Had someone die from a welding explosion at my employer(15 years before I started there). Vented the space before the job, but didn't constantly air monitor the entry. There was some rotting vegetable matter stuck under the floor plate, and walking on it squeezed methane into the space. The explosion broke free some overhead equipment that impaled the guy. Horrific way to die.

This reminded me of something that slipped my mind. I lived in a city named Victoria BC. This was in 2013 or 2014. I was awoken by a massive explosion that rocked the building and others near it. My first thought was "Great, it's the apocalypse and I'm not prepared". I made my way towards the explosion and found cops, firefighters and ambulances blocking nearly every street. I was able to get closer through the nearby park and boy oh boy. There was construction on the corner with a foundation and some nearby shipping containers. The containers were bent out of shape and one of the doors was in the middle of the park. All the buildings around the construction had their windows shattered. The container in the middle looked like someone blew it up like a balloon. I was able to talk up one of the cops in the park and found out that a propane leak was the cause. The first worker on site was preparing for the day with a smoke in hand. He went to open one of the containers (used as office space) and leaking propane ignited and sent the worker and one of the doors flying. The guy was insanely lucky. The foundation was cement and rebar but there were certain spots with plywood. He landed on the plywood and not the cement or rebar. He was hospitalized but it could have been so much worse. Surrounding buildings had glass shatter but there were very few injuries. A guy driving by in a truck was nearly flipped and had his windows shatter as well. It could have been so much worse over all. I think you can find some news articles on it if you search for Rutledge park explosion and add Victoria BC. I still have some of the pictures from that day. Scary stuff.

Huh, crazy, I didn't think a lit cigarette could light the gas like that, I thought it had to be an open flame or lighter. I guess it depends on the gas, different ones probably have different ignition points.

The gap in the between the agitator shaft and the tank makes this even more ridiculous because they had a way to slide a monitor wand into the tank to test the atmosphere inside.

Asbestos had been a known health hazard going back to the Roman Empire. It was recorded by at least one of the usual historians of the day (one of the Plinys, maybe?) that slaves working in asbestos mines sickened and died after a few years.

It was used not so much because it was dangerous, but because it took a long time to come up with alternatives (and it's still used because in certain applications there are still no alternatives) and ultimately it is used as a safety device. In fact the insistence on safety without fully considering the consequences is part of the reason why it was so widespread. People just hear "safe" and they stop asking questions. People don't want to actually be safe, they just want someone to tell them they are and have someone to blame if something goes wrong.

Regarding what you say about asbestos; it's still currently used in many laboratory applications and chemical manufacturing to this day. In many cases there is simply no better/safer option. It's extremely heat resistant and fire retardant

Very true asbestos is invaluable in industry so it's used in those applications. But not for home and recreation as safer alternatives were developed unfortunately no such thing exists yet well atlest affordable for labs and industrial facilities

@@shinrapresident7010 you know what kinda dangerous crap that is used in chemistry and industry? All kinds of radio active, carcinogenic, face melting etc chemicals. Even ones that are dangerous *and* expensive, unlike asbestos, make them prime candidates for replacement with safer options. I'm not trying to defend asbestos use specifically, but saying sometimes, a better option doesn't exist.

Any container being worked on or near with hot tools should be either filled with water or purged with a non-combustable gas after being vented. Never weld on a sealed container or trust someone else purged a container.

@@gadsdenjack no reason to think they wouldn't have sometimes made it to a jury. The csb is the giving us the summary of investigations into accidents. Some of these facts may have even come to light as a result of the following litigation.

My father worked in Texas City at a refinery. An employee needed to weld in a hole. The safety mgr used a sniffer & told him it was safe, but it wasn't. He was burned badly & got a huge chunk of $$. He said he'd trade the money just to have a normal life again 😔

These explosions like these are so scary for me. My husband works for chevron pipeline and literally watches and gets permits etc for all these different construction jobs throughout there plants and there pipes outside plants. He said these contractors can be so unsafe and rushing and he constantly stops or doesn't let these guys work because there are so many wrong locations etc and just want to rush. And these accidents like welding on a hot line can blow up everyone. People never learn

Obviously fault goes to Dupont. But want to make note that they genuinely believed and had reason to believe their process was safe, and still did further precautions to ensure the safety if the contractors. So while 100% fault lies with them, I would say they would not be liable for punitive damages, whereas in most other of these videos I'd award punitive damages.

