@@lullabygoodbye25 I had asthma in the past, but this was something entirely different. With asthma you breath but not enough air gets into your lungs. This time, it felt like I could not "complete" a breath, like something stopped abruptly my lungs when they tried to fully expand. Like being only able to make "incomplete" breaths. It was really, really bad. And yes, it did not change at all with nose or mouth breathing, since whatever it was, it was in the lungs area. One thing that made me realize it could be false dyspnea was that it did not change at all with exercise (with real dyspnea it gets sensibly worse with fatigue).

Thanks Anthony. It was a real crappy ride with this thing and yes, no one gets it! Your circle of friends / family will suggest stress - anxiety and often times a doctor will run a standard set of labs, try an inhaler and then arrive at the same conclusion. I made several videos about the journey with the recent one covering VCD/PVFM which relates in part to the reflux as reflux is one of a number of triggers that cause the vocal folds to close on inspiration, resulting in the sensation and the following panic / worry that something is awfully wrong. All those feelings of helplessness; not wanting / seeing how I could live like this etc. Go tell your doctor some smart middle aged dude from New Zealand says to refer to a pulmonary or ENT to investigate it and rule it in or out. There’s a breathing test also called spirometry that can also help to show a flattened or bluntening on the inward breath, showing the obstruction in the form of a graph. The challenge of course is that often times you go for these tests in between having an episode, a lot of people will find that this happens episodically and seems to be triggered by something. Anyway, thanks for comment, glad you found some relief and I just know it will get better for you as you peel back the pieces of the onion to understand more about VCD, your triggers and some techniques you can learn to ease it out of your life. I’ll make another video one day soon!

Good video Craig. I've had dyspnea on and off since I was a kid and I'm now 75 and still gets bouts of it from time to time. You're absolutely bang on when you say that most of the medical community has no clue what causes it. I've been taking Omeprazole for 20 years and yes it usually calms down the excess acid production in the stomach but there's one other thing I'll add and that is the diaphragm. Recently I started doing exercises on my doctor's orders and as I did certain leg-ups and such it caused the contents of my stomach to swish into my hiatal hernia and cause me some new bouts of dyspnea. A word of warning to those who might have a hiatal hernia to be careful when exercising and bending over. Thanks for the video!
@CraigStewartShares

Thanks Steve. I want to say actually I’ve made several updates to this video. I feel I was incorrect regarding the esophagitis; I think the more appropriate term is inducible laryngeal obstruction; and again - correct, it’s a long road to find a referral to an ENT that may diagnose it correctly. What I’ve also learnt is that whilst acid reflux can trigger the paradoxical closure of the vocal folds on inspiration resulting in the dyspnea; once the vocal folds start doing this; it seems like it perpetuates itself through some kind of episodic pattern. I’m intrigued as to what causes this; others have reflux but don’t have their vocal folds / larynx effected; and in older medical literature they refer this condition as emotional laryngeal wheezing, irritable larynx syndrome & munchausen stridor, and it was almost always associated with conversion somatoform disorder. More recently I have found the work of Allan Abbass interesting in exploring how personality profile may influence whom develops the condition.

I’m sorry you had to find this little snippet of my journey and that you are dealing with this; it’s hard. Your history does sound similar to mine and there are many folks reporting similar symptoms. I’ve since made a few podcasts, most recently describing how the larynx, specifically the vocal folds, can close on inhalation causing causing the sensation of shortness of breath / air hunger. Without a stroboscopy or video endoscopy of the vocal folds when triggered / episodic for shortness of breath to observe the abnormal closure, it’s hard to diagnose inducible laryngeal obstruction / VCD however after 2 years of reading medical literature about shortness of breath; it’s my view VCD has got to be one of the most under diagnosed and misunderstood conditions out there! Gosh, I wish you well in your journey.

