Ear phones are like ear plug which goes into your ear canals. Ear phones are universally designed, that are not custom made. So there can be a possibility of an ill-fitting earphone. So it can cause local trauma, or infections of the ear canal if it is ill-fitting. Ear phones tend to plug the entire ear canal to take off external noise and if you are listening to music at very high volumes, you have a lot of sound being presented to your ear drum and that over a long period of time. If you are listening to music at very high volumes it can cause trauma to the inner ear leading to hearing loss. Third is ease at which we can carry the ear phones everywhere, you can just put them into your pocket. Whereas the headphones are very cumbersome to carry. It is not as easy to carry as it is portable until and unless it is on your head. But it is softer on the ear. It tends to cover the entire pinna. So the whole outer ear tends to be covered. The amount of noise it actually cots off from outside is better with that and it can protect your ears from external source of noise. But with respect to listening loud music, whether it is ear phone or a head phone, if you are listening to music at the same decibels, in either of the instruments. If it is loud and constant in addition long duration of time, it will cause inner ear damage and you can have a permanent irreversible hearing loss if you listen to music very loudly.
