Case history of a patient with Kandinsky-Clerambault syndrome, born in 1905:
“Patient S. has been in a psychoneurological hospital for 2 years. He answers questions willingly, to the point. He roughly navigates place and time. He declares that he is a “prosthetic person”, that the group of persecutors has completely subjugated his will by constantly influencing him. First of all, the pursuers act on him with "voices", which they transmit through the "umformer" directly into his head. The patient calls his pursuers "Fancogols" and declares that this is a special group of people engaged in research into "ultracurrents for interplanetary relationships." "Fancogols" are able to take away the necessary thoughts from the patient and put their own into him. Through a “mindformer" they often influence the patient’s speech, forcing him to utter a set of incomprehensible words. Often the pursuers act on vision, “for this they touch the eyes with an electric current and cause them to move.” Surrounding objects at this time they begin to blur and disappear completely. The patient accompanies his statements with a sluggish emotional reaction. Often freezes with a “absent” gaze directed into the distance. When asked what is happening to him at the moment, he responds with a great delay that the “fancohols” are not leaving him alone even now."

Kandinsky-Clerambault syndrome is the most common among chronic delusional syndromes. With this syndrome, the patient exhibits pseudohallucinations (devoid of sensory aliveness, incorporeal, producing a feeling of being “invested” in the head), delusions of influence (mental, telepathic, laser, hypnotic, etc.) and mental automatisms. Automatisms are the alienation of the patient's own mental acts. They are felt as processes occurring automatically, against his will. A person ceases to be the master of his thoughts, movements, sensations, and is unable to control motor acts with volitional effort.
The phenomena of mental automatism usually instill in a person the idea that he no longer belongs to himself, experiences a feeling of mastery of himself, feeling like only a puppet. It is also possible to think that the entire mental and physical life of the patient is controlled by someone using hypnosis or some technical devices (delusion of influence).
Kandinsky-Clerambault syndrome is usually considered a characteristic manifestation of paranoid schizophrenia.
