How did Brian Johnson do it all those years? He knows what he's doing, that's how. Specifically he knows how to sound that way without hurting himself. It's not necessary to hurt himself to make those sounds. He himself may not be able to articulate what he does, but he knows within himself how to do it when he performs.

Clifton Isaacs is there a way James Hetfield could have screamed the way he did live in 85, 86, 87 and 88 without doing the damage he did? Like, is it possible to scream that intensely and still sound that way, have the same vocal tone, same brutal grit, etc only WITHOUT hurting yourself? Or would you need to alter the way you sound a little when putting those techniques to use?
