Wow amazing video. The patient appears to have receptive aphasia (Wernicke's Aphasia), a neurological condition in which the patient has difficulty understanding speech but retains fluent syntactical speech. It results from damage to the particular part of the brain responsible for language comprehension. Most frequently from stroke but there are other causes, like alcoholism for example.

So he has a form of word asplasia (sp?). He hears her and he processes it all just like you or I. But then it gets mixed when he tries to output. If you ever did any really strong psychoatives you might have experienced a similar sensation. Someone said to give him art supplies and that is probably a good idea if it's so bad that they can't get any ideas or intentions across to you. This guy however: he was actively trying to communicate around his jumble. He seemed aware of it and a steady interaction of almost any professional should show improvement. Short form communication worked and he was able to do calculation so it's just not as jumbled inside him as it is when it becomes words.

PTSD from the Vietnam War, which ended just three years before this was filmed. He's being filmed here in a military veterans' hospital. That war, and front-line action in any war messes up a lot of men's brains permanently. They snap because of the extremely hellish experience. Tragic.
