One thing to be aware of is: Just because the circuit you're working on is locked out and considered de-energized, still treat every wire as if it were "hot". The hardest I've ever been bit was off of a shared neutral (referred to as a multi-wire branch circuit in the NFPA70). While the receptacle circuit I was working on was dead, it's neutral was spliced into another circuit, carrying the load of an industrial shop compressor. Essentially, I created a series connection between my two hands when I went to re-splice the neutrals. Rookie mistake, but something that could happen to anyone.

I worked on punch presses for a few years. I've seen arms crushed, I've seen women scalped, I've seen fingers cut off. All because they didn't lock out. I seen a lady get her fingers crushed in a steel roller because she thought it was a good idea to reach around a guard when her machine wasn't powered down and obviously not locked out. It's no joke. Everyone gets comfortable, too comfortable and that one time the machine doesn't do what they expect... It's too late. It's not just your life, it's your families as well. I've seen men hurt so bad they can never work again. Now they collect a pitiful government check for disability. Went from making $150k a year to like $30k.

Yeah most people fail to use the stop blocks. I had to fight my last employer because the machine didn't have them, Just a chain attached to the electrical lock out. There's so much you need to know and so much ignorance surrounding punch presses.

My father is a master electrician, and his coworker died because an employee cut the lock on a breaker because he couldn't do his job wolith the power off..... Manager was screaming at the employee because he wasn't meeting quota, it is very important to realize why things are the way they are!

Ah, yeah. Something like this happened were I did my electrician training. I was tought to always bridge all the conductors between themselves and earth after the breaker and using a plug to do it again outside the breaker box if posible, so if anyone is mad enough to remove the lock and unbridge the wires, my plug that is shorted internaly may, MAY save me.

If was that worker, i would just go to managers boss and tell them this guy is shouting at me for not doing the job, witch i dont have electricity to do, so is there anything else i can do in the meantime or how we resolve these issues? If boss is asshole, go to owner and if same apply then change the job. Alternatively just sweep the floor so you are doing something (they cant claim you are not doing anything) until you got power back.

A fun little story came to mind when I saw this. Years ago, a friend worked in a large engineering shop, and he told me about a member of staff who got threatened with being fired because he religiously tag out the large caged CNC mill he ran whenever he had to enter the caged off area (To swap the workpieces, replace worn tooling, or just clean up swarf inside the cage). It seems a bean counter had noticed him locking out the machine over and over again throughout the day, and realised that doing so took about an extra 2 minutes each time, and that added up to a couple hours a week. That guy told management. They decided that it was an unnecessary practice, and that he needed to stop doing it. He refused, so they tried to fire him by deciding that locking out the machine was an electricians job, and as he wasn't an electrician, it was gross misconduct on safety grounds........ The union had fun with that one. Apparently, in the meantime, the rest of the workforce (Who were annoyed by the treatment of a colleague) decided to implement this new ruling FULLY, so started calling the onsite electrician to come and lock out machines every time they'd usually just have isolated them by themselves. Their pair of onside electricians ended up just running about frantically after machinists all day, so couldn't get any of their usual work done. This went on for over a week, so the union got involved there too. He told me that managements efforts to save about £20 a week in lost productivity by the original guy, ended up costing thousands in lost productivity, and it seems that because the union deemed even the original workload placed on the electricians to be excessive, they had to hire an extra guy for that department to. :D

That's exactly what i wanted to comment. I work with machines and we usually lock out the mains switch. But if you have a cabinet powering other cabinets or lines, this one may be a perfect solution. Nice to find more knowledge :)

Lock out tag out is soo important. I can't count the number of times I've had a truck tagged out for whatever reason, and some manager comes along and removes the tags and issues truck to a driver wo contacting the tech who tagged it. The only person who should ever remove a LOTO is the person who placed the LOTO.

yeah, back in the day i had supervisors remove an obviously locked out vehicle and give it to me and I would take it too. These days If I see anyone even thinking of doing that I fire them on the spot.

