My mothers doctor gave her this test. I almost cried when I saw that she had drawn all of the numbers on one side of the clock!

I was working with an elderly woman in a nursing facility who had difficulty drawing the lines so I instead asked her to point on a circle where 12, 3, 6, and 9 would be located and followed up on that with having her place a finger on the clock face where the big and little hand would be for two different times. She had no difficulty whatsoever pointing to the correct locations. Because she was elderly, had a thick accent that was difficult to understand, and she resided in the dementia ward someone might quickly assume she failed this test because she couldn’t draw a line. She just had tired hands and some arthritis in her fingers.

There are as well many elderly that because they do not hear (or see) very well therefore misundestands questions, or the person who is talking to them do not wait enough long for the elderly to think about what to answer so they consider them having dementia. I even met an elderly person who they said had with severe dementia problems who, when you patiently waited while the person was thinking, could answer the questions perfectly. Where is the clock test for those who performs the tests?

My mother did this test. She did a good job drawing the round circle, and placed the numbers around the circle pretty well. Then to draw the hands to depict 10 to 11, she drew one hand to the ten, one hand to the 2, and one hand to the 11.

Lol it shows she still has a great sense of humor. None of our young people could do it. Give that in schools. They rely on digital and phones for that. Lol

My best friend was showing definite signs of dementia, so I asked her to draw a clock face. All her numbers were in the top right quadrant, and went 5, 10, 15, 20, 25, 30.

Just saw this test done by a character of a tv show I watch often...The elder woman drew the entire clock....The only issue was that she placed all the numbers onto the clock but she placed them all in the lower left (Where the 7 would be on a "typical widely used" clock.)....She placed the hands correctly (At 8:00)...It was so sad...The woman's adult children had to fight the words to come out of their mouths that "Mom just might have Alzheimer's."

This test assumes that the patient knows what a clock is and how the numbers are laid out. What if you are testing a tribal Indian from South America ?
@DrODonovan

Thansk for the comment - you are right that this test can only be used for someone who has prior knowledge of how to do this. I am not aware of how you would test someone who could not draw a clock or tell the time, but it would have to be something that was socio-culturally acceptable and that challenged the person in terms of spatial dysfunction and neglect (which is what the clock test tests for)

Ashley Bendiksen what I was wondering! It may have been a joke on your part but I’m 17 (have been to what are generally considered ‘good’ schools) and I’m unsure about the placement of the hands. I rely on my phone so it’s just not necessary.
