Indeed it doesn't. But the bridge pickup can share the middle tone control if you connect two lugs on the switch, the free one and the one that goes to the middle tone control.

Hi.Thanks for all the great info and diagrams! I have a H-S-S configuration, that Id like to use with individual on/off switches.AnyHelp there?? thanks!!

Thank you! The simplest one pole on-on switch will do; use on for each pickup so that it either does or does not interrupt the hot lead. Three connectors in a row: one goes nowhere, the middle one goes to the output (volume/tone) and the last one goes to your pickup.

Yes, it is. Reversed winding alone would put it out of phase with the other pickups; reversed polarity puts it back into phase.
