You mentioned that it is better to just wash the face once a day. I'm planning to cleanse my face at night before using a retinol. Will it be okay to put other products without washing the retinol off in the morning?

hello doc may i ask? i am currently using adapalene 0.1% as my nighttime cream because its affordable and i used to have acne. But i am 25 now i dont have it anymore i want to switch to Retinoid, in ester + retinol form formulation. I want to now how we compare retinol efficacy? or what parameter we can use to know its efficacy to our skin?

Used retinol for almost 6 months, been using tretinoin 0.05 cream on my sensitive skin for 3 weeks now, no irritation whatsoever all thanks to DR. Dray.

Similar experience! Used Differin for a year. Switched over to Tretinoin 6 weeks ago, absolutely no irritation; peeling etc. even my own dermatologist is happy and shocked. Now I just enjoy the process. All Hail Andrea, MD PhD 💙💙💙

I just started tretinoin .5% every 3rd day & I use the sandwich method. I have not experienced irritation or any side effects, but I've only been on it a month. Next week I am bumping it up to every other day. I was told baby steps & so far it's working out great. It's a marathon- not a sprint. 😉

I have been using tretinoin 0.05% for over a year and will the peeling stop. My face is not red but I shed like a snake. I switched to curology about two months and still have peeling. Is this normal?

I've been on 0.025% for the past two years, and I'm still peeling! There's no way I will ever go up to the 0.05%!!! Some people with fair or more sensitive skin types don't tolerate it as well as some others.

I’ve been using adapaleno for about a year, and without a doubt, made my skin new,, and cured my acne. But the first 6 months weren’t pretty, a lot of flaky skin and dryness, but it was definitely worth it

I alternate from salicylic acid and benzoyl peroxide wash off in the morning then sunscreen and at night I use my moisturizer then differen adapaline!

I am so glad you addressed the gland dysfunction/dry eyes. You mentioned this is reversible, but I have read tens upon tens of personal stories on skincare forums of people who said that it was not reversible for them, which is a bit disconcerting. They tend to be quite passionate about begging people to reconsider using retinols because they have had such a devastating experience. Have you heard of this condition of gland damage being permanent?

Well, in all fairness...meibomian gland disfunction is a fairly common thing in contact lense wearers and people looking at screens for hours. It is also linked to autoimmune issues and inflammation in general - and the way most people eat, you can just go ahead and assume, they got a fair bit of that going on. Or in other words...correlation doesn't prove causation and I wouldn't be surprised, if a fair number of those people would have developed it anyway, even without ever using a retinol. I did, for example. It's so bad, I have my fitness tracker wake me up at both 3 and 5 am, to apply eyedrops and a salve, because otherwise, I'll wake up with my eyelids glued together and have very red and inflamed eyes all day long. Fun times. Of course, knowing how bad this disease affects quality of life, I wouldn't recommend using retinol on the eyes, if there's even a chance, it could cause this...so there's that.

Dry eye condition (diagnosed) is not reversible, but it can be treated using over the counter eye drops and other treatments, as recommended by an Ophthalmologist. My Ophthalmolgist told me it is not curable and does get worse with age. I was shocked when she told me I had dry eyes. That explained why my eyes were red, and it felt like I had sand in my eyes. Trust me, you don't want dry eyes! Using retinol around your eyes is NOT worth the risk. IF you develop dry eyes over time, the damage will be permanent. Personally, I'd rather have the wrinkles than dry eyes.

OMG Thank you so much for answering the Redermic question! I've been using the CeraVe Retinol per your recommendation but for some reason can't find it reasonably priced (when you include shipping) on Amazon anymore and as I live in Europe the La Roche Posay seemed like a decent available alternative. It does have fragrance but it's not at all strong and so far hasn't caused any issues for me. Please do more videos on European alternatives for your recommended retinols, as well as other types of products. And thank you for all your help!

