@@zochert57 That's true , and I agree with a lot of what he says - especially LDL being good (if it is in the large particle form). However, he is making the leap that the high fat diet /low fat is responsible for his and others' improvement; and the only way. I have the exact same lipid pattern - low triglycerides, high HDL, high LDL, good sugar, on the thin side - 5'5" 105 lbs, 62/y/o; low BP, no cardiac disease, good health, active, no medications and I have always eaten what I'd call a diet of moderation - and now, I'd call it high carb . So, obviously this diet is not the only way to achieve good health. I think the removing of processed foods, and avoiding pesticides/chemicals is responsible for the improvement many feel - b/c I hear it from all types of extreme diets, from carnivore to vegan. I worry about people thinking they need to do extreme diets when it is more complicated. Also our artificial light, exposure to non native EMF's raises blood sugar and has metabolic effects that are ignored by most MDs and diet gurus; but we must start to be more broad in looking at the entire environment - not just food.

My weight and health story is very similar to those of others. I'm 5'6", 57 years old. I weighed 155 when I completed my undergrad in 1987. In June 2021, my weight had climbed to 220 lbs, mostly due to working from home due to COVID (less activity and spending too much time a few steps away from the refrigerator). My family doctor told me that I was morbidly obese, and that hit me between the eyes. That day, I decided to change my lifestyle. At first, I was going to do the usual low-fat, calorie-restriction diet. But, I knew that that would ultimately be a failure, long term, because it always is. I did some research and decided to go low-carb (ketogenic). At first, I thought I'd just do it to lose weight then switch to something else, maybe Mediterranean, but then as I learned more, I realized that low-carb is a very sustainable, healthy, life-long dietary option. Along the way, I also incorporated restricted eating (18:6 or 20:4 daily) and periodic multi-day fasts. I absolutely loved the fasts, coupled with low carb and absolutely no sugars or starches. At the beginning of July 2021, I weighed 220. A week before Christmas 2021, I weighed 150... less than my college weight. I had been pre-diabetic, high-blood pressure, high cholesterol. After losing the weight, my A1C is 4.5, my BP is usually 115/70. Great HLD and triglycerides and am Pattern A lipids. But, my LDL is high (not surprisingly), and my doc wants to put me back on a Statin. We're going to have a very long conversation about that, and I doubt VERY seriously that he'll be able to convince me to go back on a statin again. I may even be looking for a new GP, who is knowledgeable of the low-carb lifestyle.

Your experience and iterative approach follows my experience closely. I think of the approach as a 3 legged stool. 1. Create a daily calorie deficit by exercising at least one hour per day while moderating food intake. 2. Adopt LCHF. No sugar, no starch, high on protein and fats. Minimal polyunsaturated fat. 3. Intermittent fasting. 18:6, most days. Result. 16% reduction in weight, BP in your range, sugar in the normal range. All metabolic markers good. Only exception is LDL which runs slightly higher.

This is a great discussion and sheds some light on my own moderately elevated LDL. I have adopted an OMAD lifestyle for the past 18 months and have attained some shocking goals. However, I’ve changed so many variables that it is unclear which ones have been the major contributors. I suspect, more than anything else, has been the fact that I typically avoid anything from a box, can, or plastic bag that has been highly processed and contains ingredients I can’t pronounce! I do consume a huge amount of food in a 2 hour window in the evening consisting of a Mediterranean style composition (high EVOO, wide range of low lectin vegetables, primarily goat and sheep dairy products, wide range of herbs and spices, and whatever protein my wife feels like having for dinner). I am not a zealot and will consume anything put in front of me when we go out to friends or to a restaurant (which is,I guess, once or twice a week). I had been on meds for diabetes, high cholesterol, and high blood pressure for many years, but shocked my doctor and no longer require any of them. I don’t know how much weight I’ve dropped, but I’ve had to donate all of my clothes and currently maintain a very small wardrobe because it is still dropping. I think OMAD has significant benefits, but suspect avoiding processed foods has contributed more to my improved health. I suspect big Pharma and the Mega food industry has been the main issues in my life. I don’t miss either one. 😀. I am now focusing on gathering data on my fasting insulin and glucose levels to track my HOMA-IR and QUICKI scores along with my triglyceride/HDL ratio and LDL particle sizes. My doctor doubted my efforts when I started this journey, but I now have his full attention (and my own for that matter). He stated at my last visit that if I am able to maintain these results for the long haul that I should write a book! I had to laugh at that and simply responded that there are already hundreds of them written - we just need to get people to read them!😊😊😊

