Had the laser surgery to remove prostate in July 2015. My psa was 7.5 (had doubled) and gleason score was 3+4 . They said cancer had not spread out of prostrate. But 18 months later psa started to rise again and subsqently had 35 radiation treatments. PSA score went to zero. But now constant pain in hip. Will get psa score this week and see Dr. According to the video I may now be a candidate for bone hip metastases . Hate this cancer crap it never leaves u alone. At least the videos give me information and hope. I am determined to get psmapetscan to learn my options.

I had a Robot Assisted Prostatectomy April 28th, 2020. I had Gleason 9, Grade cancer, but only had a PSA of 8.4 at the time of the surgery. They did not check my PSA for 12 weeks after surgery because of a potential false positive, but when the finally did, my PSA was 52+. Thinking something may be wrong we did another test a week later and it was over 67, two weeks after that it was over 165 in August of that year. I was immediately transferred to oncology care in lieu of my urologist. They put me on hormone therapy, Lupron and Erleada and by March of 2021 it was down to 0.20. However since then it has been rising is now back to just under 12 in about a year. My doctor seems to be reluctant to do imaging with the PSMA-PET scan, but to me this is going to be the only way to really get to why my PSA is exploding. According to bone scans and CT Scans in the first part of January, everything is stable and some locations have been resolved. Am I right to demand the PSMA scan? And they have told me that they have one.

Late reply here, but according to the information from Dr. Kwon and these educational videos a PET PSMA should be repeated every 3 months if I understood correctly. PSA every 4-6 weeks. Your Dr. should know that PET PSMA is superior in finding metastatic lesions (the ones smaller in size). I hope you insisted on it. Have you ever had one?

Diagnosed in March 2022 with Gleason 4+4 after biopsy, PSA was 8.12. After being referred to Radiology Oncologist treatment recommended was IMRT (28 treatments) and 18 months of hormone therapy composed of Casodex (daily) and Lupron (every 3mos.) Second opinion at Stanford concurred. In April, one Mets sclerotic area on left hip ilium after CT Scan. Bone biopsy confirmed positive. Treating doctor wants to include 6 months Chemotherapy because of the bone mets. Stanford says since only one area on bone mets, it can be radiated and that the chemotherapy wouldn’t be necessary. I’m inclined to go with Stanford doctor recommendation.

First of all, appreciate the details as you are not alone in Stage 4 at diagnosis. Have you gone ahead with that treatment plan (minus chemo?) and, if so, what portions of it are simultaneous. And how is it going? From what I’ve heard, hitting it early and hard is the way to go and it sounds like that is the plan you and your docs have in mind.

All very interesting. The question then is, what exactly causes so many to die from this each year? It seems there would be both a primary and secondary cause of death, e.g., liver failure secondary to Pca, etc. I'd like to know the breakdown. I enjoy your videos !

Why is the FDA not fast-tracking the approval of Lutetium? It is a sad statement about health care in the United States when our patients are forced to travel to other countries to receive this potentially life extending treatment, and must pay all expenses (which can be astronomical) out-of-pocket. Nearly all of the patients who do this have run out of other treatment options.

Hi, very informative video. My father is suffering from prostrate cancer, metastasized, spread to lungs as well according to reports. My fathers PSA is 5.369. For somedays blood from throat and urine was there. They performed CT Scan, lungs and prostrate biopsy. Gleason score is 4+3= 7 and grade group is 3. Dr. recommended hormonal therapy and to go under chemotherapy. Patient has given Degarelix 120 injection and blood from throat and urine has stopped. We are very fearful to go under chemotherapy since I had seen some known persons who collapsed during chemotherapy. Any suggestion for me shall be highly appreciable. Thank you

65 Year old father 226PSA and gleascon score 9 after biopsy. Bone scan and CT scan came back clear except for one 10mm right external illiac lymph node involvement. All bones and organs were clear from these scans on SEP 3rd 2022. Nov 10 2022 he started 240mg of firmagon. Since the bone scan and CT Scan are two months old they are having him redo these two scans.. can I ask you what the odds are that this has spread further over the 2 months from the September scans to the Nov 10th firmagon treatment? Oncologist is also telling his he will likely have a 5 year life expectancy if any further spread.Do we accept this or do we have treatment options that could give him better odds at a 10+ year lifespan? Asking so I know what to ask when I go with him to the next appointment on Dec 9th

I wish Dr Scholz would stop referring to regular PSA testing as the 'proper thing'. Such terminology places the blame on patients who are under enough stress as it is. Many men who are diagnosed with aggressive, metastatic prostate cancer are under the age of 50. In Australia men without family history are not encouraged to have regular PSA tests until they turn 50. My husband was diagnosed with stage 4 prostate cancer at 45 years old. We were completely blindsided by the diagnosis, as was our GP and oncologist. The way language is used is important, particularly when you are talking to a vulnerable and distressed population.
