I'm a GI nurse; when we have a patient who is suspected of being lactose intolerant or having lactose malabsorption - someone having abdominal pain, cramping, diarrhea, bloating from gas, poor weight gain in younger kids - we can actually take a small piece of tissue called a biopsy from the 2nd part of the duodenum and test it for a lack of dissacaridase(s). This makes so much sense. As a kid I didn't have any issues with dairy, but we also had fresh milk with the cream on top. My symptoms started when we had to start buying store milk. I can have some dairy but not all and I think part of it is how it's processed. I can't drink full cream milk but can have full cream ice cream. I can have one brand of yoghurt but not another. Absolutely no soft cheeses. I tried A2 milk (meant to be good for lactose intolerance) and still had symptoms. I just keep my dairy intake to a minimum and keep away from milk.

This was a great breakdown of lactose intolerance. I was born without the ability to process lactose, including breast milk. I vomited up most of what I was fed. The doctor ended up putting me on soy (not too common in the early 1970s). As a kid I did eat ice cream, cheese, and have milk with my cereal, but had the symptoms you describe. We had to limit what and when I could consume dairy. As an adult I even tried Lact-Aid, but after a short time of being on it, I had the same symptoms as with regular dairy. These days I manage my symptoms pretty well, while still having the occasional dairy treat (home made ice cream!) and natural cheese doesn't usually bother me. One other problem I have with dairy is that sometimes it can make me pass out. I haven't heard of anyone else with this issue. As a kid it could either make me hyper, or sleepy; now as an adult I can't risk driving after having dairy as I almost had a wreck on the highway because I felt drugged. Any insight on this would be most welcome.

I'm one of those who can't handle ANY lactose. When I meet someone else who is lactose intolerant I ask them if eating cheese leaves a weird aftertaste in their mouth. Almost everyone answers yes. I'm that person who was lactose persistent and then became lactose intolerant. I drank a lot of milk in my childhood (up to 1 liter a day when I really wanted it because I really love it) and when I became a teenager, I stopped drinking it regularly for no particular reason. I began to experience the symptoms in high school, so I was afraid to drink milk, which made me drink it even less often. Now, in my 20s, just 150 mL of milk is enough to ruin my day. Luckily, if it's ice cream, cheese, or butter, it's okay.

I think I've been lactose intolerant since birth. According to my mother, the first time that she breastfed me was ... Unpleasant. I often vomited after eating and had diarrhea. I was small and thin. No one did any tests then (at least in our country), and purely from the experience of the medical staff, my mother was advised to feed me with special food. She then thought that something was not right specifically with her breast milk. Speaking of my first memory of a glass of milk, it was a stomach ache for an hour or two. I just lay in bed. This happened every time, so I just refused to drink milk. Thank God I was very, very stubborn. The most interesting thing is that I still ate things like ice cream. I also felt bad after it. BUT I THOUGHT EVERYONE FELT BAD AFTER ICE CREAM! It's true, I really thought so. It's just that no one talks about it. And I didn't talk about it. It was like a "payment" for delicious ice cream, so I thought...😔 At an older age, I switched to lactose-free milk, cheese, sour cream, and so on. It has become much easier. And only then I switched to lactose-free chocolate, lactose-free cookies and now I check everything. I want to say that a lot of pain has gone from my life, which I (for lack of an alternative all my life) perceived as the norm. Edit: Perhaps it was not lactose intolerance from birth, but there were some other problems that made me feel bad. As I wrote, no one did the tests then. Even so, I haven't been able to break down lactose since I have a memory of eating.

Have you ever visited a dairy farm and asked why calves are separated from their mothers? It's for the health and safety of the calf. Dairy calves are born with very few fat reserves which are quickly used up in cold weather, especially if it is also wet as it often is in spring and autumn. Calves can get scours very easily out on pasture with access to endless milk (seeing as dairy cows produce enough to feed 4 calves) which leads to dehydration and death very quickly. Out in a field that would be impossible to monitor so more calves would die. Cows can bully calves, cows in heat can attempt to mount calves and severely injure if not kill them, calves can spread disease by sucking on multiple cows. It's for the good health of the whole herd. I keep a few cows that I also milk for my own supply of delicious milk. Even I separate calves soon after birth because I've experienced the bullying of a calf and I've also had a calf rip it's mother's teats to shreds. I also appreciate having access to veal from bobby calves because it's one of the very few things that doesn't upset my rescue cats stomach.

That was particularly cool on how you went along the whole digestive tract with the cadaver, Justin! I'm one of the lactose persistent people and enjoy fermented dairy (cheese, yogurt, kefir, creme fraiche and the like). Would this have an effect on lactose tolerance and ability to digest it due to a change in lactose to lactic acid? Cheers, Justin
@theanatomylab

Thanks for the kind words! I appreciate it! To answer your question, possibly, but it’d be circumstantial and individual considering this happens naturally in the colon already by the microbiota. The microbiota changes based on meals, time of day, sleep, alcohol consumption, and plenty of other factors. If enough species that lacto-ferment lactose inhabit the gut in an individual who’s technically lactase non-persistent, it’s possible they’d never experience a single symptoms of lactose intolerance, which from a diagnostic perspective would mean they technically don’t have it.

