I wonder if he's actually the ghost of Libby's first husband called David.

This episode really highlights both mental health and insecurity, as well as how they can both feed on each other. He wasn't real, but the words he said were. These were all just thoughts Hurley was too afraid to express to himself. He struggled with sanity. He got rich, but still couldn't be happy. He felt cursed. He felt like a burden. Now hes on an island where he feels he carries less value as a person. So his insecurities manifested themselves into what he feels is the last time he truly felt value. His brain struggled to find peace under the stress, so tried to reset itself in a mode where Hurley felt safe. And it almost killed him.

I find this episode really philosophical. None of this is real. Our reality is our subjective perspective and there are no coincidences. Life is a series of chance and mysterious plans for all of us that we’ll never actually be able to understand. Hurleys experience is enlightening even if it was all just a psychotic episode. Season 2 probably the best season
