A routine training flight from March Air Force Base in Riverside County went tragically wrong on January 12, 1961 with asymmetrical fuel loading of the wing tanks being suspected as the probable cause for the loss of aircraft control shortly after take-off. When efforts to resolve the control problem failed Capt. Wray ordered the crew to eject/bailout.
The pilot, co-pilot, and navigator all safely departed the aircraft by
ejection seat. The instructor navigator had to bail out manually, and during his egress Lt. Col. Irwin I. Williams struck the aircraft and was killed.
