I'm 42 and just had a yag done to eliminate a pco. It made me wonder given my age if it would be possible to replace my iol with the bag open if it became needed. Question answered.... very well done.

There is something that is being glossed over here. Dr. Wong doesn't seem to explain why the left eye was corrected to -2.50! That is really a very large degree of myopia for a symfony IOL. I have never tried any more than -0.50D of myopia with a symfony. I am assuming that this must have been unintended and is a large part of the issue. Why was an attempt not made to correct this residual myopia with touch-up lasik? The other thing that is not clear is why a Yag capsulotomy was done in the left eye. In cases in which there is a reasonable likelihood of a lens replacement (such as an obvious refractive error) I would have hesitated before doing the Yag because it obviates the possibility of a lens exchange. Overall the surgery is very nicely done but there is a high likelihood in these types of cases of vitreous loss and all the attendant complications. Personally I am a big fan of monovision especially in patients who have had years of monovision with lasik or contacts. Another alternative would have been to correct the left eye with the symfony to -0.50 using either lasik or a contact lens.

You bring up some good points here. I am assuming two things; 1. that he had a refractive "surprise" from post LASIK A-scan readings; OS and 2. that Dr. Wong did a CL trial to correct the myopia of OS. (but who knows; maybe she was too flat or could not tolerate a CL? Or maybe her pachs were too thin - but prob could do -2.50 PRK enhancement if she liked the CL). Replacing MFIOL with MFIOL with open capsule seems aggressive; but he is a good surgeon; and did a great job. Appreciate how he is putting these patients on line for the world to see. Im learning a lot watching his videos.

Interesting! I am wrestling with this decision at the moment. So you prefer having on lens monofocal for near and the other lens monofocal for far = monovision? I already have a monofocal installed in my right eye. It keeps me myopic, as I do a lot of computer work. Now the lens in my left eye needs replaced and I am trying to decide between a monofocal that allows me to see distance vs a multifocal like the symfony or panoptic. It's a very difficult decision, so I'm curious why you feel monovision is the better option.

Dr. Wong is much less conservative than most doctors. He seems much more willing to use multifocal IOLs than most doctors, especially when the patients have issues with them. Many doctors will only replace a multifocal IOL with a monofocal IOL if a patient has any issues with the multifocal. I wonder if Dr. Wong does a higher percentage of multifocal IOL implants than most doctors.

is there any consideration for a class action lawsuit against Symfony? My wife was told she would not need glasses after this very expensive surgery. She did not need reading glasses prior to her surgery but now requires them as her near vision is now very bad. we will sign up if one pops up.

I hope the doctor indicates whats the maximum time to have an IOLX , could that still be possible after years or just after a few months of the first IOL implant? I had an mono-focal IOL February 2023 and contemplating to replace them.

This is my biggest worry about lens replacement. I am very picky about my vision I dont think lens replacement is going to make me happy even when its something I have to do it.

Dr. Wong: Very interesting and difficult case. Can you tell me if this patient's right eye was her dominant eye? I am very curious since if this is the case, her uncorrected vision in her right and left eyes are almost identical to my own as I prepare and contemplate cataract surgery. The only difference is that I have had no previous eye surgery. I am considering both the Symfony and Pan-optix lens. I am a bit concerned about the possible difficulty in my brain being able to process the different specific focal points of the Pan optix lens outweighing the advantage of its ability to provide near, far and intermediate range vision. At this point I am thinking of using the Symfony providing ability to focus at all distances in my non-dominant left eye, and using the Pan optix in my dominant right eye. Thanks!

Dr. Wong, I had panoptix implanted in both the eyes recently and my eyes feel dry all the time and get burning sensation under the eyelids and discomfort similar to when I used to experience with extended contact lens wear. Is this a usual experience people undergo.please suggest.

I had to have Cataract surgery as a precursor to a FOV. I also had lasik in 2018 for distance. And used readers for reading and up close. A monofocal lens was placed into my left eye. After both surgeries my left eye (cataract eye) is major nearsighted and glasses will not give me distance in my left eye. So I ended up with Monovision and do not like it. The FOV dr did the capsulotomy during the vitectomy. They plan to exchange lenses but there is a special process to do this since the back of the bag is open.

I have a pan optic lens in both eyes, since Jan 2024, I have a lot of glare , to the point of not seeing can Lear because of the glare, insult the rings, where can I get more information, Iam 80 years old, have good health, a lot of faith and hope done one can help me, my dr says to give it more time, I believe I have been patient, please help me, am even woo to ho to Austin .👍😊

4 years ago the optometrist said I was getting a cataract in my left eye....that explained why I thought someone was sucking the light out of the world!! then two years ago..exam showed the cataract in left eye had advanced and one had started in the right eye....I am now 65 and feel like I am not enjoying life as much because of this....oh, I had perfect vision until I turned 40...seemed like the day after my 40th birthday I needed glasses...the strange thing is that no optometrist has been able to write a prescription that corrects my vision..and that problem is exasperated by the cataracts....my question is, will new lenses correct this problem in addition to the cataract problem?

Dr. Wong, given that Brigid originally had Lasik in 1999, when you did the Lasik enhancement after the placement of the symphony lens, did you lift the original flap or create a new flap?

Not so much for epiretinal membrane, but definitely increases the risk of retinal detachment, especially in myopic eyes.

Any intraocular surgery increases the risk of RD and epiretinal membrane, despite this, the rates are very low

Does PCO make it difficult to exchange the IOL due to fibrosis? I want to exchange my Symfony to Pan Optix or Synergy when it arrives here. I am 2 years post op with untreated PCO and never had good near vision with it.

I find that with diligence and generous use of viscoelastic most lenses can be explanted safely indefinitely.

If your PCO is visually significant, then consider a YAG capsulotomy. Disclosure: I don't use the LAL.

@@ShannonWongMD Dr.Wong , the reason i choosed the LAL was because the ability for the surgeon to make up to 4 adjustments post op, whereas typical lens implant you are stuck with what you have, i had a retinal detachment surgery in 2018 and 6 months later i developed the significant Cataract, i addition i also had Lasik done for that same eye in 2013. The Cataract surgeon stated that the Lasik procedure made it more difficult for him to give me the best outcome from the LAL. Dr. Wong do you offer consultation for people out of state ?
