It's a rehearsal, not practice. When someone turns up having not practiced, just move on to the next song, don't allow them to practice in rehearsal, otherwise they will never stop.

a band plays a show ,after the set backstage the singer says "good show guys, but something didn't sound right"...,the drummer raises his head and says "I fumbled a couple of times"...,the singer replies..."no...it was definitely something musical !!!".

You hit my two pet peeves. Cell phones and friends/girlfriends/boyfriends. Those and completely derail rehearsal and focus. Another one, you kind of touched on but in a few bands I've been in, the vocalist works on the lyrics later and doesn't contribute while we work on songs. Even if it's not your part, give feedback, stand at the mic like you would be singing if the lyrics aren't ready.

Well that's tip #1 straight out of the window. I would NEVER leanr a cover song by listening to the record; better still, not hear it at all. This way I would stamp my own approach on doing someone else's songs. I couldn't just slavishly learn something and play it the same way every night.
