Thank you so much, Dr. R! After watching this video, I was promoted to Jr. Butcher at the Costco I work at! Just kidding. I started watching videos on the Titan sub CEO, Stockton Rush, then began trying to understand his thinking.. which led to a few Neurological videos. I work with Databases and Web Development in the real world. After 30 minutes of your presentation, it saddens me to think about how much I didn’t know about my own Brain. There I was trying to understand someone else’s, when I don’t even understand my very own. Kudos, and thank you, for all of the effort you put into these videos, Dr. R.

My dad had a hemorrhagic stroke on his right side which was due to chronic hypertension a berry aneurysm rupture in the midbrain. He did have a craniotomy to release pressure but he did have a midline shift and hematoma. The doctors and the neurologist really can't State what kind of syndrome he has or exactly what all has been damaged it's been a very frustrating for us family. He's is now quadriplegic. After surgery he was in a coma for about 3 weeks and then he started making more progress as far as conscious goes vegetative in between minimal conscious. Now he's in and out of it he's not really fully conscious ever but he can open his eyes sometimes he's got a tracheotomy but he can't move his arms and his legs. I don't know if he's locked in or if he just has severe significant brain damage and there really isn't an exact disorder to his condition. It doesn't seem to fall within any syndrome or disorder that I've found so far it's kind of a mix of a little of this and a little of that but nothing concrete exact maybe you can help me a little on what you think it could be. I would be so thankful for your input.

Rosalina, I am so sorry to hear about your father's condition. As far as your question goes unfortunately there is a divergence between what you may learn as a medical student, and what tends to happen in real life. In textbooks, boards questions, and academia, everything is straightforward. The syndromes depicted are always classically represented with features and catch phrases. In the real world of clinical medicine, as you will see on rotations and in residency, there is a grey area to these same topics. Many neurologists will sometimes be able to postulate based on MRI and clinical findings, where a lesion may be located. An example of this in the literature, may be decreased consciousness in the setting of infarction surrounding the reticular formation. I am wishing you and your family the best, and if you need anything, please don't hesitate to reach out. So basically just to clarify...brocas and wernicke's area are in both the left and the right hemisphere, but in order for it to be a truse Brocas or Wernicke's aphasia it has to be in the dominant hemisphere? If the person if right handed, which is the case of most of the ppl, then their Brocas aphasia would be in the left hemisphere since the left side is the more dominant brain..and vice versa for left handed people..... Is this correct..I'm jst trying to summarize my understanding of what you said..let me know if I have it right or not pleasee thank you! :)

Great question! Broca's and Wernicke's area are generally only found in the dominant hemisphere (for exam purposes). In most cases of right handed individuals the dominant hemisphere is on the left side of the brain. If handedness is not mentioned in a question, they are presumed to be left brain dominant. Left handed individuals can be more complicated as they have more proportionate right, left, or dual-brain dominance and this is beyond the scope of most USMLE exams. Unless the exam explicitly tells you the patient is right brain dominant, you can assume the default left brain dominance. I hope this helps!
