Hear from Care Coordinators George Harris and Emma Smart who explain what they do for the residents of Nottinghamshire and Nottingham. They can help by:
• preparing you for upcoming conversations about your health and care.
• monitoring your needs and responding to any changes.
• supporting you to understand and manage your care.
New services from General Practice Teams
These are just two of over 400 healthcare professionals recruited for local general practice surgeries since April 2019, working alongside GPs, Nurses and Healthcare Assistants. It’s part of the NHS Long Term Plan to personalise care and provide the right services, at the right time, in the right place.
Having a range of professionals at your general practice means you receive the most appropriate care as quickly as possible. You can often see these specialists straight away. For an appointment, just check your Practice website or discuss your needs with Practice staff.
