I always thought of the TIBC not at a true number but as an implied number, very much like calculating the "indirect bilirubin. Transferrin can only carry so much iron, & at any given time, transferrin will have some "available" iron binding sites not presently occupied by iron. That is to say, with IDA & low iron everywhere (serum iron, ferritin/storage iron), then the amount of iron being transported on transferrin is also reduced with the resultant increase in the open & available sites. Kind of like there's a finite number of seats on a bus & with fewer passengers there are more empty seats. So, to answer muneeb khwaja's question, IDA means that there are few seats occupied on the transferrin bus (low transferrin saturation) & more seats (TIBC) available.

That's a good point. Transferrin saturation = serum iron / TIBC. Therefore, there's an inverse correlation between TIBC and transferrin saturation, provided that serum iron remains constant.

@@varshitharisetty8911 transferin is meant to be a transporter of iron, so in anemia of chronic disease, body does not want to give pathogen the iron so that's why IL-6 inhibits liver from producing transferin(TIBC decreased). Therefore no transporter = no iron in the vascular = no iron for pathogen and to the transferin saturation = serum iron/TIBC, reducing serum iron plus TIBC can make the trans saturation value still normal or if the serum is reduced more tremendously than TIBC, the saturation can be reduced as well

Thank you for the explanation. However many doctors in the UK only test RBC and reluctantly ferritin. If ferritin is low they prescribe additional iron. What you are saying is that without Transferritin saturation you cannot even hope to estimate if that client is truly iron deficient? It is rare that doctors do a full iron panel or even understand how to interpret it. My ferritin has always been low (46 at highest) and often in the twenties yet I have been told (in Australia not the UK!) that my iron saturation was absolutely normal. Clinically though I get symptoms of exhaustion is my ferritin falls into the twenties....so I go of symptoms.

I seem to have the same issue. One time cooked liver helped but it seems hit or miss with the liver quality and cooking it perfectly is difficult. Raw is not bioavailable so don’t take Liver King’s word

From what I've understood so far, iron can be essentially kept in 3 places: in red blood cells (hemoglobin), serum (bound to transferrin) and stored (in ferritin). So low ferritin alone seems inconclusive, as you could technically have a lot of hemoglobin using it instead or it could be being transported in transferrin (to be used either in hemoglobin or stored in ferritin). Don't know what the proportions are here (e.g. how significant amount is serum iron) and what other factors can play into this, though. Another interesting question is how is ferritin measured? because it appears to be mainly in places such as the liver and bone and not in the blood. Maybe it can be derived from amount in the blood. It's also unclear how we derive amount of stored iron from amount of ferritin, which don't have to match necessarily.

My results are Iron 27 ug/dL Iron binding capacity, unsaturated 257 ug/dL Total iron binding capacity 284 ug/dL Transferrin % saturation 10% My doctor doesn’t seemed concerned if my results but I’m worried about the low transferrin level.

So why do you get high TIBC in haemochromatosis? Here you have increased transferrin saturation, raised transferrin but a low TIBC? EDIT: I get it now. In haemochromatosis, you get iron overload so... --> higher ferritin (you want to store this excess iron in a non-toxic form) --> lower transferrin (decreases as a homeostatic response) === lower TIBC (indirect + cheaper measure of transferrin) --> higher transferrin saturation (you have less transferrin but more iron) Altogether you get: High iron, high ferritin, low transferrin/TIBC and higher transferrin saturation.

total iron-binding capacity (TIBC) is the ability or capacity of transferrin to bind to iron. so think of transferrin like a "taxi" transporting iron. when you have low iron, there will be a higher amount of transferrin in proportion to that iron, ready to bind to iron, i.e the transferrin binding capacity is HIGH therefore TIBC is elevated. another way to explain it is the capacity for the "TAXIs" to occupy or bind iron is high because there is few iron available in proportion to the "TAXIs" or transferrin.

I just got results, but can’t see my dr till next week. My Transferrn Saturation is 45%, does that mean Hemachromatosis? I feel like I’m carrying around a bunch of iron literally, I feel heavy. It’s miserable. My Hemoglobin , Hematocrit Bilirubin are borderline high. AST ,175 ALT 184. I always learn more here, so I thought I’d ask. Oh and could medication cause any of this?

Sirisha Laxman when my ferritin levels get low the only thing that helps are iron infusions. I get three, one a week for three weeks. When the levels get low another round. This time drs didn’t catch it in time and I have been down too long. Start cycle on wed and should feel lots better soon.

Iron is stored in the macrophage on a protein called ferritin...So, you can say, it’s iron stored in macrophages, or it’s ferritin stored in macrophages...I don’t see any difference, it’s just words.

