Brazil is going to require refueling. Refueling costs a HUGE amount of $$ in terms of fuel burn, gotta take off and climb to altitude again, plus you might get stuck circling waiting to land for a 2nd time. Something like the Embraer E190 or Airbus A319 has a much longer range and similar seating capacity and cheaper maintence costs due to it being two engines vs 4