@@theanatomylab Good points, which align with my thinking. Individuality and context have a pivotal role. Like you said in the video, caring for our gut microbiota may be beneficial according to the latest research. It's interesting how you phrased it with that lactase non-persistent people who may have sufficient lacto-fermentation in the gut, which ironically wouldn't confirm in a test that they don't provide any or sufficient lactase (or even vice-versa with poor gut health yet high lactase production that it wouldn't necessarily show health issues with dairy, which is then something else).

A major limiting factor for animals is protein. When people evolved to be lactose tolerant, it opened up a huge, easy supply of protein. Humans no longer needed to kill animals to obtain protein, they could just continue to get protein from animals. In addition, when you kill an animal for protein, you had to consume it before it goes off. The larger he animal, the larger the problem. Collecting fresh milk overcomes the problem. If you can preserve the protein in milk, the more the benefit. Milk preserving techniques include butter and cheese.

I hope I never lose my lactase persistence. And you're telling me the best way known to help that is to keep enjoying dairy products with abandon.. um, ok! Haha. We discovered that my husband is now lactose intolerant about two years ago. I would say, in retrospect, that the decline probably happened over about 10 years. It was only then that we discussed the symptoms with the doctor because it really was bad. Not just for him but everyone around too! It had never occurred to us that lactose was the problem since for the first 30ish years of his life he consumed it with no problems. We initially thought the doctor was barking up the wrong tree when he suggested eliminating lactose for a while as an experiment, but we did it anyway. Yeah, we realised within just a few days to a week that we were wrong and it definitely was lactose intolerance. On the plus side, things are much better now with a combination of using lactose free alternatives, lactase pills and sometimes he just goes without. I have learned a bit through this process about lactase persistence and how some people have it and don't and some of the genetic stuff behind it all, it was really great in this video to be able to connect all that with what is happening in the digestive system at each point. It's never that easy. Did they talk about raw dairy vs.pasteurized diary? Once diary is pasteurized it becomes a completely different entity. It's not even milk anymore. Everything that is nutritious is destroyed when pasteurized. As a matter of fact, once pasteurized it actually becomes highly toxic and indigestible. That's why so many people cannot digest lactose now because it's not even a food anymore once pasteurized. All this can be looked up and proven. Don't be so quick to believe everything you hear or read. Do your own deep research. Ask questions.

That’s exactly what happened to me lol. Used to have all the milk I wanted. Then I lived alone and never bought milk. And now a medium blizzard is my limit 😂

I'm 37 and I'm in the minority that has lactose persistence. Which is good for me because I depend heavily on dairy and whey protein sources.

Can you explain what happens inside the stomach if you get an abdominal tear or scarring from surgeries? This happened to Lisa Marie Presley and I was just curious on how that can come about, a death from scar tissue or Bariatric surgeries. Love your videos! this one is most interesting to me as I have problems consuming lactose.

Try to explain to people that you have a milk allergy. Lactose intolerance is not the only issue. I have been unable to fully process casien since birth.

To answer the question: NO YOU SHOULD NEVER CONSUME DAIRY, under any circumstances. Its not good for you, no matter what any media/ad campaign told you…

Guess I'm one of the lucky ones, because I've never had issues drinking or consuming milk products. Besides, they're super delicious and filling for breakfast.

Humans: the only species to drink milk from another species while simultaneously doing it past infancy. Also humans: why am I lactose intolerant?

Not for nuthin... But if other animals had the ability to milk other animals, or if other animals would allow different species to suckle on their teats, then it may be a different story

As shown by the video, not all humans. Europeans, Asians, Africans, every culture that has cows (and other milking animals) available, have developed lasting lactose tolerance. Evolution. With your logic, everything humans do with their food that other species don't do should sicken us all: cooking, preserving, drying, smoking, fermenting, distilling, etc. Of course, ultra-processing does sicken us all, but in comparison to any animal, making tamales, tofu or olive oil is ultra processing.

You obviously have never seen a cat in a milking barn. There are lots of animals that will drink milk if or when they can get access to it. Only difference is they can’t get to the store or grab it out of the fridge like a human can.

Plenty of animals will eat milk products if you give it to them.

“Hope for the best” so funny! Story of my GI life. I feel so much better now that I’m dairy and gluten free. What a fantastic video! Very good info! It’s so helpful to see the structures of the GI tract to understand what’s happening. I appreciate the biochemistry info broken down so I can understand it lol

Possibly the most overlooked underlying problem here is still milk allergy. Not lactose intolerance. Question, if the sugars that aren't digestible in the small intestine, but are digestible by the bacteria in the large intestine are the problem, why don't we have the same problem with sucralose?

100 years ago, the per capita consumption of cheese was below 5 pounds a year. Today, the per capita consumption is over 35 pounds a year of cheese. No wonder we are facing a diabetes an obesity epidemic.
