Hello Doc, you just said my story. I’m devastated with chronic Fissures (with sentinel piles) since more than a year now. Got several treatments but to no avail. Comet poop is what I’m noticing recently as the reason even after taking much fiber and almost 5L of water a day. I’m so relaxed to know that at least someone acknowledges such a situation. Almost all doctors sing the same fiber song.

Wow! You are the only doctor that has said this! Every doctor I spoke to was saying ‘take more fibre’ but I was having comet poops all the time! I restricted my diet to mainly fruits & vegetables (which was making me miserable because I couldn’t eat what my family & friends were eating.) It was cutting my backside & making my anal fissures much worse! Listening to your video, I dialled my fibre & ironically my stools are less bulky & smoother! Thank you Doc!

I had the same issue. When I was diagnosed with grade 1 hemorrhoid, my physician asked me to get more fiber to avoid worsening my hemorrhoid. I started drinking 2-3 spoons of Metamucil or eat high-fiber cereal during my breakfast. They caused comet poops very frequently. I changed that habit and now I eat 1/4th cup of high-fiber cereal right after lunch and also right after my dinner. This helped me immensely. I have been practicing this for 8-9 months now and never saw a comet poop except once, which was not as bad. I also changed my diet to eat more brown rice and Quinoa with cooked vegetables and I believe this is also helping a lot.

I think the poop at the beginning of the train is usually pebble poop or comet poop because it's that little bit from the previous session that didn't come out. It sits in the colon for too long losing water and that's why it's hard. When you are fully emptying and on a good schedule, it's minimal to nonexistent. Of course diet and many other factors affect the ability to keep a good schedule. The only reason food with soluble fiber comes out softer is because it brings in more water. Food itself all gets chewed and is soft going down the pipe. How it comes out has everything to do with water intake and retention. Of course many factors affect this like fiber balance, sugar, salt, stress, exercise, sleep, etc.

It’s insoluble fiber that either ferment and can’t hold water but speeds up transition time. All fibers are Not created equal. Go for soluble fiber and be sure to hydrate well. One can’t say “eat more fiber” or “it’s due to too much fiber”. It is due to the wrong type of fiber. If you still have hard stools mainly consuming soluble fiber you are doing something wrong or should look at food sensitivities etc etc. Fibers like psyllium, β-glucan, and pectin absorb and hold onto water, forming a gel-like consistency. This helps to soften stools and make them easier to pass. When recommending fiber supplements for regularity, it’s crucial to choose fibers that: • Are non-fermentable (e.g., psyllium). • Have a demonstrated high water-holding capacity. • Have clinical evidence supporting their effectiveness in increasing stool output and improving stool consistency. Professionals should not assume all fibers provide the same benefits. They should consider the specific properties of the fiber in question and recommend those with proven efficacy for the desired health outcome.

Omg this helped me so much! I forgot to take it the other day and suddenly my poop was soooo soft again. Now my fissure is finally healing. Also using Diltiazem which has been amazing! Thanks so much. I will tell my doctor about this when I see him next time

I’ve been there, but has managed to get it under control. Changing my diet to more veggies and drinking lots more water has seemed to help. Thanks dr Chung, and you described it so well I had to laugh a little. My wife asked me what I was smiling about, so I answered «a video about comet-poop» lol.

Thank You for all your helpful advice! You always have such a great disposition and you share your knowledge in such a motivational, fun manner! However, I have recently experienced a HUGE problem because of “pushing” during pooping. My left eye has suffered a subconjunctival hemorrhage 4 times within 5 months due to “pushing” while defecating! I never knew that could happen! And I think that’s insane because I’ve pushed harder in previous years with nothing happening! My gastroenterologist told me that I MUST hydrate often and eat plenty of fiber in order to help my poop exit my system more easily without having to push so much. So unfortunately, I MUST eat a whole lot of fiber and drink plenty of water and prune juice and hope it all helps because I don’t want to compromise my eye anymore! But thanks again for all your super advice! 👍

I don't eat a lot of fibers but comets occur pretty often anyways. The first part is a bit harder, It feels like the flow is misaligned and I need to push to get thru, what follows is at the speed of light I found that a bit of a push and then relax, before the comet reaches the exit, will slow down things considerably and prevent a lot of damage. Who knew that at 56 I had to learn to go #2 again

I had this and I do think it’s the fiber because recently l’ve been very conscious about eating more fiber. Even added chia seeds to my water. The RDA for fiber is 30 grams and that is actually a lot and probably too much for many people.

