This video is amazing! You really did your research to find the correct person for this system. Many of the people giving online advice are totally wrong about Omnipod 5 and it’s algorithm. It’s not their fault they just have some majorly incorrect assumptions about this newer algorithm. Im glad you featured someone who really knows the facts. I will share this video with anyone I know starting OP5!

Omnipod's lack of app development progress is concerning. While they cite FDA regulations as the reason, it is difficult to believe that this is the sole factor. Samsung, for example, provides developers with ample time to develop and test their updates, demonstrating that large pharmaceutical companies can prioritize patient experience by taking advantage of this.

I’ve been using DIY Loop since 2019 with Omnipod and I absolutely love it. It’s not for everyone but the flexibility it has and the fact that I can use both Dexcom g7 and my iPhone to control it are HUGE advantages for me. I could get the O5 but definitely still prefer DIY Loop.

I was looping with the old Omnipod Eros. I had built my app and it worked with my iPhone and Apple Watch. My sugars were perfect and A1C was 7 or below with little effort. 5 months now I’m using the Omnipod 5 and A1c has gone way up to 7.8!! Compare your own a1cs and decide I suppose which system works better. Even if I leave my device pdm in the other room let’s say, and I just let it communicate by itself from dexcom g6 to omnipod5, it hardly brings down my sugars at all. Literally, my sugars are perfect only if I do not eat for 6 hours or more. Even when I wake up in the mornings, sugars are way high and nowhere near my target of 110.

No dexcom G7 integration or family follow app with the omnipod 5 is a deal breaker for us. We're sticking with diy loop until those two issues are resolved. Loop is amazing and literally changed our life for the better!

Originally listened to this on your podcast. Rewatching here on YT and will watch again at least a couple more times. Such amazing insightful information. Clear. Concise. Easy to understand even for newbies (I’m 3 months diagnosed T1 and on my first pump). I’d like to try omnipod 5 when it works with G7 and iPhone and this video with no doubt make that transition much more successful. I wish there was a chart somewhere that showed how each pump (including iLet) calculates basal and IOB. Thank you so much for putting out such invaluable content!!!

One of the biggest benefits I discovered when going from MDI to CGM and pump after being a diabetic for over 40 years was no longer needing to wake up at night to either eat carbs or go to the bathroom. A solid 6 to 8 hours of sleep every night with no stress cannot be understated. Mental burden is the number one thing people overlook when talking about diabetes. You are a diabetic 24/7 for the rest of your life. Any way to step back from thinking about that is worth its weight in gold.

Hi Justin, I follow your videos, I find them very interesting and well done. I currently use G6+DASH+iAPS and following a normal diet there is no need for any correction, everything happens automatically. However my big mental problem is not being dependent on the iPhone! Another essential thing is to get rid of the G6 which actually lasts 9 days, for Eversense. As for me today I would opt for Omnipod 5 + Eversense CGM if they were compatible.

Great video. My issue with Omnipod 5 is that I eat a very low carb diet. The Omnipod algorithm assumes a set percentage of your total daily insulin is basal. That assumption dramatically underestimates basal needs if one eats less than 30 carbs a day. My history would show a continuous string of maximum microboluses as the program unsuccessfully attempted to reign in my BSs. Because of the baked in algorithm which can not be adjusted it cannot learn its way into the correct bolus range. I have worked on this both with my diabetes educator and Omnipod reps playing with various variables to try to fool the pump to the correct bolus rate. I eventually abandoned Automatic mode and just run it in manual. A disappointment to be certain, but I have despised tubed pumps since the first Medtronic I got. We all make choices. :-)

I’ve been Looping since ~2017 and initially needed to use a Riley Link and Eros Omnipod. Now, I use a Dash Pod and my iPhone (along with Dexcom G7). I briefly tried Omnipod 5 and found the switching between menus time-consuming, the need to confirm every choice irritating, switching between automated mode and manual when changing pods or therapeutic settings, etc. The software just seemed cumbersome. I also found that the algorithm seemed to be built around the concept of keeping you from getting too low of a glucose. It wouldn’t aggressively attack high readings. I couldn’t wake up below 115 in the mornings. For me, I need to manage high readings rather than low reading most of the time. My A1C also went up from 6.2 to 6.9. The last two straws were that it didn’t work with my iPhone and it didn’t work with the new G7. Insulet was falling behind the times and I would’ve had to wait for App development (still waiting, right?) and device integration. Also, if carb ratios for the Omnipod 5 need to be lowered by 25%, did the total daily insulin amounts go up? Is that leading to weight gain? I personally don’t find that I need to manage or play with the dosing of the Loop app either. Just my 2 cents.

@@diabe_tech Yes, I’m back on Loop. I may give Omnipod 5 another try once they have an iOS app, but for now it’s been relegated to my backup system😉 I just watched your first video on iAPS. I hadn’t heard of this DIY app before. It has me intrigued and I want to learn more. I am a T1D (42 years and counting) but started taking Mounjaro 2 months ago, which delays digestion so I’m trying to figure out this new variable. 😉 iAPS may be a good match with that aspect. Keep up the good work! I enjoy your videos- they are presented clearly, have good topic matter and teach me about new tech and what’s around the corner. 👍🏻👏🏻

Great video, I used Loop for a year, really liked / used Johns Patches (Basal Lock), then switched to iAPS. I found iAPS to be less burden than Loop, as no need to edit and add false carbs for fat and protein rise. I'm based in UK and my daughter is 17 and also T1D. My daughter started Ominpod 5, five weeks ago and the initial results have been really good, transitioning from omnipod dash, my daughter didn't loop. I have now been offered Omnipod 5 and still debating on if to give it a try.

I think a HUGE positive for OP5 that doesn’t get mentioned enough is no basal testing! My basal changes from week to week. Sure I can basal test and figure it out but a month later it’ll be 25 to 30 percent higher or lower. I was always in a crappy mood with all that fasting before. Plus it felt like a lot of wasted brain energy that I now can spend on something else.

I’ve been using OP5 since 6/2023 & since then no night time lows, but still waiting on the G7 approval ! Also w/the Activity mode I can go out on a long run/walk & not have to be concerned w/lows, my A1C for has been pretty consistent of 5.6 to 5.9 for yrs. Have been wondering for those like me that got the G7’s in 2023 & finding they don’t work w/OP5, I’m understanding there’s a chip in the updated G7 & if Dexcom is going to switch out the old sensors for the updated version? Best Regards JG

One question I can't seem to see answered anywhere is what the Omnipod 5 is using to forecast 60 minutes into the future? I understand that the TDI for a given day is determined by the weighted rolling average of the previous days, and then TDI/48 is the insulin rate. But I am still a bit unsure what it does on the minute by minute level - ie. at a given time, what does it use to predict glucose in 1 hour? I assumed this had to be some kind of population model - eg. Insulin(t) = Insulin(0)e^{-at} and Glucose(t) is modeled by some ODE which incorporates insulin absorption like dGlucose/dt = - a Insulin(t). Or is Omnipod 5 doing something dumber to forecast glucose at 60 mins? And then is the decision to deliver at the given rate a binary decision based on "is above target"?
