For whatever reason, KZitem isn't showing a "Reply" link under Ms. Barbara Keating Wolk's comment, so I'll answer here. Her original question was "Is it okay to do these exercises after an injury...(fell on outstretched arm very forcefully) I have a foam roller but not nubby kind but will get one if it will help the pain in arm and shoulder. thank you!" The answer is this: How does the average untrained person know he/she doesn't have a hairline fracture or a tear of some sort? THEY DON'T. So in a case like the one she mentioned above where she fell on an outstretched arm "very forcefully", the smartest course of action is to see a doctor FIRST before trying a self myofascial release protocol like the one I demo here. If the doctor runs the orthopedic tests and any imaging is negative for structural compromise, then absolutely give this a shot and DO get a Rumble Roller or a Beastie Ball.

Hi Mark - Great clip, like Cook and co, your explanations and application for this stuff is brilliant. We are lucky to have you present your ideas and work! I work with many elderly, obese and poor mobility clients / patients. Getting down on the floor at times is very difficult. I have found where safe, using a treatment table is great for these people. When the problem doesn't present as a SMCD or tolerance to body weight on a foam roller is not great, standing against the wall using a tennis ball or massage ball with different are positions can also provide ideal results.

Awesome video Dr. Cheng! I was recently performing an overhead triceps exercise with a 45lb dumbbell, and banged out 8 solid reps. However, whenever I bring it down, I always tend to let my right hand carry on to it and rotate the dumbbell from the back to the front and rest it on my shoulder. Obviously this isn't a good idea. When I was doing it, my hand slipped slightly and it put a lot of strain on my shoulder. After about 15 seconds of it happening, I began to get shooting pains and then they went away after about a solid 30 seconds. This happened on April 27, and I have been icing and resting for a few weeks since then. After a week of rest, I decided to go back, and still felt a weird feeling in it during my back workouts. I have decided to go this week after week of rest and plenty of rotator cuff strengthening. I decided to use lighter weight, but I am going up slowly back to the weight I was at before I got hurt. Any advice?

WOW! Mark,i've seen chiros about my rotators but all they gave me were stretches that never helped.Always diagnosed as hypertension in the chest and super tight deltoids, which I'm sure it also is.But I just did this on my left arm pit and when i stood up it was like my left shoulder was relaxed and my right side was all tense. that was one of my best foam rolling experiences ever. I'm using the small trigger point roller now. was going to get the big one. think the rumble roller is better?

I done this and I know I have a lot pain in the rotator cuff in the back and along the front of the shoulder where it seems to attach to the bicep, almost feeling like it wants to tear where it joins at the shoulder and bicep. The rolling only helps so much, is the roller releasing a group of tight muscles or a damage muscle. What else can a person to be rid of pain. Especially when a I have to lift something over my shoulder height, which is basically unavoidable.

If you've done this foam rolling on your infraspinatus with a Rumble Roller, Beastie ball, or lacrosse ball correctly, and the problems still persist, then this isn't the root problem. You need a medical consult, preferably with a professional who is SFMA (Selective Functional Movement Assessment) certified.

+Allen Bay You might have something else at play that's causing your pain. A visit with a licensed chiropractor or physical therapist who's familiar with the SFMA might be a wise decision. Please check out www.functionalmovement.com & locate a provider.

Entirely possible, depending on what you're doing in training/sparring. The tight side is the one you'll want to go after, usually.

+Stonesimpler3 Hard to say without an in person examination. Stretching tight muscles is usually not the best move. You might want to look at getting a hold of Kettlebells From the Ground Up & studying that DVD set.

Don't bring a fitness solution to a medical problem. If the cartilage tear is severe, it'll need surgery.

Hi thanks for the video. Can You also use is the foamroller when you have a calcific tendonitis in supraspinatus?? thanks again!
@DrMarkCheng

The supraspinatus is a much smaller muscle, mostly wedged between the two spines of the scapula. Trying to get at it with a flat foam roller is very difficult.

Hello Doctor i would be extremelly happy if you could answer me this. While i do every ab exercise i feel pain/burn at my lower back after few repetitions. I went to a doctor and he said i have very tight hamstrings and thats what causing me the pain on the lower back. I bought a moderate foam roll recently, but i cant find any trigger points at hamstrings with this. Should i keep doing it or i should try something else? Should i avoid to foam roll any nearby muscle group? Thanks in advance!

@DrMarkCheng

You'd probably get better results with a lacrosse ball. A smooth roller is too broad and flat of an object to really get any meaningful penetration into the trigger points of the rotator cuff.

If the "impingement" is caused by compartment syndrome caused by competition for space in the shoulder joint, it can help. Nothing's 100% in medicine.
