LISTEN WITH HEADPHONES. Recording made with a Thinklabs One Digital Stethoscope. Bicuspid aortic valve, recorded at the apex, without significant stenosis or regurgitation. There is an extra sound early in systole that sound like a "split" S1, but it does not vary with respiration as would S1 splitting, and is heard well only at the apex. This apical ejection sound is very suggestive of a bicuspid valve, and may be the only clue on physical examination when the valve is not regurgitant or stenotic. Occasionally even an echo has difficulty distinguishing bicuspid and tri-cuspid aortic valves.