Thank you so much for posting this! In the last year I went from 217 pounds at 5'7" 37 year old female to 154 lbs. My fasting glucose last year was 113 and it is now 86. My total cholesterol last year was 230 and it is now 215. My triglycerides last year was 114 and is now 49. My HDL last year was 51 and is now 52. My LDL last year was 156.7 and is now 153. 6 months ago my total cholesterol was 188. My LDL was 125.6, HDL 49, and Triglycerides was 68. My HR went from 73 last year to 59. I am now normal BMI. 6 months ago my doc told me she wouldn't bother me about cholesterol meds anymore but then I got the letter from my last appointment telling me to eat a low fat and heart healthy diet. I was worried. I mean I've lost 63 pounds in a year. Corrected my blood sugar, and had my cholesterol corrected so I thought (and now think again lol). I also had an EGFR of 59.1 last year and now have an EGFR of 84. Here I am hearing that a plant based/low protein diet is good for my kidneys and cholesterol but on the other hand, high protein and fat is good for my blood sugar. It's so freaking confusing! I am doing the absolute best I can and one doc or another will say I'm doing it wrong. This video shows me that I did everything right. I did also have a lot more beef in the last two weeks before my labs than I usually do yet my kidney function is still normal. I'm not going to hear I need to go on statins simply because my LDL fluctuated only 27.4 points. Sounds like that beef did me a favor after all.

He didn't bat an eye at any of the rest of the data but told me I must take statins because I was " so healthy " and If I wanted to stay that way I needed to take them . My experience with LDL and A1C is somewhat unbelievable. I had high total cholesterol (225) and high triglyceride (325) measured after eating good breakfast (I should have not done that) and my A1C was 6.4. My doctor put me on rosuvastatin 20 mg per day (equal to 40 mg of atorvastatin per day). I became quite sick, weak and plenty of muscle pain. My A1C went up to 6.4 from 5.9. I put on 8 pounds. During those days, I used to eat rice (once), bread and some fruits (2-3 types) daily and some meat (on and off) and two eggs. It is quite possible that I am very sensitive to Rosuvastatin. 6 months later, my total cholesterol went down to 120 (LDL was 69), HDL went up by 30% (40), triglyceride was 63 and my A1C dropped to 5.4 (all measured after 14 hour of fasting). I lost 8 pounds. What did I do differently? I reduced rosuvastatin by taking 5 mg every alternate day (that is like 2.5 mg per day) and I stopped eating any added sugar. I eat twice a day now (lunch and dinner and eat absolutely NOTHING in between). I reduced my carb intake by 50%, stopped eating FRUITS completely (fructose in fruits is BAD), increased my protein content (120 gm per day; chicken, two whole eggs, some cheese) and take approximately 4 teaspoon of good oil such as olive oil and avocado oil. I significantly increased vegetables like green stuff (twice a day). I eat a mixture of nuts (walnut, pumpkin seed, flax seed, almonds, peanuts) once a day. I walk 30 minutes everyday, indoor bike for 20 minutes and that is it. I do my best to keep active most of the time (rather than sit for hours etc) by doing household stuff etc. I started avoiding stressful situation as much as possible by not reacting to every little stupid things in life. I have more energy than what I had 20 years ago. I don't get irritated much now. I sleep better. My focus is better. I hardly get tired now. My resting heart rate is very normal now. I am thinking of stopping rosuvastatin once I talk to my doctor next month. I will add resistance training (three times a week) to my routine. And I am a 65 year old person.

Just a technical note: Hb1C does not change after meals. It is a measure of the "sugar pressure" in the past 3-4 months. Although a good indicator of insulin resistance, it can vary a lot over time, so testing every 3 months is a must. Another relevant test for insulin resistance is HOMA-IR (search the internet).

I have a great interview coming out later in October with Dr. Michael Twyman. He said that in most circumstances statins would not be recommended, but it is an individual decision that should be well weighted against pros/cons and other lifestyle changes, supplements, medications. Ideally this decision is made in collaboration with a cardiologist after further testing than just "high cholesterol."

I’m 44 years old. I’m in great shape for my age. I can actually run laps around people half my age. I’ve been intermittent fasting 16-20 hours a day since 2017. Last year I had a total cholesterol of 203. This year I went to the doctor and he told me my total cholesterol was at 283. He yelled at me and said I’m going to be dead of a heart attack or a stroke within 10 years. Then tells me I better start doing cardio, get more on a plant base diet, and prescribed me statins. I told this doctor I’m in very good shape. I run sprints 3 days a week and run 5 miles 2 days a week. This doctor said to me that he doesn’t believe me because of how high my cholesterol is. I said to him that doesn’t make sense because of how much i fast. I do 1-3 24 hour fasts per month. I don’t remember my LDL numbers but the doctor said my LDL is so high that he’s expecting me to have a stroke. Lol. The doctor said fasting doesn’t lower cholesterol. I stroked he’s ego by saying I’ll go a plant base diet and I took the medicine and threw it away. Does anyone know how my total cholesterol sky rocketed like that?