Punitive damages are stupid. They are just another way to encourage businesses to implement "anti-sue" rules which are just rules so ridiculous and unjustifiable no one follows them and therefore there is a greater chance that the blame can be put on someone else. Safety is like a game of chance. The only way to run a safe business is to not do business at all. Every industry has thousands of potential safety incidents that they could get sued to hell for - even businesses that try their hardest and actually care. The only reason they don't is because those businesses are lucky enough that nothing has happened yet. The court system and punitive damages don't make the industry safer, they just fork money out of the businesses that pull the short straw and don't encourage anyone to try harder because it's easier to just hope you won't have a major incident and keep operating with sketchy practices. There is also no incentive to change the system because far too many people would rather get injured and sue (even if they come to regret it should it happen) then not get injured in the first place. If payment could be made that justifies hardship from injury then why would anyone do things safely.

People drastically underestimate how pervasive welding sparks can be One time I was welding and a spark bounced off my safety glasses… the INSIDE of my safety glasses… The spark had bounced off the back of the booth I was working in, entered my welding hood from the open back bounced off the inside front then off my cheek bone to hit my glasses on the inside So just because you can’t see how a spark could get somewhere, don’t assume it can’t find it’s way there

0:22 "Hey, I've been there before" -- and sure enough, I have. Yep, that exact one. Their safety regulations are insanely stupid at this point... "one person can't lift anything over 35lbs, 2-man lift maximum weight 100 pounds, anything heavier only by machine" -- when we had to deliver and install equipment (~500lbs) which is impossible to handle and move with machinery without damaging it. Ah, what a nightmare, and in the end, we had to just ignore their rules. Each time we showed up, had to sit through 4 hours of classes, explained our problems with their rules and what we needed to do, they got mad and sent us away, brought us back, repeated, 3x, and eventually what happened is exactly what always needed to be: yeah, at some point, it required 2 guys lifting and carrying 500lbs, just no way around it. But bleh, what an annoying place. Other stupid rules: whenever you back up you have to honk, wait a second, honk, wait a second, honk again, then start in reverse, *_even if your truck has a back-up warning beep_* -- things like that are just so annoying. And in the end, 2 hours of work, but had to sit through 12 hours of the same damn instructional videos by some condescending supervisor asshat. "Well, at least it's safe now".

See this is the problem with the current system of safety, they think they can make everything "safe" by just adding in more rules and training. Eventually it gets to the point where people either become so disillusioned with the system they just ignore the rules, or they become so sure they are "safe" they blindly walk into danger because they weren't using their own head. Businesses just implement stupid rules to stop themselves from getting sued and whenever they do get sued the courts find a way to blame the business anyway because you can always blame the business because "training and rules" is always the responsibility of the business. The only way to conduct a safe business is to not conduct business at all.

I’m really enjoying and learning from your video. When we do a shut down for a repair (which usually should’ve been performed six months earlier) it is the units responsibility to see that everything is locked out and blocked on your unjt. Our plant safety guy would sniff for us. Contractors usually have their own firewatch . Do you think it’s less liability to run like this?

It depends on the language and scope of work provided by Dupont at the outset of the work. If Dupont's plant/project engineers on this task were worth their salt, they'd have had some language to cover good work practices for safety (stressing the work is conducted in proximity to highly flammable gasses) that passed the responsibility, once the tank was isolated and emptied, to the contractor. The operations staff should have put one of their spare valves on the OF/vent to completely isolate that tank

Every ship in the ocean has a rotating shaft and is still sealed. Occasionally seals fail, but this has had an engineering solution for well over 100 years. It is a design flaw. I don’t believe it’s the primary root cause, I agree that’s what you identified lack of accurate PSM. Because if it was properly isolated the gap wouldn’t matter here.

