Here's a video of me getting a score of 999,999 points in the Heart Mode of the original Game Boy Tetris from 1989, also clearing 1000 lines on the way. I was majorly clearing Singles, which slows down the level 20 pace by a lot. I've got also a video where I go for the more risky Triple and Tetris clears in normal mode: • GameBoy Tetris 999,999...
Heart Mode can be enabled by holding down Down while pressing Start in the player selection screen. If activated, you'll see a heart next to your current level. Heart Mode means that the gravity (falling speed) is 10 levels higher than usual (maximum level is still 20), although the player doesn't receive more points than usual. So Heart Mode is disadvantageous for scoring purposes.
I started on level 9.For the first 100 lines I played at level 19 speed but just got the level 9 score multiplier. For lines 100 to 109, it's level 20 speed and level 10 score multiplier, and so on. Without heart mode I sometimes reach level 20 with 600,000 points; with heart mode 250,000 points is pretty much the maximum before reaching level 20 speed (and the score multiplier is lower for the next hundred lines).
So, maxing out in Heart Mode is harder to do. However, I would rank a Heart mode max out lower than a normal maxout done in 500 lines or less because for a normal maxout in 500 lines you have to go for Tetrises and Triples in level 20 which puts you in great danger. For example, I got the Heart Mode max-out on my third try where I reached level 20 speed. On the other hand, for the 500 lines max-out I needed much longer.