Hi, thank you so much for your advise. I’m going to check out for retinol posts if not again, as I stared using neutrogena rapid wrinkle moisturiser and I’ve been feeling so dry and tight, I then layer moisturisers/Vaseline over the top but that uncomfortable dry feeling doesn’t go away. Not used to it at all, and I hope that in the long run I’m doing good rather then the reverse for my skin, in 42 and can’t make any mistakes from here 😅

Unfortunately, at least in my country in EU, there are very few of your recommendations available and they are usually very pricey. They aren't doctors but there are two channels that have reviewed this product. Neither channel takes sponsorships and buys the products with their own money, No BS Beauty and Aliceintherabbithole. No BS Beauty really likes Geek and Gorgeous. I know Doctor Anne (germany) has reviewed some of their products as well but not sure if she's posted about the retinal yet.

The Retin-A micro tretinoin I use is prescribe by my doctor so there is no cosmetic name for it just the pharma who produce it. I use it once or twice a week with cerave cream on top of it. It seems to work for me

Perfect timing! I was planning to give something from the retinoid family another try soon. I’m so sensitive that no amount of buffering and spacing out helped in the past. But I’ve been treating my skin with azelaic acid and a have super simple routine, so here’s hoping my skin will cooperate this time. These tips are great reminders.

Yeah, that 'bad sunburn' look you get from retinol irritation is not very pretty 😆. Have you tried something like the new(ish) Granactive Retinoids by The Ordinary? They're advertising them as being way less irritating than regular retinol formulations. Full disclosure: Dr.Dray did a video about them and said, there is a lack of studies proving their effect and one should just stick with the tried and true products...but well. Lots of people in the comments said, they were very happy using them and maybe it's worth a try for you. Better luck this time!

Now that you mention it, I think granactive retinoids (or even retinol esters) might be the better option until I get that sorted out.

I am super sensitive too. Just changing from one cetaphil or cerave product to another will cause me problems. But I just started using Maelove Moonlight Retinal .11% and so far so good with no irritation whatsoever. It does sting for a few minutes, but I don't even get red. I have never used retinal before and tried to use retinol with the neutrogena, but neutrogena retinol product causes me a rash.

I use this and my skin tolerates it better than straight up retinoid. And it’s a retinoid. But maybe because it also contains and exfoliant and gets the skin cells off.

@@double6ixes Well the doctorly video was concentrated on expensive skincare. I know Dr Alexis Stephens like's the skinbetter altodefense (antioxidant serum). She has said that even though she will try and recommend affordable products she does base her recommendations on the ingredients/formulas.

Thanks for answering all those questions. My dermatologist prescribed adapalene for aging, his reasoning was he thought I’d tolerate it better with my rosacea and affordability with my insurance. He also prescribed azelaic acid twice daily. He also referred me for pulsed dye laser (Vbeam) treatments. To quote him, « let’s put your money where you will get the most benefit for the next 3-5 years ».

May I ask if you've been prescribed 0.1 or 0.3? I know the latter has shown similar effects to tret 0.5 in a study. I'm using 0.1 but didn't see any effect on existing fine lines, so will try 0.3 soon.

@@MyLoVeLyMaN 0.1%. The main focus is my rosacea, but was told with time that I will see a reduction in fine line and wrinkles. Essentially I was told to wear sunscreen and be patient; skincare is a marathon not a sprint. Have a great day.

@@MyLoVeLyMaN Perhaps if my goal was to be treated specifically for signs of aging, then we would have had a more in-depth discussion. My goal is getting my rosacea better managed. He suggested the use of adapalene towards the goal of aging gracefully because I had been using a retinol before my rosacea got out of control. So, I decided that my investment will be in the pulse dyed laser, which is all out of pocket, instead of anti-aging. I hope you find what works for you.

Loving the Q & A's. Was hoping you'd consider my question for another time, concerning using a retinol, ( I'm using the Neutrogena Rapid Wrinkle Repair), along with at home red light therapy. I'm using the Omnilux mask. Getting mixed information on it. I've been using both so far with no negative reactions, and using them apart on alternating days.

