Great video, thanks for education everyone on these important topics for patient care. One thing I noticed in this video minute 8:14 describes that 1 unit of platelets(6pack) volume is only 50 mLs... when to my knowledge a 6 pack should have around 200-300 mLs. Thanks for any feedback.

I used PCC once for eliquis reversal in a patient with a massive GIB. I’m surprised it’s not used more often. Is that just because it’s expensive?

I always learn a new tidbit from your videos! For platelets, since we do not need a pump for that transfusion, are we still required to use blood tubing or just a regular primary tubing? Also, I never knew that some hospitals required a blood transfusion consent for albumin! 🤯

Awesome! I always try to think of some things to include that may be helpful so really glad to hear this. For platelets, though we still do need to blood set for the filter. And yeah, it is weird. Only one hospital I worked out did this, large system. So weird. Literally every other place has been just like any other fluid/med.

I was referring to a 6 pack, which is typically what is given. The numbers vary based on source, but increase is close to 50,000 for the whole 6 pack.

I also have seen both. We are not supposed to use platelets in the rapid transfuser, but I thought iv pumps were ok.

Honestly I've heard both pump is ok and not ok. Definitely no for rapid transfuser. I've always just defaulted to free hanging as there shouldn't be an issue.

Yeah, TXA can certainly have a benefit in trauma. As for which is more beneficial, it really depends what is happening. So I been wondering in the context of trauma what's the point of giving plasma, PRBC's ect when you can give fresh whole blood with a combination of all of these? Doesn't whole blood just contain everything your body normally needs to clot and keep your body alive?

Really a great question. In fact, the military will often use whole blood, as well as some places do in trauma. In cases of massive bleeding, we are essentially giving whole blood in parts. I think it's just used in far less situations and has a greater risk of being wasted, where as the different parts are used all the time. That's my best guess.

It technically is in massive hemorrhage, but for most people we a transfusing based on a deficiency in just hgb and not a need to replace platelets or plasma necessarily. Therefore we can do that just with the PRBCs and have the other products for people who need those specifically.

Checked out the quiz on your website, a good idea to reinforce learning. But I was disappointed that name and email address are being harvested prior to being able to see quiz results. This makes it seem like your motivations are a lot less about teaching and a lot more about something else. I hope you transition to a less coercive and more opt-in practice in the future.

I’m sorry you feel this way. I do think it’s very reasonable to ask for this info. I put a lot of time and energy into this channel and the hope is that as I build out more in the future that I’d like to communicate with those who have interacted with my content. I could very well put everything up behind a paywall like others, but that’s not my intention. I also do giveaways for those who complete the quizzes so I would be unable to contact someone. As for the placement, the software that comes with the platform I use doesn’t allow me to change how it asks for someone’s email, so I can’t have it ask upfront as I would have preferred.