I work at a power plant in Germany, and we use this stuff literally everywhere. At some point it becomes silly, but it's better to constantly run a LoTo system on every part of your equipment than to risk people's health.

At times it may almost seem silly, but if you break the system of loto at one point, it becomes untrustworthy, and accidents will be far more frequent. I do remember a time when we removed equipment and it still had locks hanging on it. It looked kind of odd, sitting there removed from the production line with no energy sources whatsoever, but everyone still had to sign off, remove locks, and close out paperwork, lest the process become untrustworthy.

Some factories have group lockout boxes with all kinds of paperwork it takes like 20 minutes to turn a break around for 3 minutes to test a circuit. As much of a pain in the butt as it is I'm going home to my family . I love the lotto

20 minutes that’s a quick lockout. I have equipment that takes close to 100 locks to make it 100% safe. We send a crew of 5 guys to lock the line out.

Worked on big industrial steel presses, robots, laser welders, CNC machines, you name it as an industrial maintenance tech. L.O.T.O. wasn’t just a good idea, it was beat into my brain! Once we lost almost half a shift of production because we couldn’t locate a guy to take his lock off. We had only started contacting the right people to cut it when he realized he left and came back! We carried our locks on our tool belts and were expected to have them with us everywhere

I found a $50 set of LOTO circuit breaker switch locks at a pawn shop and they sold it to me for $10. I had to tell them what they were, and even after seeing the price, they gave me such a great deal. I was so happy

My friend nearly lost his hand to this sort of event He used to work at a plastics plant and all the machines were supposed to be locked up between uses. He goes to feed in the first roll of the day and up comes the saw to start cutting the plastic because someone turned the machine on. He instinctively pulled his hand back (had a "feeling" and jerked it back) just in time and still ended up with a nasty cut and a lawsuit since the safety guard was also missing on that machine, but his hand was jacked up for a good few months

I know a buddy who was just clearing out a jam in a feedline auger on a farm who nearly lost all his fingers due to someone flipping on the breaker while he was working on it. Definitely get some of these if youre doing any sort of work with equipment or electricity, it's worth the cost.

Or you run it and the hydro leaks and it leaks down when the boom is up and you crush something or someone ur at fault. Most the time things don't get locked out cus they have on site mechanics and they are there with in ten minutes.

I was a maintenance supervisor at a large manufacturing plant. I was also the LOTO trainer for the maint department and the production supervisors who did tool changes etc for thier production lines. 99% of the accidents that we did have happened when the person was not using proper LOTO procedures. LOTO work. Most machines will not hurt you if they are powered off and LOTO so they can not come back on. The few accidents that I saw they were NOT due to non use of LOTO had to do with stored energy like air pressure, hydraulic pressure or something lifted on a cylinder that was not bled off.

LOTO is so important in most workplaces now that even tho I only work in retail in the produce department one of our "fast tracks" (it's the computer stuff we do to tell us how to do our job and show us safety stuff) is about the LOTO system, everyone needs to know it even if only the managers can actually do anything about it

Fun fact the object he is holding in the beginning of the video is called a "multiple hasp". It allows multiple locks to be used on a single switch and is used when multiple people are working on a piece of equipment. Each person will use their own lock and it makes sure that noone else can start up the equipment while someone is working on it.

I remember reading about a case where one of these tags was fitted with multiple locks on some huge industrial machine, and as one of the people involved couldn't test his work until others had finished theirs, he left his lock in place while he went off to do other work on site. Everyone else finished earlier than expected and wanted to test the equipment, but couldn't because that one final lock was in place. The guy had told everyone his work was done, and they assumed that after doing the other stuff on site he'd probably just gone home. No one thought to try phoning him, and eventually, some bright spark just took a pair of bolt cutters to his lock !!! Apparently, it's a HORRENDOUSLY big deal if you remove someone else's safety lockout here in the UK. Someone reported the guy with the bolt cutters, and his company got investigated and fined by the Health and Safety Executive (A government agency for workplace safety). So if your boss ever tells you to "Just cut the damn lock off", make sure that at BARE MINIMUM, you've got it in writing, and that it's worded as a direct order from someone in your management.

