Hair transplant without Finasteride in 5 case rports of patients who did not take the DHT blocker after their hair transplant. People think that they can get away from the onslaught of baldness by circumventing Finasteride. Some think it to be a virtue to not take the drug. This narrative is borne out of loss rationalisation or reckoning in the face of hair loss at an early age. Patients cling to any advice which is contrary and holds out hope for their hair, with minimal risk or effort on their part.
Though more and more patients are today conditioned to refusing Finasteride, those that have been through the process, have realised that the advice had substance. They have seen the gains achieved through hair transplant initially for 2-3 years slowly disappearing since they had chosen to go against medical advice.
Samir is one such person who fell for the commercial advice that alternatives to Finasteride do exist. 8 years after his first hair transplant with me, he recently sent me this note:
“ Dr Bhatti, I was so dumb back then and worried about side effects, so I didn’t take Finasteride. I was only on Minoxidil. I am ready to take it now.”- Samir
It is easy to guess his baldness had progressed and he is at his wit’s end.
And he is not the only one out there. Today I present 5 patients who in the long term had only the initially planted hair left and it looked so out of place that they would have been etter off not starting the hair transplant journey in the first place. To watch these 5 cases, hang on..
(Case 1) One of my oldest patients whose complete progression I have with me. He underwent a conservative procedure with me since he had extensive baldness which had already manifested at 25. After the hair transplant he did take Finasteride for 5 months but then he alolwed baldness to ravage his native thing areas. He came to me for 2 more procedure to cover the new bald areas. He regretted the fact that he would have won the battle of baldness if he had taken low dose DHT blockers. In the last procedure I did on him, you can clearly see where the transplanted hairs are and the large bald areas that have appeared. These areas would likely be much less if he had followed medical advice
Finasteride may have side effects like many other medicines have. But it is not a banned drug for hair loss. Nothing that is truly beneficial in life outside hair loss will be without a flip side. But these side effects are rare. The people who are likely to get side effects need to be weeded out by deep counseling and tests to determine sexual potency before starting them on this medication. Pre-existing erectile dysfunction is so common today. 45% men in the age group 25-35 have some form or the other. And this is the age for hair fall. These are the patients who need to be identified and counselled about not taking Finasteride.
At no age is balding so shattering than for a young man at 25. But his baldness more likely than not is not yet stable. In this setting, he does want to go in for a hair transplant but he does not wish to arrest the progress of baldness by using any medications. This is the recklessness of youth which later in life comes home to roost. I have hundreds of past patients who regretted the decision they took about firstly a hair transplant when they were not candidates for one and were warned against it, and secondly of not taking Finasteride after a hair transplant. And this number is building up.
So the choice of making a decision on a hair transplant with Finasteride versus that without is like choosing between fantasy & reality.
Unless advised by your doctor not to take Finasteride, abstinence from this medication is not a virtue.
There is no point getting a hair transplant if you have no desire to stabilise the progression of baldness.
