I followed your advice; purchased a set of Tread Labs Pace insoles. Their website suggests one step onto a dry surface with wet feet, step away, and then look at the imprint to determine arch height. According to that method, my arches are high, so I ordered accordingly. I also ordered a pair of metatarsal pads (medium) and, by testing and adjusting, put it where it's taking the pressure off my metatarsal heads and helping to restore the transverse arch. Takes some getting used to, but after three days, I no longer notice the pads, and my bunched-socks feeling has completely been eliminated. Thanks for your clear, thorough explanation and suggestions.

What about pairing the Tread labs insert with a Xero type (wide toe box) shoe? Sounds like the wide toe box helps alleviate metatarsalagia but those shoes have very little cushion which can help flare up metatarsal pain. I don't want to wear big cushiony shoes that people recommend but are not a long term solution. Thanks for your video.

Does it make a difference whether you get a small or large or are they very similar in size and thickness?

What if I only have this pain directly under my big toe metatarsal? Could this still be metatarsalgia?

could be sesamoiditis! i just had surgery for my sesamoids in one foot, and possibly will need another surgery on my other foot. go to a podiatrist and get some proper fitted insoles for YOUR feet
