Hi, thanks so much for all the great quality video & info on ortho. Wonder whether it is worth to buy the smart clip self ligating system book? Cause I saw online, the comment is that it is just an expensive user guide for smart clip. Is it worth buying for someone new in ortho? Besides, is the bracket positioning for smart clip brackets the same as the conventional brackets? Any good practical book to recommend for ortho learning?

Bracket positioning is the same for all types and brands of brackets and getting the brackets positioned perfectly is the second most important thing in the entire treatment process (the first most important thing is a proper diagnosis and treatment plan). If you can position the brackets nearly perfectly, the case is exponentially easier to treat. About the book, in my opinion the only useful part of the book is the chapter on bracket positioning but that chapter is so well written and complete that it makes the purchase price of the book worth it just to get that chapter. But that’s just my opinion. The video is a fairly complete summary of that chapter but not 100% complete.

excellent video Dr, I have a question, what happens when we position the anterior brackets in that place that is very close to the incisal and we begin reading the torque with rectangular arches, according to what I read, many brackets are manufactured so that they go to the center of the clinical crown so that you can correctly read the prescribed angulation, greetings

The absolute height of the brackets on the teeth is not as important as the relative height of the brackets on the teeth, meaning that all of your brackets can be placed higher or lower on the teeth as long as they are the same height relative to one another. In other words, the lateral incisor bracket should be about 0.5 mm closer to the incisal edge than the bracket on the central incisor, so that central incisor ends up being about 0.5 mm longer than the lateral incisor. As long as those relative height differences are maintained, you can place the brackets as low or as high as you like. The natural curvature on the facial surfaces of the incisors will effect the expressed torque of the teeth slightly but not a lot, and the results you see will help you decide where you like to place the brackets in your office. Most bracket placement tools measure from the bracket slot to the existing edge of the tooth. However, at the end of almost every Ortho treatment I will adjust/smooth the edges of the anterior teeth with a high speed handpiece so the final edge is almost never exactly the same height as the original edge. For this reason I use a perio probe because it allows me to place the height of the anterior brackets relative to the final edge that I expect to have after the smoothing rather than to the original edge that exists at the bonding appt. I hope that makes sense.

Different brands of brackets are slightly different sizes but I’ve never seen a brand that was too big for any teeth I’ve ever treated. Generally, you would not place braces on anterior primary teeth and those are the only teeth I could imagine that might be too small.

The position of each tooth relative to the other teeth doesn’t matter when you’re placing the brackets. You just want to place each bracket on its tooth as close to ideally as possible, then the NiTi wire will align the teeth.
