I strongly admire your professionalism! As a client with long-term low back pain issues (tipped pelvis, falls, etc...) having a pelvic area pillow while prone and a bolster when supine (the larger the better) on the table, I find that I can relax "into the massage table" MUCH deeper. (Less automatic guarding of those painful areas.) Offer some different size pillows, even those bean kinds. I also sleep with a firm pillow between/ under my knees at night for supporting my lower back. For me, my glutes are usually tighter than a drum (ouch!) and also getting up s.l.o.w.l.y. off of the table (even sitting a bit before standing up) helps my newly relaxed lower back muscles stay that way a bit longer. When I hear, "Take your time getting up" - to me, that means taking a pain-free nap first!

Loving your videos and as someone with LBP issues I love your gentle approach! In school we are also focusing on abdominal muscles that may be shortened and muscles of respiration for LBP. I've personally been noticing a change in my back with that work too.

As a massagetherapist I love to watch your videos to diversify my pallet. Some techniques are new and some remind me of techniques I have learned in school, but left of my treatments a while. Quite a few of my clients have low back problems which I tend to with similar techniques plus some stretching from therapeutic Thai Yoga Massage. Once the muscles are warm, the stretches can give so much 'room' in that low back area. But anyway, I just wanted to say thanks!

Awesome demonstration. Love the use of "opposing traction". With clients who come in for low back work I always say something like "and, respectfully, could you please undress completely? I need access to your hips (then I do a little *tap tap* on my own hips)". Often times the clients are like "Yes. A million times yes". For the ones that dont, its no biggie. Here are my tips and tricks to add on: I got this from your Sciatica video actually: when working prone on the hams I go up past the glutes and into the low back "act globally" ;-) This area is usually draped but by this point the client has seen me a couple of times and I explain more or less what im gonna do. Quite recently, I started doing a similar drag while the client is supine: I start on the quads, take the drag past up and around the hip and finish the stroke in their low back, by the spine. I then "scoop" the low back, and bring the hand back down to the quads gently medially rotating the hip/leg. I got this idea from receiving work earlier this month, the therapist went up to my hip and back down and I thought "what would happen if I took the stroke beyond and into the low back?" It feels good to me, doing it, and to be able to bring some movement, in a different way, to that area of the body, connecting it with everything else. Clients seem to appreciate it as well. Video Suggestion: Stretching. I love stretching people. However, I tend to ask the client to help with with draping and I would like to move away from that. I want a client who's completely undressed to feel covered, safe, but well stretched all at the same time. TIA!

Oh dang, that supine move that reaches the low back sounds amazing! Like you said, some of these approaches are best done with clients who are familiar with how you work, and with whom you have a strong base of communication. Kudos on your comprehensive work! As for stretching on the table, I just plain got tired of wrestling with the drape and started doing most stretching with clothed clients prior to the session proper: kzitem.info/news/bejne/uWydzn6Bqnlmm6A Not a perfect solution, but it's versatile, and I greatly prefer it to all the diaper-draping and whatnot. I have just been glued to your video's for the last hour and find them really interesting. I am taking a sports massage course here in the UK at the moment. You have a great way of explaining and demonstrating. In this vid, your reference to the Glute med / min area is especially interesting. Great content, as always. I was wondering what your thoughts are regarding at-home massage chairs. These machines are incredibly expensive, but I can see why people with chronic back and shoulder pain might see them as good investments. Would you recommend them to a client with persistent pain, or are people better off relying solely on a professional?

Ha! I do my best to keep my clients' comfort in mind, so all of these contacts should be well within their tolerance, and should feel broad enough to not be pokey or pinchy. If you're working on someone and they find it painful, think broad and slow!

I really appreciate this video, I have a lot of clients who come to me with low back pain and it is tempting to hyper focus on that area, but I too also work the gluteus medius area and hamstrings etc in order to loosen the tension in that whole area. A less invasive way as well to work on hip rotators is using NRT, neural reset therapy, as it's very quick and super effective. I like the idea of the large bolster and the pillow under the abdomen, so thank you for lending those ideas!