Hi Dr. Dray!! I absolutely love the weekly Q & A's!! Well if I'm going to be honest, I love all of your videos.. You just make my day with your vlogs and awesome skincare videos! Thank you so much for the amazing content as always. I admire you, you're so intelligent and so no nonsense. I could say many more things but I don't wanna make you read too much. I did wanna say I switched to arazlo for my cystic acne after being on 0.5% tretinoin for a year. I had some results with the tretinoin but not so much with the cystic acne. I also use dapsone 7.5% gel in the morning. I just wanted to say that I am loving the arazlo. It's actually very moisturizing ( I believe it's the same concept as the altreeno lotion as in its supposed to be a more moisturizing lotion vehicle for less irritation I believe ) and I apply it after my cerave pm dries after applying to damp skin and I am seeing better acne control so far. ( I always double cleanse at night and apply moisturizer on damp skin, the one of many other things I do because of your recommendations 🤭) Do have any thoughts on arazlo in comparison to the original tazarotene cream? I'd love to know. Thank you Dr. Dray ♥️😁

Excelent video! I used the ritin A 0.05% for the last 28 years because I had a lot of acne when I was young, now I'm 48 and my face is very well. But the last year I used niacinamide+resverastrol and retin a 0.05% the same routine and I feel my skin more smooth than before, but I don't know if this combination is correct or if it's too much. I would I appreciate if you told me if my combination is right. Thank you very much!!!

I put my moisturizer on first and wait 30 minutes before putting on my tretinoin. I have sensitive skin and I have had a fairly easy time adjusting to my tretinoin. I use XeraCalm A.D. cream which you recommended in on of your videos. Love it ❤️. Ty

Can you please tell us the difference between the la roch posay cicaplast balm and the lipikar ap+ balm? I use the lipikar before I use my tretinoin .1% but I gues that means its also not penetrating since its a balm???

I tolerate retinol quite well and have been using retinol 1% on a daily basis. The thing is, this routine can be quite costly in the long run. How often should I tone back the use of retinol while still maintaining optimal benefit? Hope to have your reply Dr. Dray, cheers

I’m trying very very slowly to introduce a retinol. My skin can be very temperamental. My routine for the past 2+ years has been a double cleanse, hydroboost, Cerave and sunscreen. If I can’t get accustomed to the retinol I’ll just not use it. I’ve been extremely happy with what I currently use.

Wow didn't know retinol/ tretinoin wasn't safe druing pregancy! So what happens to the built up resilience during/ after pregnancy? Do you have to build usage up slowly again?

Take in mind that L'oreal and J&J test in animals, it is not vegan, not cruelty free... For someone to look for products that are vegan it is super super difficult.

If you mean the olay 24 she's okay with it. It's not one she recommends regularly but she says there's nothing wrong with it. Dr Sugai likes it texture-wise compared to the neutrogena one that Dr Dray tends to recommend.

I agree about the pregnancy because your skin natural stretches during pregnancy I don’t find I need it . But after the pregnancy you can’t use it while breastfeeding that kills me every time because my skin is in need after nine months

Thanks for the Q&A and all the videos you make. I’ve learned so much about skin. I have a question that wasn’t taken up here and perhaps for a future Q&A. I started using Differen about 20 days ago and I started off slowly plus I layered with used moisturizer under and have had no problems or dry skin or irritation! I’ve been very happy, and there’s not been any great change but skin feels different. The past ten days I’ve tried a putting it on without moisturizer a couple of times, being cautious. I don’t want to cause any irritation. However, 7 days ago I accidentally got Differen on my neck, I have been avoiding my neck. I wiped off the Differen immediately (but I didn’t wash it off with cleanser). The next day the part of my neck that got the Differen dose has been stingy and just some small red spots - not a raised rash. It’s not horrible, but quite annoying and now it’s been a week and it’s not getting worse but not better either. I’ve been putting Vaseline on my neck every night. Is there something else I should do? Should I stop using Differen on my face in case it’s perpetuating the irritation? I’m not sure I’ll ever opt to use Differen anywhere except my face after this experience.