May not be a design flaw. Keep in mind a ships impeller is not surrounded by a flamible liquid or gas. This thing is so anything you can do to reduce the chance of friction heating or spark is welcome. Also under normal circumstances the top of this thing is nowhere near being exposed to flame, gas leaking from it performs to safely measures a pressure release and allows the exterior alarms to pick up the presence of uncontained gas.

I can’t give examples due to NDAs, but flammable, corrosive, high pressure, low pressure, high and low temperatures are all situations that can have a sealed rotating shafts entering them. I chose propeller shafts as an example because it’s one almost every person can understand.

If you are an expert on sealed shaft rotating assemblies im sure you can give an explaination on other examples. Example a ford engineer can tell me about v8 engines since generically all v8 motors work the same just cant tell me propritary parts of the specific ford version of their own v8

I work with a lot of heavy machinery, and whenever someone gets injured in a preventable way in the company, it goes out nationwide and we need to sign that we read the notice. Two of them stand out to me, one was a worker who jumped down into the dock while a truck was reversing and got his head crushed against the brickwork. The other was somebody getting pinned against a forklift when he moved a load on the pallet (he was fine). When I signed off on reading them I said "That almost happened to me" for the first one, the second one I was like "I have done that before".

People get injured not because we don't have the means to prevent their injuries. People get injured because ultimately we don't care enough to stop it from happening. You might think that's a bad thing but try living in a world where all injuries are prevented to a ridiculous degree. You'll find you don't care that much either. For us to be able to do anything as a society, even to keep the status quo there always has to be a risk, different people just disagree on how much risk we are willing to accept. Most people don't agree with how much risk we have to face right now, but even less know how much it costs society to do things more safely. Somebody has to pay, whether it be dangerous or safe, the real question is how much are we willing to pay?

I'd say 70% on the company for not properly training/informing their employees, 20% on the manufacturer for not having a better design to keep out sparks, and 10% on the employees for not using common sense.

How on earth can you decipher blame? Maybe the grinder was grinding too deep, maybe he had his welder feed and amperage too high? Maybe he forgot his welding and grinding blanket? I am certain he was not using his welding blankets and he was not in control of his workspace. There is just a lot of unknowns. I’ll bet hindsight lawyers will say welding in a wet environment is dangerous and could kill you. I’ve seen people welding UNDERWATER! I’m not saying that satan wasn’t to blame, but the welder should have taken steps to ensure his environment was safe also. I say 99% DuPont. 1% welder ;)

A moving billboard showing a really good meme has lost its brakes and is out of control going down hill. At the bottom of the hill is a bayou. Before the intersection is a school bus(the short ones )full of grade school kids going to the aquarium downtown. On either side of the road are 200 pedestrians in the crosswalks and sidewalks. Hypothetically what should the driver choose to do? Hit the school bus or people on the sidewalk which might be more people than kids on the bus. The driver and vehicle are going to go into the bayou, no matter what he does. Also upon impact of the bus, the batteries operating the billboard will explode.

Sorry sir, but your "hammer" can not be brought onto premises due to it possibly creating sparks on premises. In order to prevent due negligence, please leave the hammer behind when bonking people on site with knowledge or replace it with a non flammable foam hammer. Thank you for your understanding 🤣

This is sad but I think this is split 3 ways with a good chunk of blame on the welder as the person who is performing the specialized job and he was not using the tools of the trade properly and lacked fundamental training for the work he’s was doing so much so I would compare it to driving a car for the first time with out a license and killing someone As much as I think the big shots have some explaining to do I think it has a lot to do with a lack of basic understanding of the jobs people are doing

Honestly, I somewhat blame the welder and the plant equally for welding a tank in service at a chemical plant. You have to be stupid to work with confined flammable gas and plasma at the same time. Unless the tank has water that is dipstick confirmed above the level of the weld, no welder should touch it. Should be the training they receive in school. The plant operators should have coordinated this before starting work.