Just some extra important thing to know if you’re experiencing these symptoms but don’t get relief from antacids like omeprazol : - acid reflux (or silent reflux) is not always caused by too high stomach acid. Actually in a lot of cases it’s the opposite. It can also be caused by a lack of stomach acid. It’s worth having your stomach acid levels tested so you know which approach to take (omeprazol for high stomach acid) or things like betaine hcl with pepsin with meals or even apple cider vinegar.

THANK YOU SO MUCH for posting this video, because had I not seen it, I'd still be searching...... as soon as I stopped putting all the spicy foods in my gut (to excess) and lost some weight, my breathing is much better now. It can only improve more as more weight comes off. WOW, back to living again and not the daily worrying about it all....

Hi Craig, Thank you for uploading this video! I've been feeling shortness of breath over the past month and have had acid reflux symptoms but did not know they could be linked with each other. It is encouraging me to talk to my doctor about it and I am more at ease knowing I am not alone and that there are solutions out there.

Craig - thank you for this information. For the past two weeks I started feeling shortness of breath and went to the doc and got an inhaler and a Covid test to eliminate that. Lately I’ve been more and more anxious because of the shortness of breath and and I’ve also notice heartburn symptoms too. Your video makes so much since! I’ve Googled some many different articles about this and couldn’t find an explanation on why this was happening. My doctor did a basic examination and concluded that my heart/lungs were fine but of course the anxiety makes you think it’s much more. My symptoms are usually at night so it makes me a little scared of night time. Thank you again for this video. I’m gonna look more into this and ask my doctor for the PPI medication you mentioned. Good day!

Wow I had the same thing... back when I was studying abroad, I often felt like I couldn't get enough air, especially after eating. Note that it was in the middle of the pandemic so all the worries about Covid were an additional factor that stressed me out. Got some severe panic attacks because of the tightness in the chest and shortness of breath. Much later, I read about LPR, changed my diet, and now these symptoms only happen when I eat trigger foods like pizza or fries (which I almost never do). Now that I know the cause, I'm not completely anxiety-free, but it has gotten way less and I notice it's strongly associated with the intensity of my symptoms.

You are a godsend. I have been struggling since February with what was probably COVID and trying to regulate my breath since. I have episodes usually in the evening where my stomach inflates like a balloon, I’m belching and I feel like my throat is coated and it is exactly when I start the hunger breathing. Initially, I thought the erratic breathing was causing the GI issues from swallowing air. Now I am looking at the opposite to be true.

Thanks Tessie you know I want to be honest here and say that this was a first video at about 6 months into my journey with it; I really sympathize it is ghastly. I have made a new video recently about PVFM / VCD which I had heard of when I made this video but for ages decided it couldn’t be anything to do with my vocal chords as I didn’t think they had anything to do with inhalation / breathing - turns out they do and it’s a really interesting and misunderstood condition which dovetails back to acid reflux as a trigger for this condition (in some cases) which is why it will be effective for some and not others. I also discuss how the pandemic may be involved citing a couple of recent medical papers and so forth. Good luck and thanks for watching, wishing you the best.

I am 24 and cannot express how helpful the video was for me. I got my Moderna COVID vaccine sept 2021. The first dose hit me like a ton of bricks which confused me a bit because typically the 2nd dose is the worse. I seriously thought the vaccine was the root cause for my breathing issue. I Went to the ER several times and no doctor could diagnose me. I finally went to see my physician and have been putting off my pulmonary function test she recommend for a few weeks now because I was so scared I was going to be left with more questions then answers. She put me on medication for anxiety in January and it seemed to make my dyspnea more bearable but was not a complete fix. About a month ago it started to get really bad even with my medicine. I was depressed and could be there for my toddler as much as I wanted to because of this issue. As soon as I saw this video I ran to the store and bought $40 worth of prosilac. Im on day 6 of taking it and after almost a year of suffering these last two days have been incredible. I didn’t remember how breathing normal felt. I cannot be more thankful for this. Although it’s not completely gone I feel like I can finally live a happier life and will continue taking this medicine as long as I need too.