At a auto manufacturer I worked at, to remove someone else's lock required security (they had keys to every lock) his team lead and his Supervisor. All three had to be there and sign off on the check list before the lock could be removed. If anyone removed a lock out lock or tag without following the procedure they was immediately fired.

@@johnhurtme Please don’t underestimate the danger of electricity. While it’s somewhat unlikely that 120v will be fatal, it is still most certainly possible. What actually makes electric shock deadly is the amount of current actually flowing, the path it takes through the body, the frequency, and to some degree, the length of time being shocked. Generally, current through the chest of 100 milliamps or more is fairly likely to be fatal. Several factors affect the outcome, such as moisture in the skin, where contact is made, surface area of contact, and other things. It is known that as little as 75 volts could be fatal, and maybe even less voltage, under worst case scenario.

@@nicholasittzes7224 we use a lot of square D and Eaton panel boards. The lockouts for most are just a stamped sheet metal bracket that snaps onto the breaker and is secured by the dead front being installed. They are kinda nifty.

This is something that has been around since the 1950s at the least, and back then it was made by General Electric, and was an option to be used on circuit breaker panels. Most of the military facilities (to include ships) that were built around that time frame, had those type of panels where most of the circuit breaker levers were inbetween two pieces of metal each with a hole in them that you could drop a 1/4" diameter pin into and then place a lock into the bottom of the pin securing it in place. Or you could just place a lock across that particular circuit breaker if you had one that would fit correctly. While the pins were mostly lost at the facilities I worked at in the military, I did however have locks that I could put on those breakers, and often did.

an old welder told a story of when him and a whole crew were welding doing some repairs on a food plant line. They had locked out everything at the line and in the mechanical room, but had been failed to be notified of an override in the managers office. He said some young guy walked into the office, noticed the line wasn't on (probably didn't even notice or know about the 5-6 welders going simultaneously), and turned the line on. thankfully he said no one was injured but gave me the advice to always check everywhere possible for places you need to lockout.

I never seen one for home breaker. I've only seen loto on full panels, valves, and key systems that can only be inserted or removed if certain conditions are made.

Same breakers are used quite heavily in commercial buildings too. Just a lot more of them. These work on double pole breakers too since the levers are tied together.

@@drrenard1277 I've seen those too. A restaurant I used to work at had a panel for the kitchen where all the breakers had a built in loto, the rest of the building was all standard 15/20 amps just like residential. Very old building too, so possibly grandfathered and not current building code.

We use them on drilling rigs and another type designed specifically for the leavers on the driller’s console and we had chain that was welded to the floor and would be hooked to the brake handle and locked back to itself and if that wasn’t enough we had a big sign that was secured to the console that said DO NOT OPERATE

Where I work , it's actually called LOTOTO Lock out, tag out, try out. Always verify the equipment has been de-energized by trying to operate it. It's possible to accidentally lock out the wrong energy source. So always verify. 👍

I worked at a very large food manufacturing facility. We had Anhydrous Amonia for cooling purposes, intricate conveyor systems, heat sealing, and fine cutting machines that unfortunately in the past people got severely hurt and one got crushed to death. The department I was in we went through OSHA 40, Hazwooper and a external company certified us for Confined Space entry and cleaning. When we did anything with LOTO, there was a extensive plan in place. When we where going to do a job the communication between US and Management and Team Leads was defined. When possible we would work around the production schedules so that the bottom line was taken care of. If a machine went down we instructed the employee working that during the work being done no lock is to be cut and no one can over ride us. The plus to this is we always had 2 different Maintenance workers put a lock on each. We had laminated ID tags attached that had our picture with a number to call if something failed. With also locking Air out also kept the ability to run the machinery down to a minimum. During my years there we didn’t have injuries because the Company did care more for safety than oh shit we lost 1.5 hours of production and a life too

