lol I learned to draw blood by shadowing the doctor for 1 day and seeing him draw 3 times. After that I was by myself and rarely miss more than 1 time on veins I can't see. You need to work on feeling them more than seeing them. Lots of patients won't have bulging veins

Mary Jane yeah i have terrible veins so I’m lucky if they get it first try. Since it does hurt more on my arm I get it on my hand which is where my veins are a bit better

Do the palpation process to locate vein . We have more difficult vein than that with only one arm to use . I work with every day 60- 80 pt per day all with IV in radiology with IV contrasts and only 4 nurses till to finish it’s really as challenging. 😭😭😭

Warm water on a cloth will help. Veins rise to the surface when exposed to heat to cool off. If un-sure if a vein or tendon.... release the tourniquet...if it goes away its a vein.

I have been using the corner of an alcohol pad as a "landmarker" for years. Nice to see others doing the same.

Al Pulley that’s a little tedious to use an alcohol pad as a land mark. It’s a nice idea but if the patient cannot keep still or the alcohol pad gets moved, you have to reposition. Plus not every vein is going to have a nice area to put an alcohol pad down. Sometimes you have to use wrist or hand veins for access. I would skip the alcohol land mark step out of convenience. I’m never sticking somewhere I can’t see a decent vein. She’s good but if that alcohol pad moves, you have to reposition your process.

Al Pulley the demonstrator had visual sight of the vein in the video. Plus the person being stuck is not as difficult as some people I’ve seen in my “rookie” experience. If they have a tattoo covering the vein then yes, you have to go by feel alone. I’ve seen the “best” nurses miss veins by feel. It’s just the nature of the practice.

Take your time, FEEL the veins, feel the tube like structures, it's all in the feel. Feel your own veins. There's no rush to any of it.

@@AmFFFers Learn to do it slow and right first time. Rushing is more wasteful and stressful on everybody involved as wrong needle placement means veins are popped, they bleed out, the tourniquet starts bruising the arm... Doing it slow and learning that way is more efficient, as your skills improve you'll be able to speed up after you know exactly what you're doing. It's only an extra few seconds but it can guarantee a successful first go.

Good notes I found in the comments 1.Using double tourniquet, if not sure is it vein or tendon, release the tourniquet.. it will goes away if a vein. 2. Using warm water cloth 3. Taping and compressing

Whatever iv set that was wish we had them, ours after you got the vein, have to thread the needle, then hook the pigtail while holding pressure on the vein to prevent bleeding

WOW my mother was a "hard stick" and as she would always tell the tech "I have tiny veins and they roll" and she had lost a lot of weight so it was ALWAYS difficult for them to get IV's and Draws. I wish I would have known this I would have suggested this method. Miss my mom she was too funny!! :)

As a patient, whenever a nurse or phlebotomist does this I automatically get annoyed & ask for someone else....I understand that I'm a "hard stick" because my veins are tiny & a bit abused but most likely I need an I.V for my sickle cell pain, don't need any extra pain from these nurses heavy handed slaps 😣

Tourniquet were sooo tight, I was told when it's that tight it can damage the sample. ( obviously I'm not referring to IV) also flicking the vein is not at all correct. But damn...great job anyway. I didn't see a vein .

Yeah this tourniquet technique can't be used on everybody, especially elderly. Many times they can't/don't tolerate a single tourniquet very well. She was also not cleaning the site properly (unless she was cleaning to draw blood cultures). And I also agree slapping or flicking the veins doesn't really help and is not proper technique.

Meh - even drawing blood tends to be a 4+ stick nightmare and sometimes a give up. An IV has always needed the ultrasound finder... and even still the few admissions I've had they blow and always need re-doing.

When called to start an IV on the floor, I always told the nurse to wrap the patient's arms, from the arm pits to the finger tips with ideally a warm bath blanket. Makes all the difference in the world. No veins turns into...where do you want the 14 ga.

the ac is there, it's deep, you should be able to palpate it, if not just go for where it should be anatomically, if that fails my favorite location for those that are difficult is the anterior upper arm/shoulder

When I double tourniquet on severely obese patients I lay the 2 tourniquets on top of each other and tie it on their arm as one. It’s extra pressure so it’s super tight but it works 😁

Great technique. Great skills. But I could see a few veins on the dorsum of the hand . It would be better to try distal veins first for cannulas, unless it's just for a draw.

That's the vein that we usually get, but now that one is scaring too. Because I have had IV's so often over the last 17 years, even anesthetist have so much trouble finding a vein that is going to last for 3 days at least with fluids constantly running through. I hate it. It took 5 times last time. My record is 19 with an ultrasound, which the doctor through across the room!!!😳

@@kungfu82able I've had PICC lines quite a few times. But the last one I had, I got septicemia. I haven't got a more permanent port, although it has been discussed and considered many times. They just don't want the chance of me getting an infection or other serious side effects. So I'm stuck with the constant poke and miss. I have IV's every few months, so I can have my treatment for Achalasia.

I have diabetes. All my veins have been collapsed over the years from getting blood draws to check my A1c. Mostly because of bad pokes.

As someone whose on my last day of my phleb class, there's so many thing wrong here. First, you never leave the tourniquet on longer than 60 seconds, you run the risk of bruising your patient. You also wash your hands before you glove up. You do not use an up and down motion when cleaning the site, you go in concentric circles. That way you are pushing the dirt/bacteria away from the site, not back towards it. You do not flick, or smack/pat the veins as that is an act of battery. If I were you I'd take a phlebotomy class to get updated on proper procedure :)

No me ,I’m always wearing gloves every-time I touch pt. skin , and kudos to me that I always get a vein on a hard stick pt. ,wherein the vein not visible and just by palpating only and deep. using either Nexiva 22 or 20 or angio cath gauge 20 or 18. Working in radiology department with 70 to 85 pt a day with most of them with IV contrast . No cockiness here .😅😅😅

Great Draw, STOP abusing your patients! It is completely unnecessary to slap or thump the veins, and that can actually hemolyze cells. Just lightly palpate.

I same applies for leaving the tourniquet on a patients arm. And some times when you release them the vein goes back to being small. Its a win lose situation