Gosh I’m very happy for you, and happy that you can be there for your young precious one at the most important time in their life. I want to say that I did find my journey didn’t end with this video and I believe the issue of acid reflux can trigger the underlying condition which is vocal chord dysfunction / paradoxical vocal fold motion. You can see my most recent video to understand the connection between the two. The VCD seems episodic and the resulting shortness of breath episodes can be brought about by “triggers”, acid reflux into the larynx being one of them and so this is why you have experienced relief. Another trigger is stress and high degrees of performance anxiety; as reported in medical literature, it wouldn’t be untoward to suggest the importance you place in your role as a mother of a young one hasn’t caused you some level of emotional stress that has manifested in the VCD. Certainly the cycle of trying to breathe through the small gap in the closed vocal chords on inspiration makes one feel panicked, and try even harder to take a deep breath, and that spirals into an awful anxiety that is really life changing; for that I sympathize with all. Thanks for the report! Go well, live well, enjoy. ❤️

Over time my heart slowly improved a little but I had very low oxygen and could only walk 50 metres before I was gasping for breathe. To cut a long story short I have seen nearly every specialist , hundreds of tests, did find low feratin and after iron infusion nothing changed. My cardiologist sent me to second lung specialist even though my lungs were ok and he picked up GERD in seconds. Now I knew I have had that for many years and just put up with it, not thinking much about it. I am 78 and now sleep with raised bed head, no pillow and take gaviscon daily. Early days and still unsure but I am also weak from no exercise. I think this has been my problem all along but it has gone on for so long I have a way to get back. Glad you finally got your answers. Hey friend. It’s a tough road for sure. I don’t like hospitals / blood and guts either it makes me cringe, in fact this all started a week after fainting at an audiologists appointment! Can I suggest when you say it comes and goes, it may be that your vocal chords go into spasm and that causes your episode of shortness of breath, as the chords close when you breathe in constricting airflow to a narrow gap. They don’t really know why the chords spasm like this but that’s the nutshell of PVFM / ILO - and the course of action is to get therapy from someone specially trained to teach you specific vocal relaxation techniques and sounds to avoid making, neck positions to hold etc, and through doing this on a retraining type basis you learn to both prevent an episode / attack from beginning or be able to end one more promptly once it starts rather than making it worse as I am sure you can familiarize with, of course one can’t help but worry about it - it seems so intrinsic to life but that worry only further locks the chords and neck muscles associated with the larynx in spasm, resulting in more shortness of breath and a longer “episode”. Hope that’s of some value, I’m no Doctor just your average dude that read far more medical papers on dyspnea and PVFM than I ever cared to. I’ve made other podcasts on the topic and I wish you all the best with the future, Craig.

Hello Craig, I’ve been very concerned lately for my breathing as all this started with the flu, I had the flu and I felt off, and I decided to look up my symptoms and for a while I thought I had heart disease, because I was feeling chest pain, I went to the doctor and he told me I was healthy, lungs were healthy, heart was healthy all of it! So my chest pain went away a few days after my doctors. And I started to feel short of breath which scared me all over again, and I was having constant heart palpitations, and some days would wake up with nausea and I was so scared, but I had another appointment with the doctor coming up, so I waited for that. This appointment was a physical so he checked everything, he told me I was healthy and I asked him about my breathing, which only affects me when I’m thinking about it, if I’m distracted it goes away, my doctor said it was anxiety. The shortness of breath feeling went away but a few days ago it came back worse than before, it’s almost like I can’t stop thinking about it and ever since I’ve had this shortness of breath feeling I never breath through my mouth, I’ve been breathing through my nose this whole time with the shortness of breath, and this feels even worse in the morning, I wake up and feel like I really need to breathe in more air through my nose, and honestly this is the scariest part, because it feels absolutely terrible in the morning, I also got blood tests for my physical and my doctor never called me about it, which obviously meant he saw nothing wrong in the blood tests, I really don’t know what to do this is the only symptom I have currently and it’s so bad, sometimes I cry because I’m terrified something is wrong with me, and I don’t know what to do.