If I have hypothyroid (auto immune issues)….and my body needs iron desperately. Does my body falsely see iron as a threat and makes my body react to it? Every time I take supplements, within one week I get very ‘water retention and fatigued’. Then when I stop taking them, I feel better. I also have some tummy issues when I take supplements. My doctor always hates seeing my iron severely low. But I have to chose the lesser of the two evils and just deal with hypothyroid fatigue. Because iron makes it worse over time. Does this make any sense? Am I the only one?

I'm often deficient and often supplymented by drs but they keep making mistakes about when to stop and now I have to ask for ferritin test iron for my overload and saturated test ect. So I know how much iron to cut out in diet for how long. Why can't they get it right. I haven't been well so called immediate dr and she said I shouldn't be on iron as tests were normal 2 months ago. Another said opposite. Ones lookin at ferritin, the one who kept me on it. The other lookin at hemoglobin saying it's normal so stop.

My Iron serum is 35.5 μg/dL just above normal range Unsaturated Iron Binding Capacity (UIBC) is 235 μg/dL which is normal range my Total Iron Binding Capacity (TIBC) is 270 μg/dL which is in normal range My Transferrin Saturation Index is 13.12 % which is low I tested ferritin in different place 3 months before it and its 17.5 ng/ml the normal range is 10 - 291 so it above normal I don't understand why everything is in normal range but my Transferrin Saturation Index is low .. should I be taking iron?

Great question...Iron is not walking alone in the blood, but carried and transported by the plasma protein “Transferrin” which literally means “a protein for iron transportation”...Transferrin isn’t iron; Transferrin carries the iron. We refer to transferrin as the total iron binding capacity. But, it wouldn’t be wise for Transferrin to be fully saturated with iron, because we will have a rough time when we need to transport more iron, that’s why 1/3 “or 33%” of the transferrin carries iron (called Transferrin percent saturation which is normally around 33%) and the rest of the transferrin is not carrying anything (67% which is the unsaturated iron binding capacity or UIBC). Therefore, TIBC equals UIBC plus the serum iron. So let's say that serum iron is 100, TIBC is 300, then the UIBC is 200 and the transferrin %Saturation is 100/300= 33%...think about a sedan car that carries 5 passengers but leaves a space inside in case of an emergency...That's the transferrin carrying iron at a saturation of 33%. Hope it helps!

Normally, there is no iron floating alone in the serum, it's bound to the transferrin...Normally, the transferrin is carrying iron to 1/3 of its full capacity, that's why the normal %saturation is around 33%...Also, normal serum iron is about 100, normal TIBC (i.e. transferrin) is about 300, when you divide 100 by 300, you get the 30% of transferrin % saturation...Hope it helps!

Imagine if in an eprouvette you have a sample of blood, in that blood you have for example 6 molecules of transferrin, in a normal state only 2/6 (1/3) transferrins are saturated with iron, this is actually SERUM IRON, other transferrins 4/6 are free of iron but they are present in blood. All of 6 transferrins are the Serum TIBC. So, 1/3=Serum iron/serum TIBC x 100%= Transferrin saturation%(33%).

@@MedicosisPerfectionalis my ferritin is also 7. My transferrin is 373 and my % is 30 (last year was 11). My iron is normal at 113 (the highest is has ever been in life. Years ago it was so low, the test came back as unmeasurable). I know this comment is 2 years old, but hoping for a reply. My doctor said to take 325 mg of ferrous sulfate. I don't know. I was taking iron supplements last year when I had my last test, and my iron still only read 59, so I stopped. I've been eating better recently which accounts for my increased iron. But I'm not liking the potential side effects of ferrous sulfate. What are your thoughts?

Because the iron storage (ferritin) is low, so the body recognizes that and signals to the liver to increase transferrin production to try to catch every molecule of iron that we possibly can. That’s why Ferritin and Transferrin are always at opposite; if one is high, the other is low and vice versa.

As per the doctor we have been giving them iron supplements for 3 months but ferritin is not increasing . Health Goat Your ferritin is good. maybe too good. I have most of your symptoms(and many more). I shed 400-500 hairs everyday all over. eyebrows and eyelashes also. I was diagnosed with Telogen effluvium by my dermatologist. What is your B12 and vitamin D? I had B12 deficiency and got many shots. My ferritin is 60. I still take iron pills to bring it up to about 150. Do you have white ridged nails and white smooth coating on the tongue?

Should I be worried about transferrin saturation of 4%? TIBC 638 and transferrin 456. Iron is 25, H/H 9.3/34.1 and MCV 70. I did this test June 8th and the doctor never called be back with the results. I just requested this on Saturday as I have been having dizziness, chest pain, headaches, leg pain and always short of breath when doing an activity. They’ve been treating me for complex migraine and the medicine is not helping. Thank you.

Brittany J I saw a hematologist who said my results were bad. I have had 2 iron infusions already and will redo the iron studies in a month. The terrible headaches are gone and I am not out of breath like I use to.
