I have been a nurse over 25yrs mostly worked in clinic setting. I recently went to LTC nursing because the pay is considerably more. for LPN,s. OMG the charting at long term care has been so confusing for me, untill now. Thank you thank you thank you. Your acronym just made everything click. I feel like a real nurse. Over the years since I graduated I have taken many distance learning courses toward my RN but have not completed because I have dreaded the charting and never got the hang of SOAP charting. Mostly because I graduated 96 right in the beginning of the change over mess of paper to computers. Software after software changes from hospital nurse big city click and pick charting to small town hospital paper to one Dr clinic to multi provider clinic in the middle of changing over from paper through 2 computer programs and patient care model changes. You are amazing at teaching. Going into the LTC on nocs no real training and no one has any time to help cause God forbid you stay a min after or work over time. Just me and 3 cna for 42 pt. I have felt so incompetent for the last few months. But after your vid I feel confident. I am back. And inspired to get my RN.

Thank you for doing this it’s well needed. I love acronyms. However, I was wondering if it’s best for documentation purposes to write DISCUSS before new med INTERVENTION since that is what we actually do. What are your thoughts?

Thanks for watching and commenting! You can write your note in any order you personally like of course. Generally, though, you inform family of new order after it's been put in place unless there is some sort of family dynamic that they want to talk about everything first. "discuss" is just referring to the notification of family. And the "intervention" relates more to a nursing intervention, what you tried before a medication if applicable. I hope this makes sense! These of course are just suggestions and how I write my notes, you do whatever you find works best for you and what you feel comfortable with :)

Yes we can! When in doubt call, if they get mad it's their problem. A bloody nose can be an issue, especially if the patient is on blood thinners. Don't beat yourself up. It's better to be safe than sorry. And if it lasted for a bit, it's important for the doc to know. I think you did a great job!

Hello Benomi thabk you for the videos. Can you please help us with notes when you detect a medication has interactions and you decide no to give, and you are waiting for the dr. To respond in the morning. Other progress note also when you have to administer something for constipation but is not due until next shif so what can you do write on the PNote. for the pt (being a night shift nurse) and Dr. Office only have the message service

You're welcome, thanks for watching! When you say interactions, did you mean side effects? It might be easier for me to help you over on IG if you'd like. Working nights is hard when it comes to that stuff! But if you hold a medication, you need to put the reason and that you are awaiting response from MD for further instruction. Something like, "____ held d/t nausea/vomiting. MD answering service notified, awaiting response for further instruction." "Pt noted to not have BM for 4 days, routine [medication] administered, not effective in producing BM. MD answering service notified of need for additional stool softener/enema. Awaiting response." I hope I understood correctly and that this helps!

@@NursingWithBenomi I appreciate your videos. I have been a nurse since 2014 and have mostly been involved with complex home health cases and agency work through the physician and specialty offices. I am newer to the long term care setting. I enjoy it throughly and believe I can make a difference there where there is a shortage. However, it is actually quite complex the skills you have to have to mult-task and keep on track. I mostly deal with point click care. If you know of any videos or are willing to make a video it would be great to navigate the system. Many nurses are unable to tell me basic things like where to find how a patient should be taking their pills? How to put a new order in the computer? What is medicare charting specifically and is it just a nursing note? Are patients that are on new meds/patients taking prn meds/patients with a change of mental or physical status the only ones you chart on in a shift? Do you chart at a certain frequency for newly admitted patients? What are the RN supervisors responsibilities vs the floor LPN. I am agency going into the facilities and I feel there is a disconnect as each facility has some different policies, but thankfully overall their is a similar systematic approach.

Lots of those things are based on each facility policy unfortunately. When it comes to who you document on, it usually depends on what unit you work on, whether skilled or long-term. I would suggest requesting to meet with the DON when you take the assignment to get a clear understanding of what the requirements are. I'm sure they would be happy to help because it will in turn be helping them meet their requirements.
