Does anyone else think this story is so dumb. A hospital gets a new hire and there is no orientation. And the rest of the staff is angry at the new doctor for not knowing what he was never told. It's hard to get into a story like this when there is no kernel of truth to build on.

What if, The doctor NEVER tells newbies the truth, so when the time comes, doctor can make a deal with the grim reaper, to take the newbie's family member, instead of a rich patient on the verge of death? Makes the rich patient sign papers donating big bucks to this hospital (or give their word or such)? See, the doctor said he's done "give a soul to rescue another soul", many times before, to rescue rich people by sacrificing poorer people, to keep the hospital financially thriving. The new workers aren't told the truth, on purpose. The doctor probably felt the need, to have a "way" of selecting which worker's relative shall be the next sacrifice, hence he decides whoever displeases him shall pay the price this way. Ignorant workers are easier to prey on. This protagonist smartened up quickly and rescued his wife before it was too late. By the way, that dude in the 4th floor with the scythe, two dark voids for eyes, is the grim reaper, for those who missed the details.
