Great discussion. I have a question for both urologists and pelvic floor therapists. What is your opinion on how sacral neuromodulation can help with pelvic floor dysfunction? I went through medications and then lots of physical therapy, then my urologist put me on a trial for sacral neuromodulation and it was determined the first two stages weren’t helping with my condition. I’m curious if sacral neuromodulation helps with pelvic floor dysfunction immediately or it’s more of a long term effect

Great question, and we'd love to hear from urologists and other providers on this topic as well. Our understanding as a PT is that sacral neuromodulation stimulates the nervous system to reduce symptoms. It does not directly address muscle dysfunction. However, a reduction in symptoms may take stress off of the pelvic floor muscles and allow them to respond better to physical therapy over time. Since we are PTs, we only get the see the patients who chose to do sacral neuromodulation and PT at the same time.

Sex isn’t painful at all for me but I have uti symptoms with or without bacteria. Could I still have pelvic floor problems without painful sex?

Absolutely! Symptoms of pain before, during or after urination can be symptoms of pelvic floor muscle disfunction. They can also be symptoms of hormonal deficienies.