Healing in recovery from hemorhoidectomy and my poo has been this exact description the entire 35 days I’ve been in recovery. I’ve not been healing correctly and this may be why. I’ve showed my doctor my stool pictures and each time they say to keep up on my fiber. Continue high fiber diet is what they keep saying. After each soft part of my BM stops, my body still seems to be pulsating or straining for a moment and blood comes out. Day 35 and I’m still in pain during BM. Still blood. Still open wound down there. I’m going to lower my fiber in hopes it helps me get back to healing.

I stopped Metamucil and stopped having the comet poos, they would cause them every single day. I also cut out about 75% of my over fibrous diet. I was only eating everything with 2+ grams, now I eat more normal and do leafy green salads with any bad meals. Your the man doc.

@@marielycarvajal3826 broccoli is a cruciferous vegetable but is one I also eat, make sure you steam it or cook it in some way so it doesn't slow your digestion down. Leafy greens I eat are spinach & butter lettuce both have nice low fiber numbers that won't slow anything down.

THank you so much for talking about this stuff. The comet poop is what caused and slowed my healing but ultimately I have not fixed it. I eat a lot of fibre so maybe this is way. First poop is a comet 80% of the time. Second and Third poops after a comet are always thin and soft. I keep trying to explain to my GP that what is the point of proctoscopy etc etc and doing bowel prep or intervention if I can't stop comet pooping!!

You should not need to do a full prep for a proctoscopy. Just taking a bisacodyl tablet 1 hour before your appointment and going #2 should be completely fine. I would try to see if someone can ask the specialist for the answer regarding the prep. I cannot comment on why you need a proctoscopy as I have not examined you. I wish you the best.

A consultant told me...FIBER FIBRE FIBER ....and hes supposed to be the ex-purt!!! Were all different. Its good to make a food diary to pinpoint foods that make things worse. Mirilax has been my saviour...

I was looking for months for someone talking about this issue and couldn't find anything. All I keep hearing from doctors about helping a fissure is loads of water and loads of fiber. Unfortunately I had to learn that this is very misleading recommendation. My fissure got worse and worse because of wrong recommendations (starting with hydrocortisone cream and suppositories and following taking fiber supplements) 🥴😟 I like the comet poop name lol. I finally noticing that what is helping me is adding more fat and protein to my diet rather than just fiber fiber fiber... It needs to be a balance and I wish doctors would say that or at least refer people with a fissure to a nutritionist that knows how to help with this comet issue. Dr, do you have a breakfast recommended meals to avoid this comet poop?

Thanks for sharing. I've heard your exact story from so MANY others. I understand its very frustrating! I find that Comet Poop is from too much fiber. My suggestion would be to keep trimming down the fiber content until the Comets go away. I wish you the best.

I think you may have solved my problem. I cut back a little on fiber. I seem a bit better. I got severe constipation after a bad nirovirus. I got scared and overdid fiber. Now I feel awful with intestinal distress. I cut back a little. I seem better.

Wow so I thought I was helping by making everything soft but it’s been leading to this issue… I used to drink 3 smoothies right after the surgery which made me go twice a day and that probably also didn’t help… 🤦♀️ it’s been two months and still in pain as the fissure seems to swell up into a hemorrhoid after every comet poop which is every day.. since I was eating very low protein to stop the hard stools from the weekend splurges.. it’s a constant battle… sometimes it’s too much protein, too much dairy, now too many fruits and fiber 😑

Very informative! Would you be able to say what is a good amount of daily fibre to aim for to prevent comet poop? I’ve recently gone whole foods plant based and upped my fibre to help manage my haemorrhoids but tracking diet on Cronometer I average 70g fibre a day which I guess causes this! Should I halve this but stay above RDA, so maybe 30g a day?

Hi doctor. A specialist I am seeing for my anal fissures said that I should take double amount of fiber supplements. I have really hard poop at the beginning then soft. Exactly as you described. I am starting to think is because of this high amount of fiber (during the day I also eat a lot of fruit) that my anal fissure is there since 1 month (I am bleeding and having pain since then). What's your best suggestion? Reduce or remove completely the Metamucil?