Do you eat a diet high in fat? If yes that may be the reason the total cholesterol is up. Not saything that is bad to eat a high fat diet (depending on the types of fat and your personal risk factor for CV disease), just saying that may be why. Dr. Peter Attia's Book outlive is an excellent review of CV risk. He suggests an ApoB test (for LDL particle number) and Lp(a) test - a genetic factor that increases CV risk.

My wife and I are arguing over LDL. My body, my decision! Trying to stay married, but I have to be alive, to accomplish my goal! My LDL went up, but weight went down, and my sugar is staying below 150, and that’s what my doctor wants! I’ve been Carnivore since August 8, and I’m staying in that lifestyle! If doctors are having trouble finding information on this lifestyle, imagine how much trouble the rest of us are having.

That’s wild… My wife who has auto-immune epilepsy uses carnivore and animal based to help with here autoimmune condition and for anti inflammatory benefits. I’ve used it to combat and completely eliminate my hypoglycemia. The WHO and other large entities have changed their stance on Cholesterol and sat fat. Maybe let her know this and direct her towards those resources so she can see for herself. It can take folks awhile to jump on and stop fearing LDL… It’s just at the scene of the accident, not the cause. The science studies used to associate cholesterol and saturated fat with fat with heart disease, diabetes etc were and are old questioners , and nutrition science is difficult to conduct ethically and accurate. We simply can’t get accurate metrics unless you lock someone up and force them to be in a lab… So these studies are faulty at best and shouldn’t be assumed that associating is causing…. What about the other lifestyles of these folks in the folks? This goes over the top of an average persons head. Added sugars, starches, , trans fat, high triglycerides, and high blood pressure is what we should be worried about, not something we’ve been eating for millions of years(from ape to human). The food the health care professionals recommend we eat hasnt really even been around for 100 years(seed oils, new age crap bread full of too much gluten, plethora of new herbicide, pesticide, gmo, varieties of vegetables, fruit that never existed.. These are all things to think about and consider. This can take many years to understand. It’s a rabbit hole. Like all things in life, surfaces are not as they appear most of the time. Interesting interview. In March of 2022 the JAMA published research (comparing over 100,000 people in various studies) and the conclusion was that treating high LDL with statin medication had no clinical benefit to the patient. In other words, statins... which lowered LDL cholesterol, didn't do anything to reduce risk of death, heart attack or stroke. So, it seems many people have been going through horrible statin side effects for absolutely no health benefit.

I had so bad inflammation in my leg that the doctor suggested amputation. I decided to go on Keto with IF, LOST 80 LB in nine months, keeping 130-135 lb for the past three years, inflammation is gone. Overall I am feeling good but my doctor wants me to go on statins which I rejected. My LDL cholesterol is above the guideline but also my HDL is high too. Ratio is 3.8 so I don't see why I need statins, especially that my triglycerides are 60. If I don't have inflammation in my leg that means I don't have inflammation at all and since inflammation is the root cause of the vessel blockage I think I am pretty safe

This is excellent. As a T2D I started on a keto diet, several months ago. My A1C - 5.6, I have stopped 2 of 3 BP meds, Zetia, Livalo, and requested my GP get me a CAC test. When it came back at 1207 with the LAD at 720 my Dr. called very concerned and wanted me to restart my statin. Granted I have no medical training, but at my insistence I had a heart cauterization last year that showed I had a 70% blockage. The purpose of getting a CAC was not in my mind to determine if I had blockage, but rather to obtain a baseline for future testing. My last lipid panel was before stopping my statin was TC 210, HDL 48 Tri 144 and LDL of 135. I also requested a test of the total lipid breakdown, but this was not performed. At this point, I do not want to restart taking a statin.

Burn daily 100 to 300 calories on a Stationary Bike, and that exercise will have numerous additional benefits

If your HDL is high and Trigs low it is likely that you have predominately large LDL. The concensus seems to be that it's the small LDL that we need to worry about. An LDL subtraction test would confirm that for you as it did for me. But it is unlikely that your doctor will order it for you. He'll probably say as mine did that the results wouldn't change his Statin recommendation. All they know is that a high LDL=Statin.

