That TransAsia accident brought the death toll to 91 in just seven months. I hate when airlines go out of business, but if you’re not willing to put in the time and effort to keep people safe, you shouldn’t be in business. Human beings are always more important than money.

It's difficult to imagine how, after 9 months of training with God knows how many dozens or hundreds of people involved, no one ever talked about the possible consequences of activating feather mode too early. For such an absolutely critical phase of the flight, getting things correct would seem kind of important. Yes, the pilots knew what would happen if they activated too late but c'mon. This was a colossal failure.

Training programs do focus on crews not acting immediately before they can adequately review the situation. This has caused so many accidents that they have a lot of case studies to go by, as you can see here.

The problem with most of the pilots (or anyone) is that most of their career life span, they operate the aircrafts without incident. When things go south, the pressure of carrying life and death of so many people on board with a very limited time, they tend to introduce more errors into the equation. The number of hours under their belts become irrelevant