Massage Sloth no problem, I've enjoyed watching your channel for some time now and always learn something insightful. I will be going to North Carolina mid next month to learn upper body NRT. I had a client today whose QL and hips were bothering her. She scheduled an hour session with me and I started out with NRT on the glutes, hip rotators, hip abductors adductor and QL. she was amazed that her problems were completely gone within 15 minutes :) then she was able to enjoy a nice 45 min relaxation.

I like to give them stretching instructions that help them stretch the psoas muscles. And if it isn't psoas muslces, there are stretches for the pelvic region that can overlap that previous stretch. To sort of cover all bases.

My biggest tip I use is after loosening up the low back, hips, and glutes some. I like to stand off near the head of the table and traction pushing onto the pelvis while having the client take a few deep breaths. Most of my clients tell me how amazing that feels and their back pain seems to let up a bit. The quadratus seems to release the best for me this way. I love some of the other techniques you are using and plan to start using some of them as well. Keep up the good work.

great info on the big bolster, i just purchased one. these techniques are part of my myofascial aspect to massage. What do you think of ASTM tools to break up that deep connective tissue that likes to stick to the ribs of clients... Also Moving Cupping, no bruising included unless the tissue is just stuck...?

Moving cupping sounds good as long as it's well-tolerated, but I'm leery of any sort of scraping in the area of QL and lower ribs. My intuition says that it'd be more likely to promote sensitization rather than resolve any fascial issue that might be happening locally. I think scraping might be better applied in areas of high tone that are harder to irritate, such as the posterior and lateral pelvis, and even areas of only tangential connection to low back issues like ischial tuberosity region and anterior thigh. Basically, if you can get the hip muscles to release their death grip and make standing feel easier and more comfortable, I expect for QL to get less referred pain and to engage in less bracing. None of this is achieved by going after QL directly (though I do like to do some nice steamrolling in the area), which is why I expect these treatments to result in less reflexive tightening and spasm.

I do my best to dissuade people from coming in with new and acute pain/spasm. It's not that I think I'm likely to make things worse, but I expect it to be an exercise in frustration where it's difficult to make contact near the area, and the nervous system isn't especially receptive to my efforts to poke around an area that is clearly stating its preferences about being poked. Positioning can be difficult, getting on and off the table can be difficult, and it's all time that would likely be better spent in a recliner swapping out hot and cold packs. That said, let's say we're on day two of an angry low back/SI joint situation. The spasm has calmed some, and the client is more mobile. This is a time when I think it's possible to make headway. I position the client as comfortably as possible (e.g., big bolster while supine, side-lying with lots of pillows), and then do lots of long slow contacts to the QL region, lower ribcage, all around the hips, TFL, maybe even some iliacus and psoas. If we reach a point where their sensitivity has dropped a fair amount, I might ask them to do some gentle flexion and extension of the lumbar spine, just by tucking and untucking their tailbone as I work. It would all be based on communication, intuition, and play. If you stay within their current touch tolerance and take your time, they can get up feeling, say, 25% better, which can be a big relief. Is it possible to make things worse? It's unlikely in the above scenario, but I could easily see a spasm-prone back/pelvis going into full-on lockdown if someone decided to really dig in during that time of high alert. So, to sum up: If the client is ambulatory and able to walk into your treatment room on their own recognizance, then you can probably get some stuff done. But if they're calling you from home because they can barely get off their couch, ask that they come in when the situation has calmed down. If they say it's an emergency, refer them to an urgent care or emergency room for medical care. Good question!

I noticed that your draping is quite loose. I do that also, but my massage manager told me it makes the client feel weird. If you go under the draping. She showed me how to do a tighter "dipering" method. But that means lifting heavy legs. I'm not that big at 5'5. Whereas she is shaped like an ox

I avoid too much tucking because I find that it can actually contribute to the feeling of exposure/drafts. If the sheet is tucked under the hip, for instance, any movement of the body will create a "tent" phenomenon. On top of that, quick tucks tend to come undone easily, especially if you're creating big changes in the shape of the body (which I always recommend), so it requires too much maintenance for me. The re-tucking and fiddling can, in my experience, feel invasive. By keeping the drape low and communicating my intentions, I find that I can strike a good balance between keeping the skin covered and still working on large areas of the body. Some clients may prefer extensive tucking, especially ones who are used to it, so it's worth having a conversation about. And of course, if you work for someone else, following their rules seems like a good idea! As far as diaper draping goes... I reserve that for times when I'm going to be stretching the legs. I find the process uncomfortable as a client. That said, if you need to do it, you could ask your clients to assist with lifting their leg to preserve your own back.