Statistical correlation does not say anything about cause-effect relation. In the morning I drink my coffee and the sun rises. Does it mean that the Sun rises because I drink my coffee? A lot of "scientists" pick 2 variables and calculate the association between them, then present this calculus as a "clear scientific proof". Which is plain wrong. Statistics does not demonstrate a thing, it is just a way to refine the hypothesis. But has statistics has no value in demonstration of the hypothesis.

Interesting presentation. Some physicians online on KZitem also talk about higher LDL levels as not being necessarily unhealthy but this is if the LDL Particle Size Test shows that the Pattern A LDL particles (the fluffy LDL) are the ones that are elevated above 80% of the total of all LDL-C, while the Pattern B (small & dense & called LDL-P) which are not good because they tend to remain in circulation for an extended time, are trouble when they begin to increase beyond 20% of LDL-C. When this occurs there is supposed to be a greater chance that the LDL-P can penetrate and often get lodged in the artery walls & form fatty plaques which can eventually build up & lead to blockages ---even if the LDL-C level is rather low. LDL-P, is apparently formed from oxidation to the fluffy LDL (which apparently is caused by insulin resistance & associated elevated bg (blood glucose), stress, & food allergies), turning it into the small dense LDL-P. LDL-P levels above 20% is thus supposed to predict the risk of CVD (Cardiovascular Disease), not levels of LDL-C (Low Density Lipoprotein Cholesterol) which may be just fine if the LDL-P is below 20% of the LDL-C.-- Is this true? Did this understanding of LDL-P as a culprit occur just over the last year or two? Can anyone clarify this, or correct this??

I recently had my first physical after doing a low carb / high protein / intermittent fasting diet for a couple years. Within 30 sec of sitting down the Dr. brought up my "very concerning" high cholesterol and how I needed to go on a statin prescription ASAP. I politely declined, but the training was so ingrained that this one number is somehow so devastating to my health... yet literally every other indicator available pointed to me being perfectly fit. I am the perfect example of what Dr. Ali is describing here. I used to have high blood pressure, high triglycerides, high sugar levels, a fatty liver - and now all of that is reversed and I am in the best shape of my life... and my LDL is high.

I think that the blanket statement "a high fat diet" may confuse some because there are healthy and unhealthy types of dietary fat. A high fat diet from trans fats and/or processed and refined vegetable and seed oils (from processed and fried foods for example) will certainly increase one's omega 6:omega 3 ratio and favor a pro-inflammatory state, which then would not be favorable to TG/HDL ratio. However a diet that is high in anti-inflammatory omega 3s (especially from fatty fish and green algae), or other whole food sources of omega 6, 9, or 3 would favor a less inflammatory state, and thus favor TG/HDL ratio. Saturated fat has also been shown to raise HDL so would favor the ratio, too. I cover dietary fat more in this video: kzitem.info/news/bejne/w5uCzWuIpoN7aag

What did that tell me at first? Go back to eating fast food, it must be healthier hahaha! The doctor wanted to put me on a statin so here I am trying to find a Cardiologist like Dr. Nadir Ali.

I am a DC and have practiced functional medicine my whole career. I have been eating a good fat, low carb, organic food, high quality meat diet for over 25 years. I have a CC Score, as 0, 10 years apart. I am 65 years old. I have high cholesterol both HDLs and LDLs, and I don’t have heart disease based on a brain scan and a scan in my carotid artery after ending up in the hospital post Covid vaccine with transient global amnesia. I have no intention to take statins and found an MD cardiologist that was on the same page. I also have a low carb diet. The problem I have now is that I have a lot of digestive problems, (it runs in my family, genetics are a factor I believe) I think related to my gall bladder and really have to avoid the fats I was doing, mostly sunflower oil (flora), EPO(Efamol) Brand matters. I have done everything I can think of to support my gallbladder. I can do olive oil and butter, and use them liberally, but am not getting the essential fats. I had an upper GI, nothing showed up.

Most persuasive is the fact that arterial plaques form not in the lumin of the vessel, but within the walls of its intima. This gives us an event horizon timeline of plaque formation because for cholesterol to enter into the walls of an artery, there would first have to be a tear in the lining of that artery. I believe the source of those tears is chronic inflammation from stress, an unhealthy diet, and a sedentary lifestyle.

