One of the ways I use is creating relationships between npc that can change their opinion on npcs they meet. For instance habe 1 guy say how great this Smith is, and the Smith says he absolutely hates the guards, and continue this train

I always try to have one unique feature in cities that I create. The one that I'm probably the most proud of was a city that used to be three large districts. One of those districts was destroyed, and the remnants of it housed undead where the upper classes used to live. The middle district had become a staging ground for defending against the undead, very unsafe. So what happened in the old lower district? They built up, so there were four or five levels to the city with various paths above the streets. And they used what they had to build bridges. In a city that had been famous for its textiles, rich cloths no longer held their value, so they would lash the cloth together as ropes to build bridges over the various levels of the city. It was a bright kaleidoscope of color, but the overcrowded nature of the city made it equal parts somber and beautiful. The one idea that the entire city came from was the idea of what people would do if a textile industry lost value suddenly. Everything else branched from there.

I love with Eberron that if i use calender events and time the characters arriving in the city or town at the right time there's always something interesting to see and do. We recently had characters arriving in Sharn around Crystalfall and was just a delight to describe the festival atmosphere around it and do something similar to a Japanese lantern festival (but with hand carved crystal spires) and just making Sharn come to life with just letting characters enjoy the event and sightsee for a session, throw in some places of interest that they can explore in the next session.

what i do is i get inspired by other things. made a D&D world that i have tested in very small games and that i am now gonna make a longer campaign in. it is inspired by a lot of things like Dinotopia, Dinoriders, Tomb of Annihilation and Overlord. :D and also use the piety system to make different big factions that the players can interact with to give the world more life. the piety system is great to give the players a feeling that they interact with the world, and the world interact with them as they get more known :D

Interesting, I think I do something similar with a morality point thing inspired by New Vegas for each faction they meet (for example, openly stealing from a Knight-Captain probably will appeal to your patron if it’s the god of tricks or thieves, not so much with the tight-knit Order you just attempted to steal from). If you don’t mind sharing, how does your piety thing work?

I also just have some interesting rumors and details. I love to think about what magic would really do to a society, another kind of fantasy realism I really like. For example maybe the clerics are running low on diamonds so they are in a crisis because children that would have normally been resurrected are now dying. To bring such an event to live I would have the city have a minute of morning that surprises the party as suddenly a bell rings and everybody in the tavern is silent for a moment. Or the teleportation system has now been implemented and everyone is talking about how it’s so good to not have to walk everywhere.

6:45 - my team loves stumping me! Whenever they do, I have a magic 8 ball that gets shaken. They love knowing I don't actually know all the answers to everything, and that they can occasionally find gaps in how I expect the team to respond to situations. Good DM practice: prepare for the obvious stuff, know enough about NPC intentions to wing the rest.

I recently published a short on the dmsguild that included 3 carnival games: a dart game, a ring-the-bell-with-a-hammer game, and a shell game. It was even played by a streaming group here on KZitem (Might Misfit Menagerie) and they seemed to love it.
