1. The success of hair transplantation depends not only on graft harvesting, but on proper distribution of grafts in the recipient area.
2. The density, direction, angle and pattern of slit (or site) making affect the final show of hair transplant.
3. Slits are three dimensional holes in recipient area for insertion of grafts.
4. The slit size must mimic the dimensions of the grafts.
5. The utmost care must be taken while making sites to minimize linear damage to the skin and blood vessels.
6. Using cut to size blades reduces the linear damage and controlled depth of slit creation prevents the vascular trauma.
7. The slits perpendicular to hair growth are called coronal slits.
8. The slits made parallel to hair direction are called sagittal slits.
9.When rectangular blade is used to create sagittal slits, the slit will be deeper compared to coronal slit. The size of skin incision will be bigger than that of rectangle cut edge of the blade.
10. The slits created by hypodermic needle are deeper because of its beveled portion, and potentially induces more vascular trauma.
11. Coronal slits with depth control create the least vascular trauma and linear damage to skin as compare to a rectangular blade creating sagittal slits.
12. Proper anesthesia to assure a pain free implantation and tumescence to get good hemostasis will make the procedure easy for surgeon and comfortable for the patient.
13.Tumescence is a solution of normal saline having low concentration of local anesthetic agent like lidocaine and or bupivacaine or other with epinephrine, and with or without addition of steroid.
14.Infiltration of the tumescence solution should be above the galea and never below galea aponeurotic of scalp.