First time I’ve heard some one talk about lockout tagout on KZitem. I work in the power plant industry and this is a huge deal. It’s one of our life critical safety rules. Spread the word. 👍🏼

Ours were yellow and red stiped. I LOTO'd our shop's gas welder torch to itself because someone had stolen its flashback arrestors. I came in the next morning to find my boss had cut the lock off and used the torch to cut something while we were signing in, drinking morning coffee. I told him this is not okay and I am not okay. He asked why I was so upset. I didn't say anything, beckoned him to follow me to the shop's back door. In behind our shop was hundreds of thousands of pounds of liquid fuels (gasoline, fuel oil, propane, etc) that were shipped in and out by the neighbouring rail company, probably ten metres away. His face went white, and I walked him into the shop office. There I pulled out the workplace safety book, pointed right at the line that said "any employee of any rank who removes a lockout tag of any equipment or tool for any reason will be fired with cause and forfeit any severance owed in their contract." Inside, the guys in the office played it off like it didn't matter because nothing bad happened, the other shop dudes tried to make excuses for him (or blame me). So I grabbed a post-it note off the boss' desk and wrote "I quit. This is my two week's notice." This was during COVID, so I got a job immediately at another shop. I should have reported it to the government, but for some reason I didn't. Regardless, in the weeks that followed, the other guys realized how close he came to blowing up the town, and they all quit with the exception of the bossman's favourite lackey. Some of you will say that the train cars should be equipped to not explode if they're full of fuel. I'd note that (1.) That's not the main point, the acetylene torch would have killed us all anyways with or without the fuel yard next door. (2.) I don't have that much faith in those rusty old cylinders. Just because you "can" smoke ten metres from the fuel yard doesn't mean that it's a good idea to set off a bomb ten metres from the yard.

Don't use it for locking automatic fuses in the on position though, that will lead to an electrical fire instead of a tripped fuse.

There is one more T.O. Try Out. Make sure your locks were applied properly and go up to the device and try it. I have seen operators "Put all the locks on the machine for the maintenance team" but electrician tested the valve manually the air was still on! Luckily no one was around the 8" diameter cylinder it was moving. The team had some kind words for the operator... don't put your safety in the hands another double check it yourself.

One time i was up in a suspended ceiling and my brother in law cut a bunch of wires. Well breaker 13 got turned on. It art and zapped the hell out of us. I could not see because it was pitch black and the brit light white flash. I broke the ceiling and almost fell to the floor. It notched his side cutters to a 14g wire.

In the Navy we just use paperwork that says that something was tagged out. Whether it actually was or not is a different story.

In my first year as an apprentice electrician. I had used one to lock out some rooms I was changing and update receptacles in. Little did I know that one of the scab labor drywallers cut the lock off to turn it back on so they could plug in some tools. I was lucky the shock wasn’t that bad

Not gonna mention the company I work for, but, there was a failed lockout tag out practice and somehow it was removed when a maintenance worker was working on a powered belt and he ended up losing his arm or it was degloved. This shot is no joke and if you’re not the one who put the lockout on it don’t take it off.

not an electrician, but if I happen to work on electric stuff it's always a good idea to just short some wires out to be safe like a "lockout"plug with all 3 wires twisted together, might trip the GFCI and you may end up in the dark, but you stay safe is that the legal solution? no, but it works and if I'm not sure if I got the right breaker, just cut a cable or put the lockout plug in, the sparks will tell you if you where right or not.

