Empower your patients! Additional education is available for your patients as they work to self-manage their chronic conditions. This is a no-cost opportunity that is available in many communities that bring patients together that are living with chronic conditions. Learn from Jill Renken, MPH, CHES, from the Wisconsin Institute for Healthy Aging, as she reviews evidence-based programming. This presentation will also provide resources about where to find the program in Michigan and Minnesota in addition to learning about how to enhance your billable services.
