I laughed for a second there when you said it can be used to describe people as earthy, and of course it depends on the context. But if not, it would sound a bit if not very rude for a Filipino, especially to an elderly. It would mean that they are close to earth, maybe smell like earth or dirt which implies that they are so old they are starting to smell like dirt and be in the dirt, also referring to a circle of life quote saying "we are all going to back to the dirt". I accidentally said it to an elderly when I was a kid, couldn't remember who and I can feel the people in the room staring into my soul, and gave me an earful afterwards lol.
