Yeah, that cave is straight up alive. It kinda reminds me of the Monument Mythos where the characters in one video go inside a "cave". Of course, that cave is actually a giant serpent. That "cave" however looked moist, slimey, fleshy even. But so does the cave in this video. Even weirder, there are steps. Why would a cave have steps? Unless it was meant for people. Or maybe specifically, meant to lure in people. Maybe the reason the police never found the entrance to this cave is because it's like a mouth, it opens and closes whenever it wants. But it also twists and turns its own inner body in order to make it impossible to escape. But that ending kinda makes me think that whatever this thing is, it's also an entrance to hell itself. The voices and screaming in the background definitely belong to other people. And they sound like they're continuously being tortured in some reddish area. So in essence, this seems to be a living cave that lures people in, traps them by changing its own body, and forces them to go to hell itself.

It could also be like a Sarlac from Star Wars that keeps its victims alive to continually absorb nutrients from them.

This reminded me of As Above So Below. Its called Film Inferno which makes me think of Dante's Inferno. What if the cave is Hell itself, and the entrance to the cave is actually a portal opening up in different places

Nobody's mentioning this but I don't think the cave is alive, I think the cave is a portal to the 'mirror' dimension or a part of it, as an alternate version of people seems to be a reoccurring trend in this series when you tie episodes together. The end of the tape is within the mirror/alternate dimension that you see so often in this series. The version of Tohko that is leading the boyfriend is maybe a 'mirror' Tohko or manipulated by one of the spirits that come from that realm. I don't think this is like Monument Mythos at all. It's like a portal more so than a living creature. The creatures that cause a lot of problems in this series probably come from such a place. Part of me thinks this has a connection with the other video involving the man who draws faces of missing persons, considering he had a long walk, nobody knew where he was going, and him and everyone else disappeared with no bodies. I have a feeling that those people likely ended up in the same situation as these 2.
Use of classical music in such a delusive and ominous manner is a really wonderful touch. For those, who interested: 21:42 31:15 G. Verdi - Aida Triumphal March. 24:35 S. Rachmaninoff - Symphony No. 2 III. Adagio.