For a data point, as a client I have never gone to anybody who did not drape loosely, and I would find it very odd/potentially unpleasant to have the massage therapist tuck it in around my body. I would imagine it would depend what people are used to though.

Dane Hrovitnir I agree, I have been told by many people the same thing. One gentleman told me he felt a draft as the therapist lifted his leg to firmly drape. And as a result, couldn't relax during massage. Because he was afraid of it happening again!

the drape is a clear boundary. it is better to move it to expose what you need to work on, so the client knows exactly where your touch is limited to

I choose to travel a limited distance under the drape as a compromise between full coverage with my contact and limiting client exposure. By keeping the drape low while doing so, and by being mindful of the direction of my techniques, I'm able to do so in a way that doesn't seem to feel invasive. I think that there are pros and cons to both approaches, and that communication can mitigate most cons.

I feel the same in regards to extending slightly into the hips when working the LB! it drives me crazy when MT stop too soon in the lower back, it just does not feel complete! Love the 360 techniques! (which means working the problem area from different aspects) I am very curious as to where you practice massage?! I really love that you use myofacial release prospective with every client. I would like to use it more in my sessions, however, I seem to get those clients that want "deep pressure" and not so much "deep tissue". How do you work with those clients?

I've got a video on how to deliver that deep pressure: kzitem.info/news/bejne/03imqXqrrGqif20 ... BUT, I want to emphasize that you should only offer the kind of contact that you're comfortable with. For clients who want more pressure than you're willing to deliver, you can start by asking them to give your method a try (e.g., "Instead of deep pressure, I work slowly and give the muscles time to make a change. Is that something you'd be willing to try?"), but in the end, there will be some clients who will want to go elsewhere. I say embrace that, because the less time you spend working on clients who are a poor fit, the more time you can spend finding YOUR clients. I recommend doing a lot to front-load potential relationships with information about who you are and what you do-write blog posts about your massage philosophy, answer frequently asked questions on your Facebook business page, and post videos of yourself in action. You'll end up seeing complete strangers who are already huge fans of you and your style.

You'll do great! If it feels comfortable for him and for you, mash those muscles. If you'd like further guidance on routines, check out "The Massage Book" by George Downing. It's from the 70s, it's by hippies for hippies, and it's a great introduction to how to apply conscientious contact to a partner.

For my low back pain clients, my favorite exercise to recommend on that first visit is the "cat cow" pose from yoga. I'll thrown a sheet on the floor and have them join me down there so that I can demonstrate gentle rotation of the pelvis, making sure they're not overdoing it. In future sessions I'll usually give them a lunge stretch (one knee on a pillow, other foot in front, steadying themselves on a wall), especially if it seems like the hip flexors are involved. Another excellent one is bridge pose. Actually, if I can get my clients doing some gentle beginner's yoga, that's my ideal :)

Look for a "9 inch round bolster" on Amazon or Massage Warehouse. Warning: It's reeaal big. I think they have ones with 8" diameter, and that might be even better.

Not if you ask me. Pain is a part of living, and my goal as a massage therapist is to act as a pain-relieving and body-integrating stimulus. Between massage, exercise, rest, and recreation, I think people can live more comfortable lives. If someone tells me they can fix part of the human condition, it makes me suspicious.

What do you mean by "working with upper hips?" The hip is a bone. "Upper hip" literally means the upper part of the bone hip. I'm guessing you're not working with the hip bone. So are you talking about gluteal muscles? Why not say "gluteal region/muscles" then?

I just use "the hips" as everything surrounding the hip joints, just like "the shoulders" can mean a fairly broad region. I find that it's more accessible to clients, and I try to use both accessible and technical language in my videos.
