Hi Dr. D, I haven’t posted in a while. So, I saw this and I thought I would give my opinion as well. I’m very proud to say you are MY optometrist and you are amazing. My experience with IPL is great. I look forward to seeing you each month. It’s painless for myself and I feel like it’s working. I’m so thankful for you and your knowledge. You always have a new “surprise “ for me each time I see you. I appreciate you for continuing to learn and help people with eye issues. It is a journey, however I do believe it’s worth it. I was so lost with what happened to my eyes until I found you. I look forward to our next visit and you have my permission to “zap away”. Yes, I definitely want my own celluma. I just know I have a road ahead to be where I can be, with your guidance and help. In the end, I will look 30 years younger! All because of you and your continued concern and care for your patients. I've had several IPL treatments, but my provider never uses it on the upper eyelids. Would that make a big difference in the results? So far it hasn't been very effective for me.

i prefer to use it on upper lids. Providers have their own opinions on this but my patients do very well.

@@EyeSchoolwithDrD Ok, thank you. I raised the issue before with my provider, but she said it doesn't make a difference whereas I kept wondering why it wouldn't. Now I just found a new study that found more improvement with both upper and lower eyelids treated. I will definitely raise the issue again, see what they say themselves.

How does IPL work if you have external eye shields (the stickers), it blocks of the eyelids from the light presumably and so the blood vessels on the actual eyelid and near the eyelid margin arent treated?

If treating chalazion/stye/hordeolum I do 2. For dry eye, the protocol is 4 and then maintenance sessions are required every 6-12 months.

Dr. D, I have 2 questions...(1) Why is the entire face NOT treated w/IPL? Many of us w/ocular rosacea also suffer w/facial rosacea that goes all the way down to chin area. (2) Does the lower half of face NOT treated eventually lead to telangiectasia spreading back towards upper cheek/eye areas?

THere was a study that looked at this. Toyos was one of the authors. Basically they compared eyelids only vs. eyelids + cheeks vs. full face. You DO NOT get the same results with eyelids only, which really points to the fact that the pathogenesis of the inflammation is in part coming from the increased telangiectasias on the cheeks as well. So yes, cheeks are a must for the dry eye protocol.

That got me too, no gloves, no mask while talking so close to the patient, and in the last bit - no glasses either 🫤 those things that were placed in the eyes were just sitting on the shelf on a paper towel (no sanitized container?!). Is it all too safe that none of the precautions were needed?!

I don't wear gloves typically in clinic or for this procedure and I do not believe that most doctors do. I would of course if the patient preferred but there are no open wounds/we are not puncturing skin. The shields are sterilized. Antibiotic ointment is instilled after the procedure as well. I did not wear a mask during filming because I wanted to narrate. It is very safe, I have had no issues with infection after doing it for 2 years. But I will stop placing sanitized shields on the paper towel, that was a good point so thank you! Also, the goggles are my own poor choice. I can't see the skin response well enough with them on so I often do the procedure without. I am in control of the button so I close my eyes with every pulse. Again, its a choice...not a good one but I know that. Bystanders and patients always have theirs on/in. :)

Dr D is amazing. I’m sorry anyone would be so critical. I’m proud to say she is my personal optometrist and she is literally taking me from hell! She’s incredibly knowledgeable and caring. So, get over it!!! Her office, her team are all extremely kind , helpful, and knowledgeable of everything she does. She has a beautiful heart, staff, office, and goes above and beyond to help hurting eyes and people!!!

BTW, when she does my IPL my son is always in the room. She makes him wear dark glasses and she knows when she’s going to do the IPL and closes her eyes. Regarding the “things laying on the eyes “ get the facts. She actually puts them under eyelids and no way you see anything. She’s gentle and follows all precautions. I know because I get IPL from her each month. She’s NEVER hurt me in anyway!!!

I will correct the eye shields statement. Dr. D is extremely sanitized shields. I never have had one negative problem from any of her treatments, including IPL. I believe God put her in my life and I travel 500 miles one way for my visits. I’m looking forward to the next. Thank you to all in your office.

I had my first treatment today. My eyes are less red. Is it normal to see a flash when wearing shields ? I had small sized shields inserted.

very normal. can still see the flash even with laser grade shields in. always a bit disconcerting but yes, this is standard.

I’ve now had 3 treatments , glands expressed and steroid drops. My eyes are less red for a few hours then the redness returns. I’ve had the settings increased on each procedure. The dr mentioned something about 15 “setting “ not sure what type of machine he’s using. It does not cause any discomfort. I wish your office was in Los Angeles. I’m not sure if I should continue after 4 treatments if I do not notice any improvement .

Horedolums get noticeably better after one treatment and depending on size can be totally gone after 2. For dry eye patients, I usually tell them they won't start to see improvement until treatment day 3 or 4.

Hi Dr. D,, I bought the popular ULIKE IPL hair removal machine for home use. I read the directions and test patched a spot on the side of my neck. I was wearing the IPL glasses as tightly to my eyes as possible, but there are still gaps in coverage due to the fact they're IPL glasses and not goggles. Are the IPL glasses safe enough? My eyes feel slightly odd after like an old fashioned camera flash. I'm regretting not closing my eyes tightly. The directions that come with it don't tell you to close your eyes as well, is that necessary? Going forward I would do it though. I'd greatly appreciate to hear your opinion, thank you.