I'm amazed to finally read about Air Hunger as a real thing. My doctor diagnosed me with anxiety, which I knew in the moment was wrong. The air hunger was causing the anxiety, not vice versa. I finally discovered that drinking (chugging) water would almost always solve the issue, which made me realize that my own air hunger was very likely post-nasal drip and/or esophagitis. Now I know that I need to stay really well hydrated and be ready to chug water if I start to feel I need air. I do also take omeprazole these days. Which one (maybe both) does the trick? I don't care. :)

I saw an ENT doctor say that silent reflux can cause real breathlessness for many reasons, inflamation of the esophagus makes airway smaller, a bloated stomach restricts the diaphragm from moving up and down fully/freely, and acid leaking into the bronchial tubes will irritate/inflame them, making it more difficult to breathe. That can also cause bronchitis/pnuemonia, which I suspect was the cause of mine a month ago. I recoverd, then five days later the lpr symptoms came on fast

Esophagitis can be caused from hiatal hernia where a portion of your stomach has slipped through the diaphragm and is occupying your lung cavity and placing pressure on the heart. The secondary result is irritation of the esophagus via acid reflux as the stomach gastric valve doesn’t close properly. Reduce your meal size, don’t eat after 5pm, and place blocks under your headboard legs to elevate the bed to reduce reflux....and talk to your doctor about hiatal hernia.

You might be interested in "The Mystery of My Burning Esophagus" in The New York Times 10/04/2023. This is the passage that caught my attention: "the nerves that transmit pain, air hunger and other information from our organs lead, like telegraph lines, to very primitive parts of the brain that are physically near one another. Because of this proximity, the neurons receiving signals sometimes have a hard time determining precisely where the message is coming from. It might be that any irritation in the esophagus, whether from an upward surge of acid or inflammation spurred by a food allergy, could be interpreted as originating in the lungs - or even the heart - and a body might respond, as mine apparently did, with the panic of someone who’s drowning."

I do notice that when I am really busy, I do not feel it but when watching TV its raging crazy. I am not sure if its anxiety, I was never an anxious person but I think this thing has made me anxious looking for answers every day. Hey Denis. Twinsy almost. Haha. Sorry you had to find me. Good on you for persevering to watch the other podcasts I made which show the progression. Acid reflux is a trigger and as I learnt later, it causes irritation to the larynx and the closure of the folds on inspiration. Your comment of the breathing specialist telling you that you forgot how to breathe; I’d place money on a bet as to who that was and I couldn’t disagree any more vehemently as to her diagnosis - she mentioned nothing to you about the larynx / vocal folds and I can almost assure you if all tests have come back negative through your various appointments so far; it’s the ever elusive inducible laryngeal obstruction. It’s been called so many elusive terms. Wandering uterus, hysteria, conversion disorder, munchausen stridor, emotional laryngeal wheezing, vocal chord dysfunction, paradoxical vocal fold motion, and now the correct term; inducible laryngeal obstruction. With your quoted 18 month time line I can’t help but query the chance of the condition being associated with post viral association with covid19 infection although there’s really no way to know. As I also eluded in my other reply, the case for personality profile is well associated and documented in the literature as affecting type A profiles: highly motivated, intelligent, competitive and self critical qualities - often affecting those in performance sports, medicine and the military according to the research I’ve “waded” through, and there’s been a lot over the last year. Ask me anything, I dare you! Haha. I know way too much about this topic than I had ever cared to, I can assure you of that.

