Antiplatelet drugs act at various steps in platelet plug formation. Aspirin inhibits cycloxygenase-1 and reduces the synthesis of TxA2. And thus prevents platelet aggregation. It should be used in low dose because high dose inhibits cyclooxygenase in endothelium also which reduces synthesis of PGI2. It should be avoided with reversible COX-1 inhibitors. Next inhibitors of P2Y12. Ticlopidine, clopidogrel and prasugrel are prodrugs and their active metabolites inhibit P2Y12. Ticlopidine is associated with sever hematologic adverse events, clopidogrel has issues with activation by CYP2C19. Due to genetic polymorphism, some patients may not activate clopidogrel, so it won’t work. Prasugrel has high risk of bleeding. Ticagrelor and cangrelor are active by themselves and inhibit the P2Y12 reversibly. For protease activated receptor we have vorapaxar. Dipyridamole and Cilostazol inhibit phosphodiesterase and keep the level of cAMP high in platelets. Abciximab, eptifibatide and tirofiban inhibit Gp IIb/IIIa. These were the mechanism of action of antiplatelet drugs. They are used in unstable angina, myocardial infection, cerebrovascular disease, prosthetic heart valves, arteriovenous shunts, venous thromboembolism, peripheral vascular disease, percutaneous coronary intervention, coronary bypass graft implants and intermittent claudication. Adverse event is bleeding.