Agreed. The stress of the last three years of Plandemic, and being an unvaxxed person (I HAD the Wuhan steain for 8 weeks and natural immunity!!) during a military psyop has caused me to have a load of stress, less exercise and resultant high blood sugar, LDL and pressure. It has caused a lot of inflammatory processes in my body. I am now trying to reverse that stress. While I haven't had a calcium cat scan I was thinking of asking for one but you didn't give any indication of how a score above 0 in combination with high LDL would be interpreted for a person on a low carb diet. While it is probably obvious a score of 0 would be great, at what score would you be concerned that a high LDL should be lowered, presumably with a statin?
@drmorgannolte

I'd refer this to Dr. Nadir Ali or Dr. Michael Twyman. Dr. Ali did mention in our interview that he had people with very low calc scores with very high LDL and he wasn't concerned. However it is person-specific.

When you already have stents placed, but you had a history of normal cholesterol/LDL and post angioplasty introduction of statins brought ldl levels to as low as 23, does it still make sense to continue with statins. Note that the patient is diabetic for over 30 years and is currently 67 years old. Now the patient follows a low carb diet and the diabetes is well under control.

The cognitive dissonance can be easily solved with the simple realization that doctors, scientists, nutritionists, etc. are just regular people with the same characteristics as all regular people. Namely, they have the same tendency as all people to operate with biases that are strongly, almost irreversibly, inculcated by the societal institutions that grant them their status. For example, I'm an accountant and my professional biases were inculcated by institutions such as the American Institute of Certified Public Accountants (AICPA). Late in my career as the CFO of a medium-sized manufacturing company I found myself increasingly getting into disputes with our independent auditors who are charged with verifying that we're keeping the books in accordance with Generally Accepted Accounting Principles (GAAP). Without getting technical, the disputes were around GAAP versus the real, actual purpose of accounting, which is to provide clarity for non-accountants to understand the financial statements. More frequently than one might imagine GAAP would actually distort the financials rather than provide clarity. Why? Simply put, because the eggheads at the AICPA are just people who are as prone to mistakes, lack of understanding, hubris and so on as all people. There's nothing special about "experts" beyond that they tend to view themselves as special. They aren't. Homo sapiens means "thinking man" and that applies to us all, not just the "experts". So many doctors are programmed to operate strictly based on numbers without regard to the big picture. All people are different and a person with high LDL could be just fine while there are many, many people with low LDL coming into emergency rooms with heart attacks every day. The big picture is that if someone is doing fine, and in fact has been improving in their physical well-being markedly, why the rush to put them on drugs just because some number showed up on a piece of paper? I'm not saying to ignore the number; I'm saying to apply basic common sense and observe further, test further, but don't let the number rule us. Homo sapiens, people, "thinking man".

See the bias there :)"
@csmats5374

@@drmorgannolte Yes, as a Homo sapiens I am indeed biased, as are we all. The rub is to first perceive that everyone is biased, then to detect where biases lie, and then finally to apply intellect as "thinking man" to determine the correct answer or at least the answer that works best for oneself. Bias doesn't necessarily mean "wrong", although it can be just as wrong as it can be correct or even neutral. For example, Dr. Martin Luther King, Jr. was certainly very biased and his bias ran heavily in the direction of Christianity -- it formed the foundation of his whole moral and intellectual outlook. I'm not religious so it was easy for me to detect his bias, but I applied my intellect to his stated moral and intellectual outlook and determined that his biases were either correct or they at least conformed to my personal moral and intellectual outlook (i.e., my biases, as different as they are from his biases). Racists, however, take a totally different view of MLK based on their own biases. The problem you bring up in your video -- the cognitive dissonance folks have with relying on the at times dogmatically stated advice of doctors, scientists, et al but seeing with their own eyes the negative results of said advice --- stems, I believe, from them not detecting the biases of doctors, scientists, et al and proceeding on a course of blind faith because "they're the 'experts'". Everyone is biased in some way. Detecting those biases is a necessary step toward independent thinking. But the step before that is the base recognition that everyone is biased to begin with.

The LDL Molecule splits into Large Volume LDL and Small Density LDL. 80% of the LDL molecule is VLDL and is neither good nor bad for cardiovascular risk; it is neutral. The remaining 20% of LDL is SLDL is where the plaque-forming mechanism lies and where the danger is for cardiovascular disease. So, this needs to be monitored. SLDL is created through the liver when sugar is metabolised. If you cut sugar from your diet, SLDL will collapse, and triglycerides will also improve. The poor man's test for improvement is to monitor the ratio of Triglycerides to HDL and keep the ratio below 1.5; in terms of cardiovascular risk, you will remain healthy. Helping the liver maintain less oxidative stress by eating less sugar and refined carbohydrates will also help the cholesterol and lipid profile to support cardiovascular health.