One of my husband's best friends was killed because a flunky from the power company came and turned the power on without checking if someone was in the hole and tore off the tags. The pit was full of water it was after a hurricane he was down there doing work at half the cost of the big companies. They had been trying to buy him out for years and ended up killing him

Damn, this would've given me so much more peace of mind when I was swapping a switch the other day. The breaker box is in a rather difficult roommate's bedroom. Definitely wouldn't put it past him to flick that shit while I was in a switch box.

Locking out the electricity is just one part of controlling energy sources when working on a piece of equipment. I've never seen the circuit breaker lock out though. I've never worked on anything that didn't have it's own disconnect.

Worked with and cleaned a lot of heavy production machinery. In meetings we were taught to always lock out and tag before doing ANYTHING. Then we proceeded (as directed) to NEVER lock out or tag for any reason.

I first heard about LOTO following the bumblebee tuna death. I’m in pest control and I’ve never been in a situation where I needed to LOTO but I have the padlock in my truck in case I ever need it.

I don't work in a directly relevant field (I'm in healthcare, just a curious safety nerd) but boy howdy, the healthcare field could use more systems like this. As in physical, not just digital semi-equivalents. Also has me brainstorming how I can implement variations on this theme in my own life to prevent my ADHD ass from forgetting things that I don't already have physical systems in place for 😂

Master lock Let's use bottom of the keyway tension A little click on pin 1, pin 2 is set, no movement on 3, 4 is set, 5 is binding Back to 1, 1 is set, 3 is set aaaaaand it's open -LPL But yeah these things safe lives, and should be mandatory for every company that has a somewhat heavy machine. Even if it's as simple as industrial belt sander. Use these lockout tabs.

Used to work in a machine shop where lockout tag out was a HUGE deal.. In fact my best friend years ago was electrocuted in a Ford dealership sign because they didn't take it seriously. (He was changing the sign bulbs) They were assured everything was safe but his boss neglected to double-check. David went up, the Oklahoma wind picked up and and his hand barely brushed a lead. He was dead before he collapsed to the floor.. The sad part about it is every attorney in Oklahoma knew what kind of battle it was going to be against Ford in a lawsuit, so his family couldn't get anyone to take the case. Mind you there was video evidence and admitted fault from at least two parties responsible including Ford's Manager.. had to hire a guy out of Dallas and Ford's first counter offer was 10 grand on a negligent death. But Ford isn't where they're at because they're dumb, Every time they went to court they made his mom relive the accident over the course of a couple years. She said it was too hard on her so she settled. Not sure what she ended up with but every attorney I talked to said that should have been a multi-million dollar lawsuit. Rest in peace David! That lockout tag out stuff is no joke!

While I totally get why LOTO works, it's almost sad. You're turning 1 layer into 3. You should be able to expect that if it's turned off, it's for a reason, don,t turn it on. If someone puts that device on not locked, doubly so. The fact that the lock is even necessary seems asinine yet I can see in the comments how many times people cut the lock without talking to the person who put the lock there. People can be really stupid, huh?

As a refrigeration mechanic, these are a must.. no offense but people and their higher ups are really stupid and disconnects and central breakers especially need to be locked. If anyone tampers with my lock and I get hit with 3 phase of 480 or 208 230.. rest assured it does not go well for them.

We use LOTOTO Lock out, tag out, try out. Sometimes this get wired incorrectly or there is stored energy. Stay safe out there.

I Work around equipment that that 3-phase forgot to lockout tagout a piece of equipment when I was clean it Got yelled at by my boss almost Lost my job. Lockout tagout is not a joke it does save lives

I was sandblasting inside some slurry tanks for an iron ore mining outfit in northern MN. Thing looked like a huge blender that would easily turn 3 elephants into a milkshake no problem. Yes, OSHA OMSHA HAZMAT &CBRN aware. Scary shit when you think about it. I like that breaker lockout, too (I use one to kill the power to the stove, my wife can't cook for shit 🤣.)

I’m an electrician in the uk, I use these everyday, especially when I have got to work on or install large equipment and I have to get inside the machine, we also have multiple points of isolation on the same item of equipment.
