When it comes to steaming silk shirts, I always suggest that you start with the back of the shirt. I say this because you don’t want whatever might come out of the steamer on the front of your shirt. Start on the back near the hem of the shirt first. If you are new steaming items it isn’t a race, take it slow, but not too slow because you don’t want to burn the shirt. When you are new to steaming or using an iron or steamer from a hotel, I suggest putting a pillowcase over the steamer. This pillowcase will catch anything that might come out of the steamer. Once you are comfortable with steaming the back of the shirt move to the front, again start slowly, and move your way down the shirt. Another technique when it comes to steaming is placing the steamer inside the garment and move it down. I would not just go down the garment from the outside, I have found that it is not as effective. You will have to use the steamer on the outside of the garment in part of the shirt like the sleeve.
