Control bleeding, assess airway & respirations, c-Collar, administer high flow O2 , treat for shock and get him rolling fast

Collars aren’t used anymore atleast here in the UK for c-spine immobilisation because evidence suggests they can be harmful. Headblocks, orthopaedic scoop, and tape is more beneficial to prevent further c-spine damage.

@@KJR-vh3rh We do still use them in the us but we do have protocols for clearing c spine so that they don't have to be used super often.

15 is going to run out pretty quick, looks like he’s riding motocross, and is probably a long ways from a trauma center, his SPO2 is 94, you could raise it up a little bit, so maybe on the higher end of an NC. Not sure if onboard O2 is gonna last that long either.

Control bleeding with direct pressure. Verify airway is patent and does not have blood in it, make sure lung sounds are intact and equal, listen for fluid in case blood as gotten into his airway (damaged trachea). Large bore IVs. Fluid bolus, I wish I had TXA. Call for helicopter, we are an hour away from trauma center. Tell Lifeflight to bring blood.

Spinal stabilization, bilateral large bore IVs and blood if I have it. 02 2L NC, keep the patient warm. TXA 1gram over 10mins in 100ml NS. I’d like to give some pain meds but not sure since patient is altered and may have a TBI or spinal cord injury.

Last I checked trauma patients don’t get “2L by cannula.” He is in shock? High flow o2, cut every bit of clothing off him till you find the bleed, check lung sounds and trachea deviation to be sure he doesn’t need decompression, 2 large bore IV’s, fluid bolus, and the fact you’d consider giving pain meds with a pressure like that despite the possible TBI and Possible spinal cord injury is concerning. You’re missing a lot of critical steps.

From Advance EMT stand point (the middle child) Scene safe, consider Spine reasses Airway every 5 min, give high flow O2, treat for shock, start IV, 500ML of fluid Bolus (first round) rapid transport to nearest trauma center otherwise request ALS intercept if necessary

Call ALS, c-spine immobilization, maybe remove the helmet, Treat for shock, O2, trendelenburg, and thermal regulation. Reassess every 3 to 5 minutes.
