Dr. Tala you have a gift for teaching! This was so helpful! I had a patient this past weekend (I'm a student nurse in my last semester, precepting in the NICU) with PPHN, and so many words were thrown around that I didn't know the implications of. I feel like I have a solid grasp on her case now. It's so amazing to me to see how PPHN affects the entire body & to now understand the neonatologist's general thought process when deciding treatment. Thank you!!!

Thank you again. All the parts in this lecture are praiseworthy. In the diagnosis part i am a bit amazed to knoe about right ventricular hypertrophy, because hypertrophy is usually a slow process, and right ventricular dominance to some extent is expected in neonates. Correct me if i am wrong.

Thank you so much for your lovely comment! You’re right- the heart is right dominant after birth (on an EKG etc) but the RV shouldn’t be larger. It doesn’t take long for that right side to look thicker and become hypertrophied (days- weeks) but you’re right - the other stuff happens first.

And therapeutic bc I had these concepts loose and disorganized floating and fading in my brain, it feels so good to have a better understanding after watching your lectures! It seems like you've really tried experimenting and explaining this in different ways to see what makes the most sense to patients and your students. This is creme de la creme! What a great explanation of the physiology and function of the heart and PPHN! I truly and finally understood it. Could you go over the whole of lung physiology, dead space, PACO and PECO, specifically FEV and FVC, also Bohr and Haldane effects?

Hi Tala, I am a pediatric cardiology fellow and this is a great video but I just wanted to correct one little thing the ductus arteriosus generally attaches to the descending aorta after the left subclavian artery has taken off the aortic arch and not after the right brachiocephalic.

Thank you so much!!! A couple of other cardiologists have written to me too!!! Thanks for taking the time to write! You’re correct and I definitely said that wrongly- most of the time (nearly always?) the Ductus is after the subclavian. With our babies we emphasize so much that the right hand is pre-ductal and I think I went down that pathway if trying to show why that is. (Whereas in reality the left hand is often pre-ductal too. But just because it’s not always! We don’t generally teach that as “pre- ductal”). I need to figure out how to change the video (without watching it cos can’t bear to watch myself!!!) Thanks!!!

Thank you so much for all the videos, especially this one, I have watched it many times and it is really helpful and clear. If I may, I am having difficulties in understanding the pathophysiology of TOPS (twin oligoamnios polyhydramnios sequence), especially the neonathology consequences. I don t know if it was already specified by someone else, but I think it will help a lot of people with the approach after birth for this particular case. Thank you very much for all the work that you do, I am a neonates resident from Romania who practices in France and I can tell you that it is a relief to watch you and truly understand the pathophysiology.

Wow...my son Gabriel-Bruno had a severe bout of PPHN in Dec.2018...I wish I saw this video then (it wasn't made yet) or had a person explain it the way you just did. As a nursing student then I wanted mightily to grasp the situation, and I did...but not as thoroughly as after watching you show it. I recalled all the steps taken, and they were just as you explained. He was taken to Cohen's children's hospital in NY by a helicopter from level 3NICU on SI (also NY) to receive ECMO....(he was gray...at 1a.m. ,the entire NICU was packed...this caused a stir at the whole place)..ultimately God reached down and turned things around...without going into too much detail...we had a miracle! No ECMO was necessary....I still feel my heart skip when I go over it. Thank you for making medicine accessible, not stuffy or unnecessarily complicated...that's priceless.

Hello BBD! That must have been such a terrifying feeling to see your baby grey and then all that chaos and noise around as they tried to make him better. Sometimes beautiful miracles do happen. Hopefully this wan't enough to cause you PTSD to get out of nursing for good (I think some of the best nurses went through something like this- and they just GET IT). How is he doing now? Thank you Dr.Tala...my kiddo is excellent. (no further complications have come forth ,i.e. hearing, neurological- at his current age of 3 anyway. At 6 months he had a cardio follow up, and at 12 months a neurodevelopmental follow up), and I still absolutely love nursing (all health sciences) I just spoke with a mom whose baby recovered after 3 weeks of PPHN and it's very satisfying. Dr.Tala I cannot tell you enough about just how amazing all your teaching is...you are brilliant, and because you took the time to write and ask about my kid, it's also very obvious that you have a caring heart too...a true healer. That's fantastic news about your son- especially by the age of 3!! How fun (but exhausting!) to have a 3 year old in the house. I miss those days already!! I bet all your patients' parents LOVE to hear it from your side too. And I'm so happy you're in the medical field still- takes a lot of strength. The Left carotid is also pre-ductal, it is only the left subclavian that is post ductal (which feeds the left side and the lower half of the body)

Cassy - you're absolutely correct- and we have received so many of these comments - to the point that I think we should probably put out another video. The ductus doesn't always join the aorta at exactly the same place- sometimes- very rarely- even the left subclavian is pre ductal. Even though the left carotid is often pre ductal- we're unlikely to get a pulse ox on the left ear. But the right hand is always ( as always as may be in medicine I guess) pre ductal and the feet are always post ductal. We tried to complicate less on the video but this added fuel to the fire!! Than you so much for your intelligent comment!

Thank God i know all these already and I confirm there is no teacher in u tube like Her . Good job doctor 👍👍👍I wud recommend ur lectures to any medical enthusiast. In New Delhi ,India I have used iv sildenafil and milrinone, oral Bosentan and iNO in neonates. Even magnesium sulfate too shows good result and is cheap. But all these drugs also cause hypotension except iNO and Bosenta so these both are my drugs of choice. I have never used ECMO. Next ...i wud want u to discuss the management of hypotension in a neonate who is having hypotension due to side effect of drugs used in management of PPHN. How low can we let BP to fall

The best thing for my daughter was when the nurses just left her alone and gave her back to my wife. Worked better than morphine. Great talk, I do think that in my area. The hospitals need to question if it isn’t their intervention which is causing the most difficulties. I’m literally on the internet trying to figure out why they felt like they needed to do what they did. So far, no credible excuse.
