Today, B.Sc. Nursing 4th semester students from Lucknow Metro Institute of Nursing and Paramedics received an in-depth lesson on electrolyte imbalances from Dr. Abhishek Shukla at Aastha Centre. He explained that electrolyte imbalances, especially sodium and potassium deficiencies, are common issues in the elderly. The body’s four most important electrolytes-serum sodium, serum potassium, serum magnesium, and serum calcium, with serum phosphate also being crucial-must be carefully managed.
Dr. Abhishek detailed the conditions of hyponatremia (low sodium levels) and hypernatremia (high sodium levels). Hyponatremia often results from diuretics like Lasix, Torsemide, Spironolactone, and Eplerenone, leading to symptoms such as confusion and muscle weakness. In contrast, hypernatremia is typically caused by dehydration, where the body’s sodium concentration (hemoconcentration) increases due to water loss, causing extreme thirst and agitation.
The students learned the importance of maintaining balanced sodium and potassium levels, emphasizing the role of a potassium-rich diet, including bananas, spinach, and tomatoes. Dr. Abhishek cautioned against rapid correction of sodium levels, as it can cause Central Pontine Myelinolysis (CPM)-a serious condition that damages the brain’s myelin sheath and can result in severe neurological issues. He also stressed that intravenous potassium must be diluted to avoid cardiac arrhythmias and that overcorrection should be carefully avoided.
Dr. Abhishek emphasized the importance of continuous clinical observation and patient assessment after correction, ensuring the students understood the critical steps involved in safely managing electrolyte imbalances in elderly patients.