Hi Craig, thank you so much uploading this video. I have been struggling with this since July, and it got really severe in September. The only difference is I have been taking PPIs since april because I started with other reflux symptoms originally. After a lot of ER, different specialists and examinations (similar to yours), I ended up with the Nissen Fundoplication examinations that indicated it was very advisable to get this anti-reflux surgery. I got it on 25 November, and sadly my symptoms have not gotten that much better yet. Although I am on Pantomed (80mg daily) currently, I have a strong feeling this medicine isn't the right fit for me and you inspired me to start up Omeprazole instead. It's been a rough couple of months and I can 100% relate to the depression/anxiety issues caused by this tightness and how annoying it is the doctors almost instantly refer to this as the cause of the problem. Thank you for inspiring me Craig! Good luck to everyone else dealing with this issue.

Recently I was suspecting something was wrong with my esophagus and the same thing that you mentioned about the nerve endings fooling my body into thinking I had false shortness of breath. Every time I wake up in the morning I do feel a slight feeling of burning in the esophagus area. It seems that taking PPI's has helped your condition a lot. One question though, after you fully heal, can you eventually get off the PPI's? I was wondering if there were ways of naturally strengthening my esophagus entrance + healing the esophagus with PPI's. How has your condition improved or been since? Peace and love to you my friend!

Hey Elliot mate I had a relapse a little later like 6 months it came back even taking reflux meds, I made two other video updates covering my journey with this and I am going to make another one shortly. After all this time now I think it can be related to acid reflux but I think the diagnosis is either going to be costochondritis or paradoxical vocal fold movement (which can be caused by reflux amongst other things, including a few recent medical papers suggesting covid19 infection in laryngeal complex). You either want a physio and supply them the costo info from the video I made or the back pod website or a referral to a laryngologist to see if they can observe and diagnose PVFM and refer to a speech therapist (or consider Botox therapy). Good luck getting a diagnosis and getting it sorted Elliot.

@CraigStewartShares
2 жыл бұрын
James I am sorry so slow to respond, hope you are better! Yes, sometimes omeprazole will work; it depends if reflux is the cause of your shortness of breath. When I made this video I didnt understand the pathophysiology but now I understand acid refluxes into the larynx causing a condition called VCD or paradoxical vocal fold motion - its this closing of the vocal folds on inspiration (inhaling) that give the sensation of not being able to get a deep breath; I made a new video on this explaining. Albeit, if acid reflux was triggering this for you, and you treated the reflux - then you'll be long since happy as larry although I think there is a larger piece here in relation to mouth breathing / breathing patterns that influence a sustained fight or flight response, effecting our digestive systems and contributing to reflux related problems. Nose breathing is where its at, once you get the immediate shortness of breath problem under control. So much to explore in this topic! Thanks for commenting.

Thanks mate 👍
@CraigStewartShares
8 ай бұрын
All good buddy and look my view on the subject over time has evolved and I would say to you that the term pseudo dyspnea, coined by one individual to my knowledge, really caught on but is an umbrella term to describe a variety of conditions that affect the upper airway. You might also say it’s an upper airway disorder, how that disorder is caused may be a range of things and is for a doctor to ascertain through a process of differential diagnosis. There appear to be a range of causes from reflux into the upper airway through to panic disorder, post viral infection, noxious odors and so on. Ultimately the physiological response I was dealing with and suggest many that watch my videos on the subject are dealing with also is the abnormal closure of the vocal chords on inspiration, creating a narrower gap for air to flow through resulting in the sensation of tightness of the throat, and that feeling of not being able to take a deep breath. The answer to this problem is a little more complex than understanding it, and I don’t think there is a doctor alive that actually has the complete picture; much like the other condition I manage incredibly well with a natural approach, cluster headache. Medicine is good at looking at things in isolation and treating symptoms, a holistic approach is to look at the larger picture and realize a multi faceted approach may be required. As an example, if the condition of vocal chord dysfunction is triggered by reflux into the upper respiratory tract then the question is, what may be causing the reflux which leads to an inspection of diet and a greater need to understand the interactions between the diet and the micro biome. All I can tell you as a piece of advice is to advocate for yourself and seek the advice or referral to an ENT whom can diagnose what may be happening in the upper respiratory tract and take it from there. Good luck mate.
