bounce a flash off the ceiling, get two white foam core boards from walmart to bounce the bounced off the ceiling light to be the fill light that the other two lights are doing. Adjust how much fill they provide by angling them 45 degrees to straight up, and moving them closer. Turn them into silver reflectors if you want by covering them with aluminum foil. Same lighting with one light and maybe $12 extra if you don't have 3 lights. Low budget way but if you're trying to do it by yourself, you can run into difficulty in finding a way to get the foam core boards to stay in a good position. A little bigger budget, get one of those 5 in 1 reflectors from Adorama, which I'm thinking about getting. Use the silver and the white.

I was gonna say a small Lantern 65-70° above + silver reflector (neck) and white reflector (front)

Very well lit and beautiful shot. For the overhead light source - you used a reflector - I would use a large softbox on a boom arm ("junior" movie tripod on wheels and parallellogram boom with adjustable counter weight is a breeze to work with because when counterbalanced well, up/down is no effort and it stays where I put it without locking). Technical BS - it's about the effect and I like this shot a lot.

looks great ... but the forehead area is just a little too bright for my taste especially in comparison to the rest of the face and its lighting, so I would have reduced the main light coming from above minimally

would it fair to say top light has the highest power or is it just because of proximity ? its simple but challenging not to overpower the top light to wash out the forehead ! thanks for sharing
